Với những kiến thức đã học được từ môn Hóa dược đại cương thuộc Trường Hóa vàKhoa học sự sống-ĐHBK Hà Nội, và để hiểu hơn về nội dung khía cạnh của môn học,nên chúng em đã lựa chọn đề tài tìm hiểu về propanol để làm bài tiểu luận cho mônhọc này Trong quá trình hoàn thiện bài, em đã nhận được rất nhiều sự trợ giúp, hướngdẫn tận tình từ cô Nguyễn Thị Thùy Mỵ Những lời góp ý của cô đã giúp chúng em rấtnhiều trong việc định hướng và triển khai bài tiểu luận.

Trong những thập kỷ qua, bệnh timmạch được coi là vấn đề sức khỏe chủyếu ở các nước phương Tây Tuynhiên, ngày nay, các vấn đề về timmạch cũng ngày càng tăng ở các nướcđang phát triển, trong đó có cảEthiopia Các bệnh về tim mạchthường gặp nhất bao gồm: Huyết ápcao; Suy tim sung huyết; Đau thắtngực và Rối loạn nhịp tim.
Hầu hết các loại thuốc trên thị trườnghiện nay chỉ có thể làm giảm tỉ tệ mắcbệnh và tử vong do các bệnh trên gâyra Do vậy, chương này sẽ thảo luận vềtính chất dược lý của những loại thuốcnày.
I Thuốc hạ huyết áp
a Nhận định chung
Huyết áp cao được định nghĩa là tạicác lần đo ngẫu nhiên, huyết áp ởđộng mạch có sự tăng lên cao hơn giátrị bình thường Hiệp hội Tim mạchHoa Kỳ đã định nghĩa: Huyết áp caokhi huyết áp động mạch cao hơn140/90 mmHg (dựa trên ba lần đo ởcác thời điểm khác nhau) Huyết ápcao có thể được chia thành 3 loại, tùytheo mức độ huyết áp tâm trương: Cao huyết áp nhẹ: huyết áp tâm

Tăng huyết áp động mạch kéo dài làmtổn thương mạch máu trong thận, timvà não và dẫn đến tăng tỷ lệ mắc bệnhsuy thận, suy tim và đột quỵ.
Hiệu quả thuốc hạ huyết áp ngăn ngừatổn thương mạch máu và giảm tỷ lệmắc bệnh và tử vong.
Để hiểu được bệnh lý của các trạngthái tăng huyết áp và lần lượt cơ sở lýluận của trị liệu bằng thuốc, một sựđánh giá về các hệ thống thường liênquan tới việc giám sát và điều chỉnhhuyết áp là cần thiết.
Hai yếu tố xác định huyết áp là kếtquả thuộc về tim (thể tích đột quỵ xnhịp tim) và sức đề kháng ngoại vitổng thể của mạch máu Huyết ápđược điều chỉnh bởi sự tương tác giữahệ thần kinh, nội tiết và thận.
Huyết áp cao thường là do sự kết hợpcủa một số bất thường như căng thẳngtâm lý, di truyền, các yếu tố môitrường và chế độ ăn uống và nhữngyếu tố khác.
Những bệnh nhân không tìm thấynguyên nhân cụ thể của huyết áp tăngđược cho là có huyết áp tăng cần thiếthoặc tăng huyết áp cơ bản (chiếm 80-90% các trường hợp).

b Antihypertensive therapies
1 Non pharmacological therapy ofhypertension
Several non-pharmacologicalapproaches to therapy of hypertensionare available These include:
Low sodium chloride diet Weight reduction
The major advantage of pharmacological approaches is therelative safety and freedom from sideeffects, compared with drug therapy.
non-2 Pharmacological therapy ofhypertension
Most patients with hypertensionrequire drug treatment to achievesustained reduction of blood pressure.

Ưu điểm chính của phương pháp điềutrị không dùng thuốc này là độ an toàntương đối và không có tác dụng phụ sovới điều trị bằng thuốc.
2 Thuốc điều trị tăng huyết áp
Hầu hết bệnh nhân tăng huyết áp đềucần điều trị bằng thuốc để đạt đượcmức huyết áp ổn định Các loại thuốchiện có làm giảm huyết áp bằng cách

Anti-hypertensive drugs are classifiedaccording to the principal regulatorysite or mechanism on which they act.They include:
A Diuretics, which lower bloodpressure by depleting the body sodiumand reducing blood volume Diureticsare effective in lowering bloodpressure by 1-15 mmHg in mostpatients.
Diuretics include:
a) Thiazides and related drugs,
e.g., hydrochlorothiazidebendrofluazide, chlorthalidone, etc.Initially, thiazide diuretics reduceblood pressure by reducing bloodvolume and cardiac output as a resultof a pronounced increase in urinarywater and electrolyte particularlysodium excretion.
With chronic administration (6-8weeks), they decrease blood pressureby decreasing peripheral vascularresistance as the cardiac output andblood volume return gradually tonormal values.
Thiazides are appropriate for most
patients with mild or moderatehypertension and normal renal andcardiac fusion.
b) Loop diuretics, e.g furosemide,
ethacrynic acid, etc.
Loop diuretics are indicated in casesof severe hypertension which isassociated with renal failure or livercirrhosis.

