hóa hay gọi u mô đệm dày- ruột (Gastro Intestinal Stroma Tumor – GIST) khối u trung mô đường tiêu hóa có nguồn gốc từ tế bào thành ống tiêu hóa hay tế bào Cajai [1],[2] Bệnh chiếm khoảng 0,2% bệnh lý đường tiêu hóa, với tỷ lệ mắc bệnh khoảng 1,5/100.000 dân [3],[4] Theo thống kê, hàng năm Hoa Kì có khoảng 5000 ca mắc [5] Trên giới, trước năm 1990, GIST thường chẩn đoán nhầm sarcoma phần mềm sarcoma trơn, sarcoma mỡ… tính chất tế bào vi thể loại giống Gần nhờ phát triển kỹ thuật nhuộm hóa mô miễn dịch nghiên cứu gen, nhà giải phẫu bệnh tìm thấy đột biến gen KIT, gen tiền ung thư tìm bộc lộ kháng nguyên bề mặt CD-117 nên phân biệt rõ loại bệnh [6],[7],[8],[9] Từ mở cách mạng chẩn đoán điều trị u mô đệm đường tiêu hóa Hiện nay, chẩn đoán u mô đệm đường tiêu hóa cần dựa vào triệu chứng lâm sàng, xét nghiệm cận lâm sàng nội soi ống tiêu hóa, chụp cắt lớp vi tính, chụp cộng hưởng từ ổ bụng, Xét nghiệm mô bệnh học nhuộm hóa mô miễn dịch tiêu chuẩn vàng để chẩn đoán bệnh Các thông tin vị trí, kích thước đại thể khối u, hình ảnh vi thể khối u đóng vai trò quan trọng để phân độ mức độ nguy tái phát bệnh [10],[11] Đây sở để nhà lâm sàng xây dựng kế hoạch điều trị tiên lượng cho bệnh nhân Về mặt điều trị, phẫu thuật phương pháp điều trị triệt căn Đối với giai đoạn tiến triển di căn, điều trị toàn thân thuốc kháng tyrosin kinase (TKIs) – Imatinib lại đóng vai trò quan trọng mà trước phương pháp hóa trị hay xạ trị không hiệu Chính hiệu thuốc giai đoạn tiến triển di căn, điều trị bổ trợ, điều trị tân bổ trợ Imatinib áp dụng thu kết ngoạn mục Gần đây, thuốc TKIs khác đời chứng minh có hiệu điều trị Hơn nữa, với tiến ngành sinh học phân tử, xét nghiệm đột biến gen áp dụng đánh giá khả đáp ứng, lựa chọn phương pháp điều trị thích hợp cho bệnh nhân nhằm đạt hiệu điều trị cao Do người thầy thuốc phải có kiến thức định bệnh học, kinh nghiệm lâm sàng, không ngừng cập nhật thông tin chẩn đoán, điều trị bệnh đặc biệt phải có phối hợp nhà nội khoa, ngoại khoa, xạ trị, chuyên ngành cận lâm sàng như: chẩn đoán hình ảnh, giải phẫu bệnh Chính vậy, thực chuyên đề “Dịch tễ học, nguyên nhân, đặc điểm bệnh học bệnh u mô đệm đường tiêu hóa” với mục tiêu: Đặc điểm dịch tễ học, nguyên nhân bệnh sinh u mô đệm đường tiêu hóa Đặc điểm bệnh học, điều trị tiến điều trị u mô đệm đường tiêu hóa I DỊCH TỄ HỌC VÀ NGUYÊN NHÂN 1.1 Dịch tễ học U mô đệm đường tiêu hóa (GIST) sarcoma mô mềm đường tiêu hoá Đây loại u trung mô ác tính thường gặp đường tiêu hoá, chiếm khoảng - 3% u ác tính dày ruột [9],[12],[13] GIST xuất phát từ mặt dày ruột có khuynh hướng phát triển ống tiêu hoá Ngoài GIST khởi phát từ vị trí bên đường tiêu hoá mạc nối lớn, mạc treo