thuốc tính nhạy cảm với hành vi hút thuốc học sinh số trường hệ THPT địa bàn thành phố Hà Nội năm 2019 Tóm tắt tiếng Việt Đặt vấn đề mục tiêu nghiên cứu Hút thuốc hành vi phổ biến Việt Nam, nhiều sách ban hành thực hai thập kỷ qua Các nghiên cứu tỷ lệ sử dụng thuốc niên, thiếu niên Bộ Y tế cho thấy, tỷ lệ hút thuốc niên, thiếu niên Việt Nam tăng độ tuổi bắt đầu hút thuốc ngày trẻ Tính nhạy cảm với hành vi hút thuốc định nghĩa thiếu cam kết chắn khơng hút thuốc, đó, hữu ích cho việc dự đốn người trẻ tuổi có nguy trở thành người hút thuốc tương lai Có nhiều nhóm yếu tố liên quan tới nhạy cảm với hành vi hút thuốc vị thành niên/thanh niên bao gồm: đặc điểm cá nhân, gia đình, trường học, mơi trường xung quanh Nghiên cứu tiến hành nhằm mục tiêu: (1) Mô tả mật độ điểm bán thuốc tính nhạy cảm với hành vi hút thuốc học sinh số trường hệ THPT thuộc địa bàn thành phố Hà Nội năm 2019 (2) Xác định yếu tố liên quan đến tính nhạy cảm với hành vi hút thuốc đối tượng nghiên cứu H P Đối tượng nghiên cứu phương pháp nghiên cứu Đây nghiên cứu cắt ngang tiến hành 3272 học sinh theo học 14 sở giáo dục hệ THPT (công lập/dân lập/TTGDTX) địa bàn quận/huyện thuộc thành phố Hà Nội lựa chọn ngẫu nhiên Biến số đầu nghiên cứu tính nhạy cảm với hành vi hút thuốc Các biến số độc lập thu thập nghiên cứu bao gồm: thông tin cấp độ cá nhân, việc phơi nhiễm với thuốc lá, đặc điểm trường học đối tượng (thu thập phương pháp phát vấn câu hỏi), thông tin điểm bán thuốc (thu thập thực địa máy tính bảng) U H Kết phát Tỷ lệ đối tượng nghiên cứu có nhạy cảm với hành vi hút thuốc 4,9% (KTC 95%: 4,2 – 5,8%) Các yếu tố có liên quan tới nhạy cảm với hành vi hút thuốc tìm qua nghiên cứu bao gồm: giới tính (ORnam=2,79 (2,29 – 3,41)), tuổi (OR16-17=1,25 (1,01 – 1,56), OR18+=1,59 (0,86 – 2,93)), khu vực sống (ORquận=1,41 (1,15 – 1,74)), nhìn thấy người thân nhà hút thuốc (OR=1,39 (1,14 – 1,70)), nhìn thấy bạn bè hút thuốc (OR=2,33 (1,88 – 2,89)), nhìn thấy thầy hay nhân viên khác trường hút thuốc với OR 1,89 (1,23 – 2,63) 1,63 (1,17 – 2,26), tham gia tiết học PCTH thuốc (OR=1,31 (1,06 – 1,62)), tiếp cận với thông điệp PCTH thuốc (OR=1,28 (1,005 – 1,63), khoảng cách TB từ cổng trường tới điểm bán thuốc gần (OR=1,01 (1,005 – 1,02) Các yếu tố chưa có chứng mối liên quan với việc nhạy cảm với hành vi hút thuốc là: việc nhìn thấy biển cấm hút thuốc trường, tiếp cận với quảng cáo thuốc lá, số điểm bán thuốc bán kính 100m xung quanh cổng trường Kết luận khuyến nghị Phòng chống hút thuốc nên vấn đề ưu tiên y tế công cộng Điều quan trọng chương trình phịng chống tác hại thuốc cho thiếu niên nên bắt đầu sớm, đặc biệt khu vực trường học, điều cần thiết nhằm làm gián đoạn nhạy cảm thiếu niên với hành vi hút thuốc Các can thiệp thuốc tương lai cần đưa biện pháp mạnh mẽ việc giảm thiểu việc hút thuốc người thân gia đình bạn bè vị thành niên để giảm thiểu nguy nhạy cảm với hành vi hút thuốc thiếu niên Cuối cùng, cần thực thi tốt Luật Phòng chống tác hại thuốc đảm bảo điểm bán thuốc cần phạm vi 100m xung quanh sở giáo dục nói chung, khơng sở giáo dục hệ THPT H P U H Tóm tắt tiếng Anh Background and objectives Smoking is a common behavior in Vietnam, despite of many policies has been enacted and implemented in the past two decades Studies on the prevalence of tobacco