Số liệu thu về được kiểm tra số lượng thơng tin và nhập liệu. Tất cả số liệu sau đĩ sẽ qua bước làm sạch để đánh giá tính nhất quán trong thơng tin, nếu cĩ sai sĩt hoặc khơng phù hợp cĩ thể điều chỉnh được, người nghiên cứu sẽ liên hệ trực tiếp với bệnh nhân qua điện thoại để lấy thơng tin ngay trong ngày.
Trong quá trình sàng lọc bệnh nhân để chọn đưa vào nghiên cứu cĩ dùng bảng kiểm để đánh giá các tiêu chuẩn loại trừ là các rối loạn mãn kinh của bệnh nhân trước mổ. Đối với các bệnh nhân tuyển chọn trong bệnh viện sẽ được phỏng vấn trước mổ, do đĩ ít cĩ khả năng sai số do nhớ lại hơn, các bệnh nhân sau mổ 1 tháng cĩ thể cĩ sai số do nhớ lại, tuy nhiên thời gian một tháng khơng phải là dài và với các thay đổi đột ngột do thiếu nội tiết bệnh nhân sẽ nhớ rõ, mặt khác đây khơng phải là biến số kết cục chính. Các biến số chính là các rối loạn hiện tại.
KẾT LUẬN
Qua nghiên cứu trên 250 phụ nữ mãn kinh do cắt hai buồng trứng, căn cứ trên các mục tiêu đề ra, kết luận được mơ tả như sau:
1. Về hiệu quả của điều trị nội tiết 6 tháng
Nhĩm rối loạn vận mạch, sau thời gian điều trị tỉ lệ bốc hỏa giảm (78,8% cịn 10,8%), đổ mồ hơi (62,4% cịn 8,8%); hồi hộp (41,2% cịn 7,6%), rối loạn giấc ngủ (27,2% cịn 10,4%)(P<0,001). Trung vị mức độ nặng trước và sau điều trị giảm cho thấy cĩ sự cải thiện cả về tần số và độ nặng của các triệu chứng.
Về nhĩm triệu chứng tâm lí: phiền muộn 35,2% cịn 7,6%, cáu gắt 41,2% cịn 6%, mệt mỏi 61,2% cịn 8% sau điều trị (P<0,001). Độ nặng của các rối loạn cũng giảm từ trung vị 3 và 2 cịn 1 sau thời gian điều trị.
2. Các yếu tố liên quan với hiệu quả điều trị
Đối với triệu chứng bốc hỏa: tuổi <50 cĩ hiệu quả gấp 3,3 lần sau khi hiệu chỉnh với các yếu tố khác (OR= 0,3) (P=0,001). Chỉ số BMI tăng thì hiệu quả điều trị bốc hỏa tăng (P=0,03). Đối với triệu chứng mệt mỏi: hiệu quả điều trị ở nhĩm tuổi 51-55 tuổi thấp hơn nhĩm tuổi 45-50 (P=0,001). Hiệu quả điều trị ở nhĩm mổ nội soi cao hơn mổ mở 1,7 lần (P=0,04).
3. Tác dụng khơng mong muốn
Căng vú là tác dụng khơng mong muốn thường gặp nhất(43,2%). Các tác dụng khơng mong muốn khác gồm: tăng cân, buồn nơn, nhức đầu, căng vú, nổi mẫn ngứa, phù chiếm tỉ lệ thấp.
KIẾN NGHỊ
1. Việc sử dụng liệu pháp nội tiết ngắn hạn (estrogen đơn độc) trên những bệnh nhân mãn kinh sau phẫu thuật nếu cĩ triệu chứng rối loạn mãn kinh là cần thiết.
2. Mỗi phụ nữ phải được tham vấn về nguy cơ và lợi ích của LPNTTT để cĩ quyết định thích hợp.
3. Trong tương lai cần thêm các nghiên cứu về hiệu quả của thay đổi lối sống để hỗ trợ giai đoạn mãn kinh, đặc biệt là các bệnh nhân cĩ triệu chứng về tâm lí.
4. Để các triệu chứng khơng tái lại trong thời gian lâu dài, cần nghiên cứu thêm vai trị của các biện pháp khơng dùng thuốc bao gồm: tập thể dục, ăn uống, duy trì lối sống khỏe để khơng tái lại các rối loạn mãn kinh.