ruột hay sau phúc mạc [2] Đại phận GIST xuất phát từ dày chúng xuất phát từ ruột non, đại tràng thực quản GIST dày gặp nhiều với tỷ lệ 39 - 70%, ruột non 20 - 35%, đại tràng, mạc nối lớn - 15%, mạc treo ruột 9%, thực quản ≤ 5% [14] Tại Mỹ hàng năm có khoảng 5000 trường hợp mắc [15] Ở Pháp hàng năm số mắc 1000 trường hợp [16] Theo báo cáo trung tâm lớn có khoảng 10 - 20 trường hợp mắc năm, chiếm 1% u ác tính đường tiêu hoá [17] GIST gặp tuổi chủ yếu lứa tuổi trung niên cao tuổi, gặp lứa tuổi 40 Theo báo cáo SEER, tuổi trung bình chẩn đoán bệnh 63 với lứa tuổi hay gặp 60-69 tuổi [15], [18],[19] Nam gặp nhiều nữ, tỷ lệ nam/nữ 1,2 - 2/1 [17],[20] 1.2 Nguyên nhân yếu tố nguy Hiện người ta chưa biết nhiều yếu tố nguy u mô đệm đường tiêu hóa Một số trường hợp GIST xảy nhiều thành viên gia đình Tuy nhiên hầu hết xuất riêng lẻ nguyên nhân rõ ràng [7] Trước năm 1998, người ta chưa hiểu biết nhiều bệnh, đặc biệt sinh học phân tử bệnh Từ phát đột biến gen KIT có liên quan đến bệnh, sinh học phân tử nguyên nhân gây bệnh làm sáng tỏ Cũng nhờ phát mang tính cách mạng này, phương pháp điều trị bao gồm thuốc TKIs đời làm thay đổi hẳn kết điều trị GIST Về mặt chế bệnh sinh, đa số GIST có chứa đột biến gen KIT chiếm tỷ lệ khoảng 80%, có biểu protein kinase KIT kích hoạt hoạt hoá liên tục Khoảng - 5% trường hợp GIST đột biến KIT có đột biến kích hoạt thụ thể tyrosine kinase có liên quan thụ thể yếu tố tăng trưởng nguồn gốc tiểu cầu α (platelet- derived growth factor receptor alpha – PDGFRA) Người ta dựa vào tyrosine gastrointestinal stromal tumours, Histopathology 53(3), tr 245-66l 10 Đến có gen phát có đột biến liên quan với GIST c-KIT PDGFRA (platelet-derived growth factor receptor, Alpha polypeptid) Khoảng 75-80% GIST có đột biến tiền gen gây ung thư c-KIT [22],[23].Các đột biến có hoạt tính hầu hất thường xảy exon 11 (khoảng 70%) tiền gen gây ung thư c-KIT (vùng sát màng tế bào), gặp exon (vùng sát màng tế bào), 13, 17 (vùng bào tương) 13, 17 (vùng bào tương) Gen nằm nhiễm sắc thể 4q12, mã hóa protein thuộc họ thụ thể tyrosine kynase (KIT) CD-117 (c-KIT) nhóm định kháng nguyên (epitope) cho KIT Khi gen đột biến dẫn đến bất thường thụ thể KIT, kết dẫn đến hoạt hóa liên tục tyrosine kynase típ III Sự hoạt hóa truyền tín hiệu KIT vào nhân tế bào làm tế bào tăng sinh không kiểm soát không vào đường chết theo chương trình (apoptosis) (hình 1.1) Đây sinh bệnh học chủ yếu GIST Tùy nghiên cứu, có khoảng 70-90% GIST có đột biến hoạt hóa gen c-KIT Khoảng 20-25% đột biến gen c-KIT, số người ta thấy khoảng 8% có đột biến PDGFRA [9],[21],[24] Sự kết cặp với yếu tố tế bào gốc (sterm cell factor) làm nhị trùng hóa cá protein bề mặt