Widespread antibiotic use should be discouraged, as it may lead to higher bacterial resistance rates, which will impact the disease burden and deaths during the pandemic and beyond
Promote AAP and use during the COVID-19 pandemic
• Mild: against the use of antibiotic therapy or prophylaxis
• Moderate: that antibiotics should not be prescribed unless clinical suspicion of
bacterial infection
•Severe: use empiric antibiotics (as soon as possible) to treat likely pathogens

Pneumonia treatment in the community1
• Don’t use ABs if COVID-19 is the likely cause or if symptoms are mild
• Give ABs if the likely cause is bacterial, the cause is unclear or if high risk of complications
Pneumonia treatment in hospital2
• Start empirical ABs if there is clinical suspicion of bacterial infection • Review at 24–48 hours or as soon as test results are available
• Stop ABs if the pneumonia is due to COVID-19 and with no evidence of bacterial infection • Review based on microbiological results and switch to a narrower spectrum AB if
• If ABs are continued, administer for 5 days, then stop unless clear indication to continue

Empirical treatment
ABs and dosage (oral doses are for immediate-release medicines)Oral ABs for
moderate or severe
Options include:
Doxycycline: 200 mg on first day, then 100 mg OD
Amoxicillin-clavulanate: 500 mg/125 mg TDS with clarithromycin: 500 mg BD
In severe pneumonia, and if the other options are unsuitable: levofloxacin: 500 mg OD or BD (consider
the safety issues with FQs)
Amoxicillin-clavulanate: 1.2 g TDS with clarithromycin: 500 mg BD
Cefuroxime: 750 mg TDS or QDS (increased to 1.5 g TDS if infection is severe) with clarithromycin: 500
mg BD in severe pneumonia, and if the other options are unsuitable: levofloxacin: 500 mg OD or BD (consider the safety issues with FQs)
There are no validated tools to assess the severity of CAP in the context of the COVID-19 pandemic; severity should be based on clinical judgement Consult a local microbiologist for alternative options, including for pregnant women If there is a penicillin allergy, avoid using AMC and use cefuroxime with caution For safety issues with FQs, see the Medicines and Healthcare products Regulatory Agency advice This covers restrictions and precautions for using FQ ABs because of very rare reports of disabling and potentially long-lasting or irreversible side effects affecting musculoskeletal and nervous systems Warnings include: stopping treatment at the first signs of a serious adverse reaction (such as tendonitis), prescribing with special caution for people >60 years and avoiding coadministration with a corticosteroid (March 2019).

Nếu có kết quả vi sinh: Thực hiện ngay khi có kết quả vi sinhxác định tác nhân gây bệnh và KSĐ.
Nếu không có kết quả vi sinh, xuống thang thực hiện khi BNcó đáp ứng điều trị tốt, khi đó dựa trên cơ sở dữ liệu vi sinhtại chỗ để cân nhắc và lựa chọn lại kháng sinh hợp lý.
Các giai đoạn:
+ Giaiđoạn 1: Khởi đầu dùng phác đồ KS phổ rộng nhằm làm tănghiệu quả và giảm đề kháng
+ Giaiđoạn 2: Thực hiện liệu pháp xuống thang KS => giảm độctính của KS, giảm kháng thuốc và giảm chi phí điều trị.
