Safety and health are very important element of the project - The contrctor will continuosly implement safety measures regarding the public and alll personnel associated with the works - The contractor will perform the work in full compliance with all safety and industrial health legislation and regulations of the Socialist Republic of Vietnam - The Contractor believes that safety is a ine responsibility and expects individual employees to be responsible for his own safety and health and the protection of his fellow employees as well as the environment - The Contractor believes that all accidents are proventable and no undertaking is so important that would endanger the life of a person Regarding preventive measures, the Contractor will provide the proper safety devices such as safety handrails, safety nets, barricades, warning signs, watchman etc - The Contractor will also display safety and industry health posters to raise the awareness of workers - The Contractor will maintanin the existing pedestrian routes in a safe condition or provide alternative routes with the intension of seuring safety of the local population - The Contrctor believes that time and effort shall be spent for the advancement of any operations and undertakings - The Contractor will provide fully cooperate and assist in safety surveillance which will be carried out by the Engineer or the Employer 1.2.2 Statutory regulations and clauses that the Contractor will apply: - Health protection law for people issued in - Labour law of the Socialist Republic of VietNam approved by the Committee of the National Assembly date 22/06/1994 and coming into force on 01/04/1995 - Fire prevention and fighting law issued in 2000 - Labor safety law issued in 2002 - The Government decree No.06 about safety and labor hygiene - The Government decree No.195 about work and rest time - The Government decree No.38 about penalties for those who offends labor safety regulations - The Government decree No.13 about intensitying labor safety - VietNam road sign regulation (TCN 237-01) - Environment protection law Consultant: SMEC International Contractor: VINACONEX.,JSC Rehabilitation of National Highway No.38B-2, Trang Bridge – Hung Yen Section Package No.8 (NIP/CP-8) - Decree No.175/CP dated 18/10/1994, directing to carry out the environment protection law - Circular of the Ministry of Science, Technology and Environment No.490/1998/TT-BKHCNMT dated 29/04/1998, directing to make and assess a report, analyzing environmental effects of investment projects - Decision No 08/2001/QĐ-TTg approved by prime Minister about Management of productive forests, protected forest and particular forests dated 11/02/2001 Besides the Government regulations, the Contractor will date the EMSTCP and apply other regulations, local regulations, local customs and other instructions from Government Agencies, including: - The use of cement from the dredging of off-shore coral reef resources is prohibited and shall not be used in the project; - Although there are no known instances of unexploded ordnance (UXO) within the proposed work Sites, the possibilities of UXO can not be excluded The Contractor shall, therefore, undertake consultations, historical reviews and surveys as warranted in regard to potential UXO issues as determined necessary in consultation with the Supervising Engineer - Before a contractor can begin work, the contractor shall submit to PMU-18 ESU approval from the relevant Provincial Department of Science, Technology and the Environment (PDOSTE) that all borrow materials from river floodplains have been subject to environmental studies approved by MONRE or the relevant PDOSTE - Contractor shall restore areas sitting temporary asphalt plants to their original condition on completion of Project works This shall include the clean-up of spillage and debris before leaving temporary construction sites In addition to the above, when there are any requirements from the Engineer for project safety, these will be met by the contractor 1.3 Contractor Management 1.3.1 Updating information: Regulations and documents relating to industrial health will always be observed, updated and made known to all site staff The approved environmental management, safety and traffic control plan will be furnished to subcontractors to be considered as safety and environment protection documents The subcontractors are requested to conform to these plans 1.3.2 Management: Sub-contractors are requested to assign a safety representative, who will be on site at all time during the subcontractor’s execution of the works The contractor will organize safety and environmental training programs for all site staff - regular program (6month/time): for staff, inspectors, managers The contractor will organize regular monthly meetings regarding safety and environmental issues to remind and inform all site staff The contractor will organize inspections, testing and maintain safety equipment on site Consultant: SMEC International Contractor: VINACONEX.,JSC Rehabilitation of National Highway No.38B-2, Trang Bridge – Hung Yen Section Package No.8 (NIP/CP-8) ITEM 2.0.ORGANIZATION AND STAFFING Consultant: SMEC International