Thiazide thích hợp cho hầu hết bệnh
nhân tăng huyết áp nhẹ hoặc trungbình và có phản ứng tổng hợp thận vàtim bình thường.
b)Thuốc lợi tiểu quai, ví dụ:
furosemide, axit ethacrynic, v.v.
Thuốc lợi tiểu quai được chỉ địnhtrong trường hợp tăng huyết áp nặngcó liên quan đến suy thận hoặc xơ gan.Thuốc lợi tiểu quai được chỉ địnhtrong trường hợp tăng huyết áp nặngcó liên quan đến suy thận, suy timhoặc sơ gan.

Methyldopa is useful in the treatmentmild to moderately severehypertension.
Methyldopa is a prodrug and must be
converted in the CNS to active α –methylnorepinephrine to exert theeffect on blood pressure.
The side effects of methyldopa includesedation, vertigo, dry mouth, nausea,vomiting, diarrhea, posturalhypotension, impotence, haemolyticanemia, weight gain andhypersensitivety reactions (fever, liverdamage, thrombocytopenia).
b) Adrenoceptor antagonists, e.g
propranolol (beta blocker), prazosin(alpha blocker), labetalol (alpha andbeta blocker).
c)Thuốc lợi tiểu giữ Kali,
ví dụ Spironolactone
Thuốc này được sử dụng kèm vớinhóm thiazides hoặc thuốc lợi tiểuquai để tránh sự suy giảm kali quámức và tăng cường tác dụng bài tiếtnatri của những thuốc khác Tác dụnglợi tiểu của nhóm thuốc này kém hơnkhi sử dụng một mình.
B) Thuốc liệt giao cảm
(thuốc ức chế hoạt động giao cảm)Dựa vào vị trí hoặc cơ chế tác dụng,thuốc liệt giao cảm được chia thành:a) Thuốc hạ huyết áp tác dụngtrung ương, ví dụ methyldopa,clonidine
Thuốc ức chế giao cảm tác động lêntrung tâm hoạt động bằng cách kíchthích thụ thể α2 – nằm ở trung tâm vậnmạch của hành tủy Kết quả là dònggiao cảm đi ra từ hành tủy bị giảm vàtổng sức cản ngoại biên hoặc cung
lượng tim đều giảm Methyldopa rất
hữu ích trong điều trị tăng huyết áp từnhẹ đến vừa.
Methyldopa là một tiền chất và phải
được chuyển hóa trong hệ thần kinhtrung ương thành α –methylnorepinephrine có hoạt tính đểtác dụng lên huyết áp.
Các tác dụng phụ của methyldopa bao
gồm: buồn ngủ, chóng mặt, khômiệng, buồn nôn, nôn, tiêu chảy, hạhuyết áp tư thế, giảm ham muốn tìnhdục, thiếu máu tán huyết, tăng cân vàcác phản ứng quá mẫn (sốt, tổn thươnggan, giảm tiểu cầu).
b) Thuốc đối kháng thụ thểadrenoceptor, ví dụ propranolol
(thuốc chẹn beta), prazosin (thuốcchẹn alpha), labetalol (thuốc chẹnalpha và beta).
β – Thuốc chẹn đối kháng beta, thụ thểnằm trên cơ tim và ngăn chặn nhịp tim

The principal action of adrenergic blocking drugs is toproduce peripheral vasodilation Alpha blockers reduce arterialpressure by dilating both resistanceand capacitance vessels Treatmentwith prazosin should be initiated withlow dose (1mg 3 time daily) to preventpostural hypotension and syncope orbe given at bedtime.
alpha-c) Adrenergic neuron – blockingagents, e.g guanethidine
Guanethidine is an adrenergic blocking drug recommended fortreatment of severe forms ofhypertension.
neuron-Guanethidine blocks adrenergic nervetransmission, preventing the release oftransmitter It lowers blood pressureby reducing both cardiac output andtotal peripheral resistance.
d) Drugs which deplete catecholaminestores,
e.g reserpine.
Reserpine interferes with storage of
endogenous catecholamines in storage

c) Tác nhân ức chế tế bào thần kinhAdrenergic Ví dụ : guanethidineGuanethidine là một loại thuốc ức chế
tế bào thần kinh adrenergic đượckhuyên dùng để điều trị các dạng tănghuyết áp nặng.
Guanethidine ngăn chặn sự dẫn truyền
thần kinh adrenergic, ngăn chặn sựgiải phóng chất dẫn truyền Nó làmgiảm huyết áp bằng cách giảm cunglượng tim và tổng sức cản ngoại biên.
d) Thuốc làm cạn kiệt catecholaminedự trữ
Ví dụ: reserpine.

