KSNN VÀ MỘT SỐ Y.TỐ A.HƯỞNG TỚI KSNN CỦA NVYT TẠI TTYT VÀ CÁC TYT THUỘC TTYT QUẬN GÒ VẤP, TPHCM NĂM 2022 HÀ NỘI, 2021 BỘ GIÁO DỤC VÀ ĐÀO TẠO - BỘ Y TẾ TRƯỜNG ĐẠI HỌC Y TẾ CƠNG CỘNG TRƯƠNG MINH BÌNH KSNN VÀ MỘT SỐ Y.TỐ A.HƯỞNG TỚI KSNN CỦA NVYT TẠI TTYT VÀ CÁC TYT THUỘC TTYT QUẬN GÒ VẤP, TPHCM NĂM 2022 ĐỀ CƯƠNG LUẬN VĂN THẠC SĨ Y TẾ CÔNG CỘNG MÃ SỐ CHUYÊN NGÀNH: 8720701 NGƯỜI HƯỚNG DẪN KHOA HỌC: TS NGUYỄN TRUNG HỒ HÀ NỘI, 2021 TĨM TẮT NC KSNN định nghĩa tượng cạn kiệt cảm xúc c.việc, dẫn đến tư c.việc không hiệu bắt nguồn từ căng thẳng TG dài KSNN không a.hưởng sức khỏe NVYT mà a.hưởng đến hiệu CSSK NB Theo NC giới VN, có nhiều y.tố a.hưởng đến tình trạng KSNN NVYT như: cường độ c.việc cao, số l.việc lớn, trực đêm c.việc đòi hỏi tập trung, xác Đặc điểm MT l.việc như: không gian l.việc chật hẹp, thiếu nơi để nghỉ ngơi, thực trạng QL a.hưởng đến tình trạng KSNN Trong giai đoạn dịch bệnh Covid-19, NVYT mảng y tế dự phịng ngồi việc phải đảm bảo cơng tác CSSK SK người dân phải tham gia phòng chống dịch bệnh COVID-19 Do đó, đặt áp lực lớn lên toàn hệ thống NVYT TTYT 16 phường địa bàn quận Gò Vấp Tuy nhiên, chưa có NC cơng bố việc ĐG tình trạng KSNN NVYT khối dự phịng y.tố a.hưởng NC thực với mục tiêu mơ tả thực trạng KSNN phân tích số y.tố a.hưởng đến KSNN NVYT thuộc TTYT quận Gị Vấp, TPHCM năm 2022 PP NC: NC mơ tả cắt ngang, kết hợp ĐL với ĐT Cỡ mẫu NC ĐL 215 NVYT công tác TTYT 16 TYT phường Quận Gò Vấp, TPHCM Số liệu ĐL TT PP PV trực tiếp NVYT thơng qua ques có phần, nội dung khảo sát hội chứng KSNN thực theo c.cụ Maslach (MBI) Số liệu nhập Epidata 3.2 xử lý phần mềm SPSS phiên 24 Những TT ĐT TT từ PVS TLN với NVYT QL NVYT cơng tác Data ĐT phân tích theo chủ đề KQ: KQ khảo sát thấy có 20% người bị KSNN, đó, nhóm có rate cao BS (25%), nhóm NVYT khác với rate 20,5% thấp nhóm ĐD/ YS/ HS với rate 17,9% Ở khía cạnh cạn kiệt cảm xúc, có 27,4% NVYT cạn kiệt cảm xúc cao Trong nhóm ĐD/YS/ HS có rate cao với 30,5% Ở khía cạnh hồi nghi thân, có 14,4% NVYT hồi nghi thân cao, nhóm BS có rate cao với 18,8% Có 32,6% NVYT cho hiệu c.việc bị suy giảm mức độ cao (nghiêm trọng) Trong đó, nhóm ĐD/ YS/ HS có rate cao với 42,1%, nhóm BS (40,6%) Các y.tố a.hưởng đến KSNN NVYT thuộc TTYT quận Gò Vấp bao gồm NVYT l.việc TTYT quận có chênh lệch bị KSNN cao so với người làm TYT phường 2,4 lần (OR = 2,4; 95% CI: 1,14-5,18) Về y.tố c.việc, NC nhóm y.tố làm tăng risk KSNN tần suất trực đêm lần so với nhóm trực lần tuần (OR=2,46, 95% CI: 1,22-4,99); nhóm NVYT phải tham gia hoàn thiện hồ sơ bệnh án (OR=2,73, 95% CI: 1,35-5,53) nhóm khơng hài lịng với hỗ trợ đồng nghiệp (OR=4,36, 95% CI: 1,80-10,55) Thiếu NL chế độ sách tài chính, khen thưởng có tác động đến tình trạng KSNN NVYT NC đưa số khuyến nghị đến nhà QL cần xếp TG l.việc khoa học cho NVYT, hạn chế việc trực đêm lần tuần, cải thiện MT giao tiếp NVYT NVYT hay NVYT QL, LĐ KQ NC gợi ý đặc điểm c.việc có tác động đến tình trạng kiệt sức nghề nghiệp NVYT Các nghiên cứu tương lai cần làm rõ mối quan hệ KSNN c.việc MT lao động NVYT theo khoa/phòng để làm rõ khác biệt, góp phần giúp nhà QL đưa chiến lược can thiệp ĐẶT VẤN ĐỀ KSNN (KSNN) mô tả lần nhà tâm lý học Herbert Freudenberger báo năm 1974 tạp chí Journal of Social Issues (1) KSNN định nghĩa tượng cạn kiệt cảm xúc c.việc, dẫn đến tư c.việc không hiệu bắt nguồn từ căng thẳng TG dài (2) Hội chứng đặc trưng triệu chứng khó chịu, mệt mỏi, thất vọng, tự nghi ngờ giảm hiệu Một rà soát năm 2001 Maslach C cs 182 NC khắp giới cho thấy rate KSNN NVYT (NVYT) ước lượng vào khoảng 67% (dao động từ đến 80,5%) (3) NC Stefan De Hert, năm 2020, phần ba NVYT có biểu KSNN dù thời điểm (4) Tại VN, theo NC Nguyễn Thị Thu Hương công bố năm 2018 cho thấy gần 20% ĐD lâm sàng VN l.việc tình trạng KSNN (5) Hậu KSNN không a.hưởng sức khỏe NVYT mà a.hưởng đến hiệu CSSK NB NC Balch cs (2011) cho thấy sai sót y khoa BS ngoại có tương quan với mức độ KSNN làm tăng rate khiếu nại NB (6) NC Welp cs (2014) cho thấy mức độ KSNN NVYT cao có liên quan đến rate tử vong cao NB (7) Ở cấp độ tổ chức, KSNN dẫn đến rate nghỉ việc cao làm gia tăng suy nghĩ bỏ việc NVYT bao gồm BS ĐD Nó dẫn đến giảm hiệu suất lao động (8, 9) Các y.tố làm gia tăng tình trạng KSNN tồn gần tất c.việc hàng ngày NVYT Có nhiều y.tố a.hưởng đến tình trạng KSNN BS ĐD cường độ c.việc cao, số l.việc lớn, trực đêm c.việc đòi hỏi tập trung, xác Đặc điểm MT l.việc như: không gian l.việc chật hẹp, thiếu nơi để nghỉ ngơi thực trạng QL a.hưởng đến tình trạng KSNN (10-12) Gò Vấp quận nội thành TPHCM (TPHCM) Theo KQ tổng điều tra dân số nhà năm 2019 (1-4-2019) tồn quận có khoảng 680.000 dân với tổng diện tích 19,75 km² TTYT Dự phịng quận Gị Vấp thành lập năm 2007 theo Quyết định số 29/2007/QĐ-UBND Ủy ban Nhân dân TPHCM đơn vị thuộc UBND quận Gò Vấp Năm 2017, TTYT nhận Quyết định số 896/QĐ-UBND UBND TPHCM việc tổ chức lại thành “TTYT quận Gò Vấp” thuộc UBND quận Gò Vấp Đến năm 2020, TTYT nhận Quyết định số 2378/QĐ-UBND ngày 02 tháng năm 2020 Ủy ban nhân dân TPHCM tổ chức lại TTYT quận Gò Vấp trực thuộc Ủy ban nhân dân quận Gò Vấp thành TTYT quận Gò Vấp trực thuộc Sở Y tế TTYT cấu lại thành phòng chức khoa Đồng thời, TTYT QL 16 TYT phường Trong năm 2021, dịch bệnh COVID-19 bùng phát quận Gò Vấp xuất phát từ ca nghi nhiễm COVID-19 liên quan đến Hội thánh tuyên truyền Phục Hưng Sau gần tháng bùng phát, tính đến ngày 10/8/2021, số ca mắc COVID-19 quận Gò Vấp 4579 Trong giai đoạn dịch vừa bùng phát, ngồi đảm bảo cơng tác CSSK SK người dân, việc phòng chống dịch bệnh COVID-19 đặt áp lực lớn lên toàn hệ thống NVYT TTYT 16 phường địa bàn quận Điều đặt lên áp lực c.việc lớn cho NVYT TTYT quận Gò Vấp Dịch bệnh dự kiến tiếp tục kéo dài năm 2022 Trước thực tế này, ques đặt thực trạng KSNN với NVYT TTYT quận Gò Vấp (bao gồm NVYT 16 TYT Phường)? Y.tố a.hưởng tới thực trạng KSNN này? Vì vậy, chúng tơi tiến hành NC đề tài “KSNN số y.tố a.hưởng tới KSNN NVYT TTYT TYT thuộc TTYT quận Gò Vấp, TPHCM năm 2022” - MỤC TIÊU NC Mô tả thực trạng KSNN NVYT TTYT TYT thuộc TTYT quận Gò Vấp, TPHCM năm 2022 Phân tích số y.tố a.hưởng đến KSNN NVYT TTYT TYT thuộc TTYT quận Gò Vấp, TPHCM năm 2022 CHƯƠNG 1.1 TỔNG QUAN TÀI LIỆU Khái niệm 1.1.1 Định nghĩa KSNN Năm 1974, Herbert Freudenberger, nhà tâm lý học người Mỹ, đặt thuật ngữ “KSNN” nhà NC công bố tạp chí khoa học NC hội chứng KSNN (KSNN) Bài báo dựa quan sát NVYT tình nguyện phịng khám miễn phí cho người nghiện ma t Ơng mơ tả tình trạng KSNN loạt triệu chứng bao gồm KSNN yêu cầu mức c.việc triệu chứng TC đau đầu ngủ, dễ nóng suy nghĩ khép kín (13) Theo Tổ chức y tế Thế Giới, KSNN hội chứng căng thẳng mãn tính liên quan đến c.việc, với triệu chứng đặc trưng “cảm giác cạn kiệt lượng KSNN;