Contractor: VINACONEX.,JSC Rehabilitation of National Highway No.38B-2, Trang Bridge – Hung Yen Section Package No.8 (NIP/CP-8) 2.1 Organization Chart ENVIRONMENT SAFETY & TRAFFIC PROTECTION OFFICER LE XUAN DUC TEL:0978054667 DEPUTY OF ENVIRONMENT SAFETY & TRAFFIC PROTECTION LE XUAN DUC TEL: ROAD CONSTRUCTION TEAM Consultant: SMEC International BRIGE CONSTRUCTION TEAM Contractor: VINACONEX.,JSC Rehabilitation of National Highway No.38B-2, Trang Bridge – Hung Yen Section Package No.8 (NIP/CP-8) REGULATIONS PERTAINING TO FIRE PREVENTION AND FIRE FIGHTING Clause 1: Preventing and fighting fires is every one’s duty Clause 2: Everyone should be aware of fire prevention, Regulations and Equipment for fighting fire should be at hand when necessary Clause 3: Being careful when using fire, heat sources, chemical and explosives, poisons, and radio active and material Abiding with all regulations Clause 4: Electric lines will not be strung randomly, Electric equipments will be checked before beginning works materials will not be stored near light bulbs or electric wires Clause 5: Materials and equipments should be arranged tidily and kept a safe distance apart Workers shall guard against fires and fight fires if necessary It is forbidden to use keys or steel objects to open petrol covers Clause 6: It is forbidden to start engines in areas which contain inflammable materials while dispatching and receiving materials, goods, and vehicles should be parked so that they are facing of the site Clause 7: Objects should not block emergency exits Clause 8: Any groups or personal who engage in fire fighting will be rewarded If any one does not adhere with the above regulation, they will receive administrative penalties Consultant: SMEC International Contractor: VINACONEX.,JSC Rehabilitation of National Highway No.38B-2, Trang Bridge – Hung Yen Section Package No.8 (NIP/CP-8) B Basic & Proper Work Attire - Safety Helmet - Safety Belt - Shirt - Long Pants - Shoes Site workers shall be allowed entry in the construction site unless wearing the proper work attire C Following are serious violations and subject to outright suspensions or termination : -  Fighting -  Possession of firearms, deadly weapons and explosives -  Entering work area under the influence of liquor, prohibited drugs and/ or other intoxicating substances -  Others D Traffic -  Maximum speed limit in the construction site is 15 km/h -  No personnel shall be allowed to ride on open truck beds or together with material and equipment -  No overtaking in the construction site 2/ Work Plan -  Determine the work method and work plan -  Work schedule (number of the days) -  Number and kind of workers and equipment -  Safety problems -  Coordination with related trades and related contractors -   For important operations and very dangerous operations, prepare a written work plan When an operation is to be performed with subcontractors, fully coordinate with all the subcontractors before starting -  Place -  Date & time -  Nature of the operation -  Safety problems 3/ Things the Subcontractor’s Site Manager/Site Engineer should -  Hold group meeting -  Check clothing and protective devices of each worker Consultant: SMEC International Contractor: VINACONEX.,JSC Rehabilitation of National Highway No.38B-2, Trang Bridge – Hung Yen Section Package No.8 (NIP/CP-8) -  Check condition of health of each workers -  Make proper work assignment of workers -   Observe how the work is conducted (according to plan or not) Do not allow unsafe conduct -  Attend schedule meetings and safety meetings - Supervise and direct workers to follow the plan 4/ Group meeting before starting work Before starting work, hold a group meeting with all workers in attending -  Explain the nature of the operation and the work plan -  Explain the role of each worker (proper work assignment) -  Confirm the qualification and skill of each worker -  Ditermine safety problems by danger searching activity -  Check clothing and protective devices of workers -  Check physical condition of workers (hangover, lack of sleep, etc.) 5/ Worker’s Clothing  Observe the following rules - Close the sleeve cuffs on shirts - Do not work without a shirt - Do not work wearing sandals - Do not put a towel around your neck or under your belt - Always wear a safety helmet 6/ Electrical Safety - Leave electrical work to the electrician and not try to it yourself - Do not place objects near the overhead electrical power lines or distributed panels - When working near overhead electrical power lines, provide an experienced lookout and follow his signals - When moving an electrical appliance, first open the switch Do not move an electrical appliance by pulling on the electrical cord - When opening or closing a switch, make and confirm signals clearly and act with caution - If you find anything wrong with electrical equipment and appliances, inform the electrician and get him to take care of the problem - The ground wire is an important part of equipment Do not disconnect it - Always inspect electrical appliances before use Is the electrical cord damaged in any way? Is the appliance wet? - Protect against electrical shock by using the ground