đoán điều trị can thiệp dị dạng động tĩnh mạch tủy DSA, tất theo dõi đánh giá lại sau - tháng điều trị Chúng tơi có kết luận sau: Đặc điểm hình ảnh dị dạng động tĩnh mạch tủy Trong 21 bệnh nhân, có trường hợp thuộc nhóm I, 10 trường hợp nhóm II trường hợp nhóm IV, khơng có trường hợp nhóm III (theo phân loại Anson - Spetzler 1992) - Hình ảnh CHT: dấu hiệu thường gặp trống tín hiệu dòng chảy quanh tủy ổ dị dạng (100%), bên cạnh dấu hiệu phù tủy (95,2%) Chảy máu dị dạng gặp 33,3 % Tất trường hợp xác nhận chẩn đoán - DSA Hình ảnh DSA: dị dạng khẳng định phim chụp số hóa xóa động mạch tủy sống Động mạch ni thường có kích thước lớn bình thường, cấp máu thơng qua ổ dị dạng lỗ rò trực tiếp, tĩnh mạch dẫn lưu chạy dài, giãn ngoằn ngoèo hình sớm động mạch o Dị dạng nhóm I chủ yếu có cuống nuôi, nằm vùng ngực o thắt lưng Dị dạng nhóm II nhóm IV thường có nhiều cuống Trong nhóm II phân bố nhiều vùng cổ, nhóm IV có phân bố thay đổi theo - chiều dọc tủy sống Tỷ lệ gặp giả phình nghiên cứu 28,6%, phình liên quan với xuất huyết có ý nghĩa thống kê p < 0,05 Kết điều trị dị dạng động tĩnh mạch tủy 95 - Số lần can thiệp: chủ yếu lần (76,2%) Các trường hợp can thiệp nhiều lần - dị dạng nhóm II nhóm IV Tỷ lệ nút tắc thành cơng: 61,9% gây tắc hồn tồn, 38,1% gây tắc bán phần, khơng có trường hợp thất bại Các trường hợp gây tắc bán phần gặp nhiều - nhóm II (62,5%) Tai biến can thiệp: tỷ lệ tai biến can thiệp gặp 5/21 bệnh nhân, chưa - thấy hậu lâm sàng sau thời gian theo dõi Cải thiện CHT sau - tháng: hết hoàn toàn dấu hiệu tổn thương 28,6% bệnh nhân, 61,9% bệnh nhân thấy giảm mức độ khác nhau, 9,5% lại không thấy tổn thương thay đổi, không ghi nhận thấy trường hợp - nặng lên Cải thiện lâm sàng: đạt tỷ lệ 80,9%, không cải thiện 19,1%, không thấy trường hợp nặng lên Sự khác biệt dẫn lưu ngược dòng TM tủy Hình chụp sau nút tắc, khơng hình TM dẫn lưu CHT sau tháng: Hết phù tủy, khơng thấy dòng chảy trống tín hiệu quanh tủy  Lâm sàng sau tháng: Triệu chứng giảm không đáng kể
