ĐẶT VẤN ĐỀ U nguyên bào men (Ameloblastoma) xương hàm là tổn thương lành có nguồn gốc từ cơ quan tạo răng thường gặp ở vùng hàm mặt [83], [112], [148], [152]. Trong đó, u ở xương hàm dưới (XHD) thường gặp hơn ở xương hàm trên (XHT), đồng thời có thể xâm lấn tại chỗ với hình ảnh lâm sàng, X quang phức tạp. Theo y văn thế giới, u nguyên bào men (NBM) chiếm tỉ lệ 10% trong tổng số các u xương hàm, 69,8% các trường hợp u do răng [108]. Trong đó 80% bệnh xuất hiện ở xương hàm dưới và 20% ở xương hàm trên [27], [52], [121]. U nguyên bào men xương hàm có đặc điểm tiến triển âm thầm kéo dài, không đau, không ảnh hưởng nhiều đến chức năng, có khi bệnh được phát hiện tình cờ trong quá trình khám răng miệng [89], [130], [148]. Tuy nhiên, ở những trường hợp đến khám và điều trị muộn, tổn thương đã huỷ xương hàm, phá vỡ màng xương, xâm lấn mô mềm hay cấu trúc xung quanh [42], [155]. U cũng biểu hiện biến dạng mặt, mất thẩm mỹ, ảnh hưởng đến hoạt động chức năng ăn, nhai, nuốt, nói và có thể là nguyên nhân gây tử vong trong những trường hợp nặng [140], [158]. Bên cạnh đó, hình ảnh trên phim X quang của bệnh thường phức tạp với nhiều biểu hiện khác nhau. Đa số trường hợp u NBM xương hàm là thấu quang nhiều hốc, dạng tổ ong, gồ xương, có khi dạng hỗn hợp [18], [149]. Một số trường hợp bệnh nhân trẻ tuổi, tổn thương là thấu quang một hốc, xâm lấn, đẩy lệch chân răng tương ứng. Chính điều này có thể làm cho các nhà lâm sàng dễ dàng chẩn đoán lầm lẫn với bệnh lý khác [8], [52], [116]. Mặc khác, hình ảnh vi thể của bệnh có đặc điểm phong phú, đa dạng với nhiều loại tế bào có nguồn gốc biểu mô tạo răng, góp phần làm sai lệch kết quả chẩn đoán giải phẫu bệnh (GPB) [62], [96], [102]. Với sự tiến bộ của y học ngày nay, đặc biệt là sự phát triển của hoá mô miễn dịch, với nhiều loại chất chỉ điểm (markers) khác nhau có thể giúp cải thiện tính chính xác của chẩn đoán. Tuy nhiên, các nghiên cứu đã công bố cho thấy vẫn chưa có chất chỉ điểm nào có đủ độ tin cậy và tính chuyên biệt tuyệt đối cho chẩn đoán [17], [35], [122], [137]. Do đó, việc chẩn đoán đúng bản chất bệnh này vẫn là vấn đề khó cho các bác sĩ lâm sàng cũng như giải phẫu bệnh [68], [89], [121]. Về điều trị u men, phẫu thuật là chủ yếu, các giải pháp khác như xạ hay hoá trị chỉ có tính hỗ trợ cho những u to, di căn [116], [155], [162]. Có nhiều phương pháp phẫu thuật được áp dụng và báo cáo trên y văn như mở thông u ra hốc miệng [27], [104], cắt u có mài xương quanh u, cắt nguyên khối, cắt đoạn xương có hay không ghép xương [42], [54], [104], [136]. Tuy nhiên, tổn thương có thể tái phát, xâm lấn, thậm chí di căn xa [86], [139], nên chọn lựa phương pháp điều trị thích hợp bệnh này vẫn còn là vấn đề đang tranh cãi [27], [58], [97]. Tại Việt Nam, bệnh lý này chiếm 4,7% trong tổng số các u phần mềm và xương vùng hàm mặt, nhưng bệnh chiếm tỉ lệ đến 77,4% trong tổng số u do răng [7]. Đồng thời, do tính phức tạp của biểu hiện và tiến triển bệnh, nên thực tế cho thấy rằng vẫn còn những trường hợp chưa được chẩn đoán một cách chính xác, có khi lầm với tổn thương nang hay dạng nang xương hàm [52], [105]. Trước tình hình đó, chúng tôi thực hiện đề tài “Nghiên cứu đặc điểm lâm sàng, X quang, giải phẫu bệnh và đánh giá kết quả điều trị u nguyên bào men xương hàm” với hai mục tiêu: 1. Mô tả đặc điểm lâm sàng, X quang, giải phẫu bệnh u nguyên bào men xương hàm. 2. Đánh giá kết quả điều trị bệnh lý trên.