fault interrupters and grounds Consultant: SMEC International Contractor: VINACONEX.,JSC Rehabilitation of National Highway No.38B-2, Trang Bridge – Hung Yen Section Package No.8 (NIP/CP-8) - Protect against electrical shock by suing protective sheaths and insulating protectors High voltage overhead power lines, use protective sheaths Working near high voltage lines, use insulated tools and protectors 7/ Entry to Enclosed Space The Enclosed Space Entry Procedures establishes the essential precautionary measures, necessary for persons to enter enclosed spaces to perform any work The term Enclosed Space shall include: - All tan, vessels - Pits, trenches, sewers and wells - Duct works and ceilings The Enclosed Space Entry Procedures requires that each and every enclosed space shall be assessed for Hazards Potentials by consideration and assessment of the following: - Flammable gases - Asphyxiates - Explosive dusts - Hazardous chemicals/ materials - Thermal stress conditions - Vertical and maze access and egress - Unstable solids and overhead scale - Dusts General Safety Precaution when working in confined space The following are the hazards that need to be considered before entering a confined space: - Oxygen Deficiency The concentration of oxygen in normal air is approximately 21% volume As this concentration decreases (due to the gas present, welding fumes, etc.) breathing becomes increasingly difficult, until level is reach where human life cannot sustain In order to determine level of oxygen or presence of gas or flammable vapors inside a confined space, a device, Gas Detector with oxygen level indicator may be used - Temperature Condition The temperature inside a confined space can be greatly influenced by the ambient (environment) temperature The need to wear the required personal protective equipment/ clothing inside a confined space can also cause rapid increase in body temperature, eventually leading to head stress - Noise Excessive noise may arise from hammering, grinding works, etc appropriate hearing protection should be provided Consultant: SMEC International Contractor: VINACONEX.,JSC Rehabilitation of National Highway No.38B-2, Trang Bridge – Hung Yen Section Package No.8 (NIP/CP-8) Precautionary measures: - Confirm from Safety Officer that work permit had been approved and all gas tests had been carried out and oxygen concentration is within safety level or 21% - Ensure a confined space is adequately illuminated and ventilated - Ensure that at least two persons will be working together, one, stationed outside or at the entrance of the man-way keeping close communication with the person inside He may also be provided with an air supply respirator for emergency use Use of a lifeline must also be considered The worker inside a confined space shall wear a safety belt and connected to a lifeline that extend to the exit In case the worker collapse (may be due to accumulation of gas) inside a confined space, the worker outside can easily rescue the victim without going inside and endangering himself If this is not possible, SCBA shall be used - No oxygen/acetylene tan shall be kept inside an confined space - Turn of machines and equipment before disembarking a confined space - Signs shall be placed outside a confined space to caution people on the ongoing works inside a confined space - No sleeping or taking short nap inside a confined space is permitted - No smoking inside a confined space is permitted - Low voltage lighting and equipment must be used 8/ Emergency/ Evacuation Procedure a.) Injury Treatment - First Aid: A first aid facility equipped with necessary medicines, medical supplies and equipment shall be provided on site - Medical Treatment: Injuries that would require further treatment shall be referred to the JV clinic first for further evaluation A driver and a service vehicle shall be on stand by for such emergencies - Safety Supervisor should obtained and keep posted the following information: - List of near-by hospitals/ clinics - Address - Telephone Numbers - Location maps b.) Floods Floods normally follow heavy rains brought about by typhoon or monsoon rains When flood water starts to rise, secure all materials, which will be spoiled by floodwater c.) Fire In case a fire breaks out, and assessed to be manageable, the fire watchers shall immediately put off the fire using the available fire fighting equipment at site and report the incident immediately Consultant: SMEC International Contractor: VINACONEX.,JSC Rehabilitation of National Highway No.38B-2, Trang Bridge – Hung Yen Section Package No.8 (NIP/CP-8) If unmanageable, safety personnel concerned shall immediately inform security guard to alarm the Fire Brigade Team All workers shall be evacuated at once or proceed to assembly points d.) Emergency Procedure in Confined Space It is recommended that the cleaning crew demonstrate their escape procedures before starting the cleaning operations As a minimum, the safety observer must have an air horn plus a spare charged air cylinder to raise the alarm should compressor failure occur Methods of signaling such as tugging airlines or lifelines can easily be mistaken due to the frequency