Phẫu thuật nội soi điều trị nang lách hầu nhƣ thay thế phẫu thuật mổ mở qua đƣờng bụng. Phẫu thuật nội soi cắt lách điều trị nang lách cho thấy là một phẫu thuật an toàn, khả thi, thời gian hồi phục nhanh, thời gian trở lại ăn uống, sinh hoạt bình thƣờng nhanh. Nhƣng phẫu thuật nội soi cắt lách đòi hỏi cao về kỹ thuật, kinh nghiệm và tính chuyên nghiệp của phẫu thuật viên về mổ lách vì lách là tạng đặc, mềm mại, không cầm nắm lách đƣợc nhƣ tạng khác, mô lách và nang lách rất dễ vỡ, dễ chảy máu và khó cầm máu.
Phẫu thuật nội soi cắt lách là một phẫu thuật khó, đòi hỏi cần đƣợc thực hiện ở trung tâm chuyên khoa và phẫu thuật viên có kinh nghiệm về phẫu thuật nội soi cắt lách. Nhiều tai biến (chảy máu do dị dạng mạch máu, thủng tạng do viêm dính) có thể xảy ra trong mổ và biến chứng sau mổ là trở ngại dẫn đến sự thành công của phẫu thuật nội soi.
KẾT LUẬN
Nghiên cứu các trƣờng hợp nang lách đƣợc can thiệp bằng phƣơng pháp phẫu thuật nội soi từ ngày 01/01/2009 đến ngày 31/12/2013, tại khoa Ngoại gan mật tụy Bệnh viện Chợ Rẫy và tại khoa Ngoại tổng quát 2 Bệnh viện Bình Dân, chúng tôi rút ra kết luận sau:
1. Đặc điểm lâm sàng, cận lâm sàng và giải phẫu bệnh của nang lách: − Gặp ở mọi lứa tuổi, tuổi trung bình mắc bệnh 33,6 ± 14,3 (16 – 71) tuổi, nữ giới gặp nhiều hơn nam giới, tỉ số nữ/nam: 1,6/1.
− Triệu chứng lâm sàng bệnh nhân nang lách thƣờng gặp nhất là đau bụng và u bụng.
− Siêu âm bụng và CT bụng chậu là phƣơng tiện cận lâm sàng có giá trị định bệnh nang lách với độ nhạy theo thứ tự là 89,3% và 94,3%.
− Nang có kích thƣớc trung bình 9,7cm, nhiều nhất vị trí trung tâm. − Đa số là nang thật lành tính nhƣ nang bẩm sinh, bạch huyết, mạch máu. Nang giả chiếm tỷ lệ thấp, không có nang ký sinh trùng.
2. Kết quả sớm của phẫu thuật nội soi cắt lách điều trị nang lách:
− Tỉ lệ thành công của phẫu thuật nội soi cắt lách là 91,9%, có 8,1% phải chuyển mổ mở (do viêm dính quá nhiều và rách mạch máu lách do dị dạng gây chảy máu khó cầm).
− Phẫu thuật nội soi cắt lách là an toàn (tử vong 0%) và ít tai biến, biến chứng (5,4%, chủ yếu là chảy máu trong khi mổ).
