1 , 2* – – *Email:dr.trantandat@gmail.com Mục tiêu nghiên cứu: ứ ng d ng gi i phẩu th , ủ ủ ủ ủ ủ o ủ ủ ABSTRACT ANATOMICAL STUDY OF THE LATERAL FEMORAL CUTANEOUS NERVE Tran Tan Dat1, Cao Thi2 HCMC Hospital for Rehabilitation - Professional Diseases University of Medicine and Pharmacy at Ho Chi Minh City Background: The lateral femoral cutaneous nerve (LFCN) is purely sensory, derives from the lumbar plexus and supplies the anterolateral aspect of the thigh Several surgical procedures such as anterior iliac-crest bone-graft harvesting, anterior approach to the hip joint can damage LFCN lead to a condition called meralgia paresthetica (MP) Objectives: to evaluated the application of anatomy of lateral femoral cutaneous nerve Materials and Method: Prospective study, describing case series We dissected the pelves and thighs of Vietnamese fresh cadavers in Anatomy deparment, University of medicine and pharmacy at Ho Chi Minh city Results: In all cases, the LFCN arises from the L2 and L3 roots, runs through the pelvis along the lateral border of the psoas muscle (PM) and crosses the iliacus muscle obliquely under the iliac crest (IC), toward and medial from the anterior superior iliac spine (ASIS) between two laminae iliac fascia Three distinct branching patterns of the LFCN within the abdomen were noted The LFCN had one distinct vertebral origin and crossed the inguinal ligament as a single process (63.3%) The two distinct branches from the lumbar plexus that joined together before traversing the inguinal ligament (IL) (23.3%) The one distinct vertebral origin, which split into two branches before traversing the inguinal ligament (13.3%) The nerve cross medial to the ASIS (90%), lateral to the ASIS (10%) Three types of branching pattern of the LFCN in thigh were observed: Sartorius-Type (60%), Posterior-Type (20%), Fan-Type (20%) The mean distances between the point where the LFCN left the lateral border of the PM and the visible medical end of the IC 54.1mm, ASIS 91.1mm, Pubic tubercle 144.3mm Distance from the IC at 2cm and 5cm lateral to the ASIS were 2.4cm and 4.1cm The distance from the point LFCN crosses IL to femoral artery 64.4mm, ASIS 10.1mm, the ratio between LFCN-ASIS and LFCN-PT was 0.08 The mean depth of the LFCN below the inguinal ligament was 5.3mm The angle between the pelvic and femoral portions of the LFCN was 111° The LFCN side-to-side diameter was mm at the level of the IL The mean distance between the ASIS and the point where the LFCN crossed the lateral border of the sartorius was 32.1mm, medial border of the tensor fascia lata was 47.2mm The distance between the point 1cm distal and medial ASIS to LFCN was 5.7mm Conclusion: The relation between anatomy course of LFCN and some bony mark will help surgeon define LFCN in surgery Keywords: lateral femoral cutaneous nerve, meralgia paresthetica, anterior superior iliac spine, anterior approach to the hip joint