of false alarms triggered by normal movement The preferred method is of teams working in sight of each other The custodian must satisfy himself that a practicable system of alarming is in operation 9/ Fall Protection a.) Safety Belts / harness Workmen working on unguarded surfaces, above open pits, steep slopes, moving machinery and similar locations or working from unguarded surface six (6) feet or more above the ground on temporary or permanent floor platform, scaffold construction shall be secured by safety belts/ harness In situations where safety belts are not feasible, safety nets shall be provided and installed b.) Ladders a Ladders must be inspected and approved by the Safety supervisor or the contractor before use b Painted ladders are not permitted c While ascending or descending a ladder, hold a straight ladder with both hand and hold a ladder with at least one hand Use a hand-line if necessary to raise and lower materials d Except when climbing, keep both feet on the ladder rungs e Always face the ladder when climbing and descending f Only one man is allowed on the ladder, unless two man stepladders are used g If it is necessary to use a ladder on top of a scaffold, the ladder must be tied off and the use must wear a safety belt h During the execution of work from the ladder, the top should be tied off i Straight and Extension ladder shall be equipped with non-slip base j They must tightly secured on the top k Top of ladder must extend at least rungs beyond the supporting oject when used as access to an elevated work area 10/ Illumination / Lighting Poor lighting contributes to construction accidents All places where person work or pass shall be provided with adequate natural lighting or Consultant: SMEC International Contractor: VINACONEX.,JSC Rehabilitation of National Highway No.38B-2, Trang Bridge – Hung Yen Section Package No.8 (NIP/CP-8) artificial lighting or both, suitable for the operations and the special type of work performed Artificial light shall be adequate at the place of work for the operations or work performed: Minimum Illumination Intensifies in Foot Candles Foot Candles Area of Operation General construction area lighting General construction areas, concrete placement, excavation and waste areas, access ways, active storage areas, loading platforms, refueling and field maintenance areas Indoors: Warehouses, corridors, hallways and exit ways Tunnels, shafts and general underground work areas: (exception: Minimum of 10-foot candles is required at tunnel and shaft heading during drilling, mucking and scaling Bureau of mines approved cap lights shall be acceptable for use in the tunnel heading) 10 General construction plant and shops (e.g batch plant, screening plants, mechanical and electrical equipment rooms, carpenter shops rigging lifts and active storerooms, barracks or living quarters, lockers or dressing rooms, mess halls and inddor toilets and workrooms) 30 First aid stations, infirmaries and offices Note: Foot Candle = 10.75 lux (for purpose of computation use foot candle = 10 lux) Formula: lux = lumens ⁄ Square meters 11/ Signage The signs indicating the specific dangers or hazards are required so that the people (directly or not directly) working in the area may protect themselves The most common signs are; “Danger”, “Caution”, and “Notice” Danger Signs: These are used only when and where an immediate hazard exists The predominate color is red Caution: Cautions signs are to be used only to warn against Potential hazards or to cautions against unsafe practices The predominate color is yellow Notice signs: These are for general information usage only They are White with a blue upper portion Consultant: SMEC International Contractor: VINACONEX.,JSC Rehabilitation of National Highway No.38B-2, Trang Bridge – Hung Yen Section Package No.8 (NIP/CP-8) Safety Instruction signs: Safety instruction signs shall be white with green upper panel 5.7 BREACHES OF SITE SAFETY PLAN Any Breaches of the Safety Plan or the statutory regulations or disregard for the safety of any persons may be the reason for the Engineer or Employer to exercise his authority to require the Contractor’s employee, Subcontractor’s employee and or the Contractor’s Project Manager’s removal from the Site 5.8 SAFETY MEETING To maintain interactive communication between construction personnel and safety staff, a safety meeting will be arranged once a month During the meeting, site safety report will be submitted and examined The safety meeting will be carried out with regard to the Contractor’s safety policy as described in the first section of this plan The safety meeting will be conducted regularly The meeting is a sort of a safety reminding process regarding the existing potential hazards in workplace It is one way of behavioral modification and allowed workers to interact issues and concerns on safety in the job site The safety meeting is designed to improve and enhance the actual safety and security condition of the workplace It would help safety department to initiate reforms and innovations to cater the necessity and problems of the works The company management has the full access on the relevant issues and