C) Direct vasodilators
These include:
Arterial vasodilators, e.g hydralazine
Arteriovenous vasodilators, e.g sodium nitroprusside
Hydralazine: It dilates arterioles but
not veins It is used particularly insevere hypertension The mostcommon adverse effects are headache,nausea, anorexia, palpitations,sweating and flushing which aretypical to vasodilators.
Sodium nitroprusside: It is a powerful
vasodilator that is used in treatinghypertensive emergencies as well assevere cardiac fairlure.
It dilates both arterial and venousvessels, resulting in reduced peripheralvascular resistance and venous return.
Nitroprusside rapidly lowers blood
pressure and it is given by intravenousinfusion.
The most serious toxicities includemetabolic acidosis, arrhythmias,excessive hypotension and death.
D) Angiotensin converting enzymeinhibitors, e.g captopril, enalapril, etc The prototype is captopril Captopril

C) Thuốc giãn tĩnh mạch trực tiếp
Hydralazine: nó làm giãn động mạch
nhưng không làm giãn tĩnh mạch Đặcbiệt nó được sử dụng trong bệnh caohuyết áp nặng Các tác dụng phụthường gặp nhất là đau đầu, buồn nôn,chán ăn, tim đập nhanh, đổ mồ hôi vàđỏ mặt – đó là tác dụng phụ điển hìnhcủa thuốc giãn mạch.
Sodium nitroprusside: là một loại
thuốc giãn mạch mạnh được sử dụngtrong các trường hợp tăng huyết ápcấp cứu cũng như suy tim nặng.
Nó làm giảm cả động mạch và tĩnhmạch, dẫn đến làm giảm sức cản mạchmáu ngoại biên và hồi lưu tĩnh mạch.
Nitroprusside làm giảm huyết áp
nhanh chóng và được dùng bằngđường truyền tĩnh mạch.
Các độc tính nghiêm trọng nhất baogồm nhiễm, rối loạn nhịp tim, hạhuyết áp quá mức và tử vong.
D) Thuốc ức chế men chuyểnangiotensin, ví dụ như captopril,
enalapril, v.v

chế enzyme chuyển hóa angiotensinthủy phân angiotensin I (Không hoạtđộng) thành angiotensin II (Hoạtđộng)- một chất gây co mạch mạnh,ngoài ra nó còn kích thích tiết

The adverse effects includemaculopapular rash, angioedema,cough, granulocytopenia anddiminished taste sensation.
Enalapril is a prodrug with effects like
those of captopril.
E) Calcium channel blockers, e.g.
nifedipine, verapamil, nicardipine, etc.The prototype is verapamil
The mechanism of action inhypertension is inhibition of calciuminflux into arterial smooth musclecells, resulting in a decrease inperipheral resistance.
Verapamil has the greatest cardiac
depressant effect and may decreaseheart rate and cardiac output as well.The most important toxic effects forcalcium channel blockers are cardiacarrest, bradycardia, atrioventricularblock, and congestive heart failure.
Lines of treatment of primaryhypertension
The initial step in treatinghypertension may be non-pharmacologic Dietary salt restrictionmay be effective treatment for abouthalf of the patients with mildhypertension Weight reduction evenwithout salt restriction normalizesblood pressure in up to 70% of obesepatients with mild to moderatehypertension Regular exercise mayalso be helpful in some hypertensivepatients.
When non-pharmacologic approachesdo not satisfactorily control bloodpressure, drug therapy begins inaddition to non-pharmacologicalapproaches.

