MỞ ĐẦU Listeria monocytogenes là loài vi khuẩn truyền bệnh qua thực phẩm rất nguy hiểm, đặc biệt với tỷ lệ tử vong lên đến 30%. Bệnh do L. monocytogenes gây ra đƣợc gọi chung là listeriosis. Đối tƣợng có nguy cơ bị nhiễm bệnh này thƣờng là những ngƣời có hệ thống miễn dịch bị suy yếu nhƣ trẻ sơ sinh, phụ nữ mang thai, ngƣời lớn tuổi. Nếu không đƣợc điều trị kịp thời bằng kháng sinh, bệnh nhân có thể bị nhiễm trùng máu, viêm màng não, sẩy thai và tử vong. Vi khuẩn L. monocytogenes rất phổ biến trong môi sinh. Chúng đƣợc tìm thấy ở nhiều nơi nhƣ đất, nƣớc, thực vật, thực phẩm, thức ăn gia súc, trong phân ngƣời hoặc phân động vật. Chúng có thể sống và tăng trƣởng chậm ở các điều kiện khắc nghiệt về nhiệt độ, hàm lƣợng khí oxy, pH và hoạt độ nƣớc. Đặc biệt loài vi khuẩn này có khả năng sinh trƣởng ở nhiệt độ lạnh (0-10 o C). Chính vì những lý do đó mà khả năng nhiễm L. monocytogenes trên thực phẩm là khá cao, đặc biệt các sản phẩm đƣợc bảo quản lạnh trong thời gian dài. Nhiều loại thực phẩm đƣợc bày bán trong siêu thị hiện nay là các thực phẩm ăn sẵn. Thêm nữa càng ngày ngƣời ta lại càng muốn sử dụng các loại thực phẩm không có chất bảo quản hoặc không bị gia nhiệt để giữ đƣợc nguyên vẹn hƣơng vị, cấu trúc nguyên liệu ban đầu. Những sản phẩm nhƣ vậy thƣờng đƣợc bảo quản lạnh trƣớc khi sử dụng. Vì vậy, các sản phẩm thực phẩm loại này có thể có nguy cơ nhiễm L. monocytogenes cao. Rất nhiều các vụ ngộ độc và triệu hồi sản phẩm do thực phẩm nhiễm L. monocytogenes trên khắp thế giới đã đƣợc thông báo. Các nghiên cứu về dịch tễ học phân tử các chủng L. monocytogenes phân lập đƣợc từ các vụ dịch lớn trên thế giới cho thấy, có những chủng L. monocytogenes có khả năng gây bệnh và gây bùng phát dịch rất lớn, bên cạnh đó cũng có những kiểu huyết thanh hầu nhƣ không có khả năng gây bệnh và gây dịch. Cho đến thời điểm này chƣa có công bố nào về dịch tễ học phân tử các chủng L. monocytogenes phân lập từ thực phẩm tại Việt Nam. Khả năng gây bệnh và gây dịch của L. monocytogenes nhiễm trong các thực phẩm tại Việt Nam vẫn là một ẩn số. Phƣơng pháp ISO 11290-1:1996 phát hiện L. monocytogenes là phƣơng pháp nuôi cấy. Một số các phƣơng pháp sinh học phân tử nhƣ PCR, real-time PCR hoặc các phƣơng pháp miễn dịch (ELISA) cũng đã đƣợc tiêu chuẩn hóa. Mấy năm trở lại đây, cùng với sự ra đời của một số kỹ thuật khuếch đại đẳng nhiệt axit nucleic, việc phát hiện nhanh các vi sinh vật gây bệnh đã đƣợc đơn giản hóa thêm một bƣớc. LAMP là kỹ thuật khuếch đại đẳng nhiệt đƣợc sử dụng phổ biến hơn cả. Ngoài ra, khả năng phát hiện trực tiếp sản phẩm của phản ứng LAMP không cần điện di là một trong các ƣu điểm nổi trội của phƣơng pháp này, giúp đơn giản hóa và rút ngắn thời gian phân tích. Kỹ thuật này đã nhanh chóng đƣợc ứng dụng để phát hiện L. monocytogenes trong thực phẩm ở nhiều nghiên cứu trên thế giới. Ở Việt Nam, phƣơng pháp tiêu chuẩn TCVN: 7700-1:2007 phát hiện L. monocytogenes là phƣơng pháp nuôi cấy, sau đó khuẩn lạc đặc trƣng đƣợc khẳng định bằng các phản ứng sinh hóa. Đã có một số nghiên cứu về phƣơng pháp PCR và que thử sắc ký miễn dịch nhằm phát hiện vi khuẩn này, tuy nhiên thời gian phân tích còn dài hoặc yêu cầu cao về trang thiết bị hóa chất nên chƣa đáp ứng đƣợc nhu cầu phân tích phát hiện trong thực tế sản xuất. Trong khuôn khổ của luận án này, chúng tôi nghiên cứu đề tài có tên là “Nghiên cứu chế tạo bộ sinh phẩm phát hiện nhanh Listeria monocytogenes và phân tích đặc điểm dịch tễ học phân tử của chủng phân lập được trong một số thực phẩm có nguy cơ cao trên thị trường Việt Nam”. Luận án này tập trung vào hai mục tiêu chính sau:  Phân tích đƣợc đặc điểm dịch tễ học phân tử của các chủng L. monocytogenes phân lập đƣợc từ các loại thực phẩm có nguy cơ cao tại Việt Nam.  Xây dựng đƣợc một bộ sinh phẩm phát hiện nhanh L. monocytogenes trong các loại thực phẩm ăn liền dựa trên kỹ thuật LAMP. Trong nghiên cứu này đối tƣợng nghiên cứu là L. monocytogenes. Phạm vi nghiên cứu tập trung phát triển phƣơng pháp phân tích phát hiện nhanh L. monocytogenes trong thực phẩm dựa trên kỹ thuật LAMP. Qui trình phân tích cần tối ƣu sao cho đơn giản, chính xác, tiện lợi và nhanh nhất, góp phần kiểm soát chất lƣợng an toàn vệ sinh thực phẩm. Bên cạnh đó việc nghiên cứu dịch tễ học các chủng L. monocytogenes phân lập đƣợc từ thực phẩm cũng giúp chúng ta thấy đƣợc hiện trạng mức độ nguy hiểm của các dòng L. monocytogenes hiện đang nhiễm trên một số nhóm thực phẩm tại Việt Nam. Từ đó có thể giúp các nhà quản lý, sản xuất, phân phối và tiêu dùng có các biện pháp thích hợp nhằm nâng cao chất lƣợng vệ sinh an toàn thực phẩm đối với đối tƣợng vi khuẩn nguy hiểm này.