concern discussed during the safety meeting and initiate immediate actions and remedies to improve the quality of safety and create better working environment for the employees The Project Manager and staff and all site personnel are required to attend the safety meeting It is preferable that the venue of the safety meeting shall be within the vicinity of the workplace 5.9 SAFETY EQUIPMENT AND CLOTHING The Contractor ensure that safety equipment and protective clothing available on site and that measures for the effective enforcement of proper utilization and necessary replacement of such equipment and clothing are incorporated into the Site Safety Plan The Contractor will provide all persons legitimately on site with protective equipment and clothing Safety Item for equipment and clothing will be but not limited to; Safety Helmet Safety Shoes Safety harnesses Safety gloves Eye protectors Dust respirator Fire extinguishers Consultant: SMEC International Contractor: VINACONEX.,JSC Rehabilitation of National Highway No.38B-2, Trang Bridge – Hung Yen Section Package No.8 (NIP/CP-8) First Aid equipment 5.10.SAFETY INSPECTION The Contractor will conduct Safety Inspection safety equipment such as scaffoldings, temporary handrails/guardrails, hoists, ladders and other temporary construction safety facilities including the warning signs and caution lights or so on to ensure these item of equipment and facilities are in good working order Upon recognition of deficiency, the items are to be immediately fixed or replaced, all personnel working such area are to be notified the condition and instructed what to / not to until the item to be rectified and what additional precaution measure to be taken if necessary 5.11 FIRST AID BASE The Contractor will prepare First Aid Base within the principal work area and the Base will be facilitated with the Equipment and apparatuses and others first aid devices, which are itemized in special specifications They shall include but not limited to; Satelite Office with fresh water Sterilizing equipment Medicine box Recovery room with chairs and footrests 5.12.PLANT AND EQUIPMENT The Contractor will provide proper Safety Devices with all construction plant and equipment, which will be used for the work These Safety Devices will be installed by qualified personnel and inspected regularly In case of deficiency are recognized in any of these, the Contractor will cease the operation and take immediate action to rectify the situations Safety Devices will include but not limited to; - Safety Catches for crane hooks and other lifting devices - Warning Devices for over lifting, over loading, outrigger condition for crane The Contractor will ensure the all construction plant and equipment used for this project will be operated by suitably qualified personnel 5.13.NOTIFICATION OF ACCIDENT The Contractor will notify the Engineer immediately when any accidents occur whether on-site or off-site in which the Contractor, his personnel or Construction Plant, or those of the Contractor’s subcontractors are directly or indirectly involved and which result in any injures to any persons Such initial notification may be verbal and will be followed by a written comprehensive report within 24 hours of the accident Consultant: SMEC International Contractor: VINACONEX.,JSC Rehabilitation of National Highway No.38B-2, Trang Bridge – Hung Yen Section Package No.8 (NIP/CP-8) PROTECTION OF TRAFFIC 6.1 GENERAL REQUIREMENTS The Contractor shall keep open to traffic existing roads during the performance of the Works, provided that when approved by the Engineer The Contractor may bypass traffic over a detour The Contractor shall at all times keep roads and footpaths, traffic or buildings and at locations directed by the Engineer During non-working hours and following completion of a particular construction operation, all warning signs, except those necessary for the safety of the public, shall be removed or entirely covered with either metal or plywood sheeting so that the sign panel will not be visible Retro-reflective sheeting on signs, barricades, and other devices shall be kept clean The Contractor shall promptly correct stretches, rips, and tears in the sheeting Retro-reflective sheeting shall be at all time maintained retro-reflection characterist Nighttime operations shall be illuminated by a lighting system approved by the Engineer The lighting system shall be positioned and operated to preclude glare Incandescent lights will not Retro-reflective Material: The reflective sheeting used on the road signs shall consist of spherical lens elements embedded with in a transparent plastic having a smooth, flat surface with a protected precoat adhesive which shall be pressure sensitive for manual application or tack free heat activated for mechanical vacuum-heat application The reflective sheeting shall be sufficiently flexible to permit application and adhesion to moderately embossed surfaces It shall show no damage when bent 90 o over a 50mm diameter mandrel The sheeting shall be solvent-resistant so as to be capable of withstanding cleaning with petrol, diesel fuel, mineral spirits, turpentine or methanol The reflective