thất trái (TT) thường ý đặc biệt Lý cho quan tâm dịch tễ thực hành Thất trái nơi bị ảnh hưởng nhồi máu tim, nguyên nhân hàng đầu gây tử vong bệnh tật Ngoài thất trái dễ dàng khảo sát siêu âm chụp mạch Gần tim phải mà đặc biệt thất phải (TP) trở thành tâm điểm ý Rối loạn chức thất phải thường kết hợp với tăng tỷ lệ tử vong bệnh tật bệnh nhân mắc tim bẩm sinh, bệnh van tim, bệnh mạch vành, tăng áp phổi suy tim [1],[2],[3] Mặc dù thất trái nghiên cứu kỹ với đầy đủ giá trị bình thường kích thước, thể tích, khối lượng chức thơng số đo lường kích thước chức thất phải lại thiếu nhiều Hình dạng mặt cắt hình ảnh tiêu chuẩn thất trái giúp thiết lập việc đánh giá thất trái liệu hình thái bình thường thất phải hạn chế phần hình dạng phức tạp Sự phát triển cộng hưởng từ tim mạch (Magnetic Resonance Imaging: MRI) cho phép có hiểu biết hình học chức thất phải Nếu siêu âm tim hai bình diện 2- dimension: 2D) thông thường trước cho phép đánh giá chức thất phải cách định tính hay phương pháp Doppler đo vận tốc dòng qua van ba qua van động mạch phổi thường bị ảnh hưởng thay đổi tiền gánh hậu gánh cung cấp thông tin tiên lượng mạnh mẽ cho định lâm sàng với phát triển kỹ thuật công nghệ siêu âm tim hình ảnh Doppler mơ, hình ảnh strain, hình ảnh siêu âm bình diện (3- dimension: 3D) thể tích ngày gia tăng tương ứng khả đánh giá thất phải định tính định lượng Mặc dầu MRI tim mạch tiêu chuẩn vàng để đánh giá thất phải, siêu âm tim phương tiện đánh giá ban đầu hữu hiệu sẵn có khả di chuyển thiết bị Do chun đề vai trò siêu âm tim đánh giá hình thái chức thất phải cung cấp phương pháp đồng chuẩn mực việc đánh giá thất phải II GIẢI PHẪU, SINH LÝ VÀ CHỨC NĂNG THẤT PHẢI Giải phẫu sinh lý Bình thường thất phải nằm phía trước khoang ngực nên mặt cắt siêu âm để đánh giá TP gồm mặt cắt cạnh ức, mỏm sườn Về giải phẫu, TP chia thành phần buồng nhận, phần phễu hay buồng tống phần mỏm hay phần bè [4] Hình Sơ thành phần buồng thất phải Phần buồng nhận trải từ nhú đến vòng van ba Phần mỏm từ nhú đến mỏm thất phải Phần phễu vùng trơn nhẵn dẫn đến van phổi [5] Trong buồng thất phải có dải cơ: dải vách, dải cạnh vách dải điều hoà Trong bệnh tim bẩm sinh, dải điều hoà dùng để phân biệt 10 thất trái thất phải Vì thất phải khơng có hình dạng cân đối truyền thống nên khơng thể dự đốn thể tích thất phải Những rối loạn vận động vùng khó để đánh giá có vùng khác Khó khăn đánh giá vận động vùng co ngắn lại thớ kết hợp theo chiều dọc, chiều ngang vòng tròn vận động độc lập khơng thể nhìn thấy rõ ràng siêu âm [6] Chức huyết động TP mặt sinh lý khác TT Đó nguyên uỷ hậu gánh thấp giường mạch phổi làm áp lực TP thấp áp lực TT Một nguyên tắc quan trọng đổ đầy tâm trương TP phụ thuộc thất thay đổi kháng mạch phổi áp lực tim thường bị ảnh hưởng hơ hấp Sự phụ thuộc lẫn thay đổi đổ đầy hiệu tâm thu hai thất [7] Trong hít vào, dòng qua van ba (tiền gánh thất phải) tăng khoảng 20% dòng qua van hai (tiền gánh thất trái) giảm nhẹ khoảng 10% ngược lại thở Các bệnh lý ảnh hưởng đến điều kiện đổ đầy viêm màng tim co thắt, nhồi máu phổi nhồi máu thất phải thường ảnh hưởng đến mối quan hệ Trong chừng mực tình trạng lâm sàng bệnh phổi mạn tính, béo phì, bệnh tim bẩm sinh dẫn đến hạn chế mặt kỹ thuật ngăn khơng nhìn rõ hình dạng cấu trúc thất phải, với điều kiện đổ đầy thất phải bất thường làm giảm việc đánh giá xác hiệu TP Các giá trị bình thường TP bị hạn chế đáng kể thiếu tiêu chuẩn protocol siêu âm tim MRI tim mạch đánh giá TP mặt cắt mà khơng thể có siêu âm cung cấp loạt giá trị bình thường Mặc dù khối lượng thể tích TP nam giới lớn nữ giới khơng có khác biệt đáng kể chức tâm thu hai giới [8]