KIT làm hoạt hóa vùng gắn men kinaza bào tương, đồng thời phosphoryl hóa phần tyrosin Tiếp sau loạt phản ứng dây chuyền xảy làm hoạt hóa RAS, sau làm MAP, hoạt hóa MAP làm thay đổi bộc lộ gen tế bào chuyển tế bào từ pha G1 sang pha S Theo đường STAT dẫn đến chép gen Theo đường AKT làm thay đổi phiên giải tổng hợp protein, chuyển hóa đưa tế bào vào đường chết theo chương trình Khi có đột biến gen làm thay đổi đường hoạt hóa, làm trình chép gen bị thay đổi dẫn đến rối loạn trình điều hòa, kiểm soát chu trình tế bào 10 69 Ngoài Sunitinib Regorafenib, TKIs khác nghiên cứu điều trị GIST, hứa hẹn đem lại hiệu quả, đặc biệt kháng với thuốc như: Sorafenib, Ponatinib, Nilotinib, Pazopanib,… 8.3 Vai trò xạ trị Điều trị triệu chứng: trường hợp GIST tiến triển, tái phát di có triệu chứng, không đáp ứng với TKIs xạ trị lựa chọn Đặc biệt trường hợp GIST di xương Mặc dù vai trò xạ trị GIST hạn chế khối u kháng xạ trị cho tỷ lệ đáp ứng thấp, nhiên kháng với thuốc ức chế tyrosin kinase (TKIs), xạ trị lại có lợi ích số bệnh nhân phù hợp [113],[114] Một nghiên cứu tiến cứu 25 bệnh nhân GIST di thành bụng di gan, kháng TKIs điều trị xạ trị cho thấy, tỷ lệ đáp ứng thấp 8% có đáp ứng, tỷ lệ bệnh ổn định đạt cao, 80% bệnh nhân bệnh ổn định với thời gian > tháng [113] 69 70 KẾT LUẬN U biểu mô đường tiêu hóa bệnh lý hay gặp u trung mô ống tiêu hóa Bệnh thường gặp dày ruột non, nhiên gặp vị trí đường tiêu hóa : thực quản, đại tràng, trực tràng, vị trí đường tiêu hóa : sau phúc mạc, mạc treo ruột hay mạc nối lớn Trong năm gần đây, với hiểu biết rõ chế sinh bệnh, sinh học phân tử, … chẩn đoán điều trị bệnh có bước tiến vượt bậc Chẩn đoán xác định bệnh thiết phải có xét nghiệm mô bệnh học nhuộm hóa mô miễn dịch với CD-117 dương tính Triệu chứng lâm sàng bệnh đa dạng, khác tùy thuộc vào giai đoạn bệnh, vị trí khối u nguyên phát Các triệu chứng bệnh giai đoạn sớm thường mơ hồ : khó tiêu, chướng bụng,…hoặc triệu chứng Đến bệnh giai đoạn tiến triển, triệu chứng lâm sàng thường rõ rệt, rầm rộ với biểu xuất huyết tiêu hóa, đau bụng triệu chứng bít tắc,…Các xét nghiệm cận lâm sàng có giá trị chẩn đoán bệnh bao gồm nội soi đường tiêu hóa, xét nghiệm chẩn đoán hình ảnh : chụp CLVT ổ bụng, chụp cộng hưởng từ,…Xét nghiệm mô bệnh học nhuộm hóa mô miễn dịch tiêu chuẩn vàng để chẩn đoán xác định bệnh Xét nghiệm mô bệnh học, nhuộm hóa mô miễn dịch hay xét nghiệm gen sinh học phân tử không đóng vai trò định chẩn đoán xác định bệnh mà đóng vai trò quan trọng phân loại mức độ nguy cơ, tiên lượng bệnh, tiên lượng đáp ứng điều trị từ giúp thày thuốc lập kế hoạch điều trị, theo dõi tiên lượng bệnh 70 TÀI LIỆU THAM KHẢO Kindblom L G., Remotti H E.,