use among youth and adolescents by the Ministry of Health show that the prevalence of smoking among Vietnamese youth and teenagers is increasing and the age at which they start smoking is increasingly younger Smoking susceptibility is defined as the lack of a firm commitment to not smoking, so it is useful to predict young people who are at risk of becoming smokers in the future There are many groups of factors related to adolescent/youth susceptibility to smoking, including: personal characteristics, family, school, and surroundings factors This study is conducted for two objectives: (1) Describe the density of tobacco point-of-sale and the smoking susceptibility among students of some high schools in Hanoi City in 2019 and (2) Identify factors related to smoking susceptibility among study subjects Materials and method H P This is a cross-sectional study conducted on 3272 students currently enrolled in 14 educational institutions of high school system (public/private/education center) in districts of Hanoi city All educational institutions and participants were selected randomly The main output variable in this study is smoking susceptibility The independent variables collected in this study include information at the individual level, exposure to tobacco, subject's school characteristics (collected by self-interview questionaire), information on tobacco point-of-sale locations (collected in the field using tablets) U Results The percentage of smoking susceptibility in this study was 4.9% (95% CI: 4.2 - 5.8%) Factors related to smoking susceptibility include: gender (ORmale = 2.79 (2.29 - 3.41)), age (OR16-17 = 1.25 (1.01 - 1.56), OR18+ = 1.59 (0.86-2.93)), living area (ORdistrict = 1.41 (1.15 1.74)), see relative smoking (OR = 1.39 (1.14 - 1.70)), see friends smoking (OR = 2.33 (1.88 2.89)), see teacher or other staffs smoking in school with OR=1.89 (1.23 – 2.63) and 1.63 (1.17 – 2.26), respectively; take lessons about harmful of tobacco use (OR = 1.31 (1.06 1.62), reach to messages about harmful of tobacco use (OR=1.28 (1.005-1.63), the average distance from the school gate to the nearest tobacco point-of-sale (OR = 1.01 (1.005 - 1.02) Some factors which were not statistical significant with smoking susceptibility in this study were: see smoking ban sign in school, reach to tobacco advertising, and the number of tobacco point-of-sale in the 100m radius around schools H Conclusions and recommendations Tobacco control should be a priority issue in public health It is important that tobacco harm prevention programs for adolescents should start early, especially in school settings, which is necessary to disrupt adolescent smoking susceptibility Future tobacco interventions should provide even more robust measures to reduce the smoking of adolescent family members and friends to minimize the risk of smoking susceptibility among teenagers Finally, better implementation of the Tobacco Control Law is needed to ensure that tobacco point-of-sale 8 KẾT LUẬN VÀ KHUYẾN NGHỊ Tỷ lệ đối tượng nghiên cứu có nhạy cảm với hành vi hút thuốc 4,9% (KTC 95%: 4,2 – 5,8%) Các yếu tố có liên quan tới nhạy cảm với hành vi hút thuốc tìm qua nghiên cứu bao gồm: giới tính (ORnam=2,79 (2,29 – 3,41)), tuổi (OR16-17=1,25 (1,01 –