Cơ chế tác dụng trong tăng huyết áp làức chế dòng canxi đi vào tế bào cơtrơn động mạch, dẫn đến giảm sức cảnngoại biên.
Verapamil có tác dụng giảm đau tim
lớn nhất và có thể làm giảm nhịp timcũng như cung lượng tim.
Tác dụng độc hại hệ trọng nhất củathuốc chẹn kênh canxi là ngừng tim,nhịp tim chậm, block nhĩ thất và suytim sung huyết.
Các phương pháp điều trị tăng huyếtáp nguyên phát

Thuốc chống loạn nhịp tim được sửdụng để ngăn ngừa hoặc điều chỉnhchứng loạn nhịp tim (nhịp tim nhanh).Thuốc được sử dụng trong điều trị rốiloạn nhịp tim thường được phân loạithành:
Loại (I): Thuốc chẹn kênh natri baogồm quinidine, lidocaine, phenytion,flecainide, v.v.
Loại (II): Thuốc chẹn beta adrenergicbao gồm propanolol, atenolol, v.v.Loại (III): Thuốc chẹn kênh kali, vídụ: amiodarone, bretylium.
Loại (IV): Thuốc chẹn kênh canxi, vídụ: amiodarone, bretylium.

Adverse effects: Hypokalemia, nausea,
anorexia, vomiting epigastric distress,fatigue weakness muscle cramps,drowsiness Dizziness, hearingimpairment and deafness are usuallyreversible Therapeutic uses: acutepulmonary edema, edema of cardiac,hepatic and renal disease.Hypertension, cerebral edema, in drugoverdose it can be used to produceforced diuresis to facilitate more rapidelimination of drug.
III Potassium sparing diureticsmechanism of action: Potassiumsparing diuretics (spironolactone,triamterene, amiloride) are milddiuretics causing diuresis byincreasing the excretion of sodium,calcium and bicarbonate but decreasethe excretion of potassium

nôn, chán ăn, nôn mửa đau thượng vị,mệt mỏi yếu cơ chuột rút, buồn ngủ.Chóng mặt, suy giảm thính lực và điếcthường có thể hồi phục Công dụngchữa bệnh: phù phổi cấp, phù nề cácbệnh về tim, gan, thận Tăng huyết áp,phù não, khi dùng thuốc quá liều cóthể dùng để tạo ra thuốc lợi tiểu cưỡngbức nhằm tạo điều kiện đào thải thuốcnhanh hơn.
III Cơ chế tác dụng của thuốc lợi tiểugiữ kali: Thuốc lợi tiểu giữ kali(spironolactone, triamterene,amiloride) là thuốc lợi tiểu nhẹ gây lợitiểu bằng cách tăng bài tiết natri,canxi và bicarbonate nhưng làm giảmbài tiết kali.

V Osmotic diuretics: these drugs likemannitol and glycerin (glycerol) arefreely filtered at the glomerulus andare relatively inert pharmacologicallyand undergo limited reabsorption byrenal tubule These are administeredto significantly increase theosmolality of plasma and tubularfluid Sometimes they producenausea, vomiting, electrolyteimbalances They are used in cerebraledema and management of poisoning.
Drugs used in hypotensive states andshock.
Antihypertensive drugs or agents areused to elevate a low blood pressureand may be classified as follows:I Agents intended to increase thevolume of blood in active circulation.These include intravenous fluids suchas whole blood, plasma, plasmacomponents, plasma substitutes andsolution of crystalloids
II Vasoconstrictor drugs theseinclude:
• Peripherally acting vasoconstrictors

IV Thuốc ức chế anhydrase carbonic:những loại thuốc này nhưacetazolamide ức chế enzymecarbonic anhydrase trong tế bào ốngthận và dẫn đến tăng bài tiết các ionbicarbonate, natri và kali trong nướctiểu Trong mắt nó dẫn đến giảmthông tin của thủy dịch Vì vậy chúngđược sử dụng trong điều trị bệnh tăngnhãn áp góc cấp tính Tác dụng phụchính của các thuốc này là buồn ngủ,hạ kali máu, nhiễm toan chuyển hóavà đau vùng thượng vị.
V Thuốc lợi tiểu thẩm thấu: nhữngthuốc như mannitol và glycerine(glycerol) được lọc tự do ở cầu thậnvà tương đối trơ về mặt dược lý vàđược tái hấp thu hạn chế ở ống thận.Chúng được dùng để tăng đáng kể độthẩm thấu của huyết tương và dịchống thận Đôi khi chúng gây buồnnôn, nôn mửa, mất cân bằng điện giải.Chúng được sử dụng trong chứng phùnão và kiểm soát ngộ độc.

vasodilator drugs (nitroprusside,
nitroglycerine) may be given along
with the cardiotonic drug Diuretics
may also be indicated to treatpulmonary congestion if it occurs.- For anaphylactic shock or neurogenicshock characterized by severevasodilation and decreased PVR, avasoconstrictor drug (e.g.levarterenol) is the first drug of choice.- For hypovolemic shock, intravenousfluids that replace the type of fluid lostshould be given.

N.B Their topical use is not