material shall be weather-resistant and following cleaning in accordance with manufacturer’s recommendations, shall show no discoloration, cracking, blistering, peeling or dimendional change b.) Sign Panels Sign panels for warning, regulatory, and informatory signs shall be flat sheet aluminum, hard alloy 5052-H34 complying with ASTM B209 and having a minimum thickness of 3mm Such sheets are to be degreased, etched, neutralized and processed prior to use as traffic sign panels c.) Warning Lights (flashing or steady) Warning lights shall be Type A (low intensity flashing), Type B (high intensity flashing), or Type C (steady burn) as approved by the Engineer Consultant: SMEC International Contractor: VINACONEX.,JSC Rehabilitation of National Highway No.38B-2, Trang Bridge – Hung Yen Section Package No.8 (NIP/CP-8) 6.3 TEMPORARY ROAD WORKS The Contractor shall furnish, maintain, and remove on completion of the work for which they are required, all temporary roads and road works such as sleeper tracks and staging over roads, access and service roads, temporary crossings or bridges over streams or unstable ground, and shall make them suitable in every respect for carrying Constructional Plant required for the work, for providing access for traffic for himself or others, or for any other purpose Such temporary road works shall be constructed to the satisfaction of the Engineer, but the Contractor shall nevertheless be responsible for any damage done to or caused by such temporary road works Before constructing temporary road works, the Contractor shall make all necessary arrangements, including payment if required, with the public authorities or landowners concerned, for the use of the land and he shall obtain the approval of the Engineer The Contractor, when required by the Engineer, shall submit for the Engineer’s approval drawings giving full details of temporary roads Such details shall include alignment, profile, pavement construction, signing, lighting and the duration of the temporary road The contractor shall make akk arrangements necessary to permit the passage along the road section relating to this Contract of Constructional Plant, materials and employees belonging to other Contractors engaged in the construction of contiguous stretches of road 6.4 HALF-WIDTH CONSTRUCTION Where, in the opinion of the Engineer, a detour is not feasible, construction on existing public roads shall be undertaken only over half of the full width of the roadway The length of such half-width construction shall be kept as short as possible Where half-width construction is necessary, work on culverts commenced in the dry season must be completed and the embankments adjacent to them must be reinstated so that at least half the full width shall be available for use by the public throughout the next rainy season Where single –lane traffic becomes necessary over a particular length of the works or over the approaches thereto, the Contractor, in maintaining through traffic, shall provide a single lane at least three and a half meters wide on the roadway or embankment to be kept open to traffic The contractor shall so conduct his operation as to offer the least possible obstruction, inconvenience, and delay to traffic and shall be responsible for the adequate control of traffic using such length of single lane 6.5 NUMBER OF LANES FOR TRAFFIC CONTROL The existing number of traffic lanes on roads at the project site will be maintained at all times during the work and if diversions are provided these will be of the same traffic capacity as the original road Notwithstanding the above, the Engineer may give approval to reductions in traffic capacity if the Contractor can show that these will not cause excessive delay to traffic If such approval is given, the Engineer may specify the hours during the day when the reduction in capacity Consultant: SMEC International Contractor: VINACONEX.,JSC Rehabilitation of National Highway No.38B-2, Trang Bridge – Hung Yen Section Package No.8 (NIP/CP-8) may be applied and it should be anticipated that thus hours should no include the peak period for the traffic movement under consideration The contractor shall cooperate with the pertinent agencies regarding traffic control and all details will be subject to the Engineer’s approval 6.6 EXTRAORDINARY TRAFFIC The Contractor is responsible for carrying out any necessary investigation and obtaining of approval, licenses, escorts and any other necessary facility in order that to enable extraordinary traffic be moved on the roads in the project area Where transport of the Contractor’s Equipment or Temporary Works necessitate the strengthening, altering, protecting and/or improving any bridge, culvert or road, the Contractor shall provide to the Engineer full details of the Contractor’s intended procedure Such details shall include but not be limited to the following: - Details of Constructional Plant, Plant and/or Temporary Works to be moved including the total weight of the load; - The proposed date and times for such movement; - The starting location and destination; - The proposed route; - The bridge(s), culvert (s) and/or road(s) which requires strengthening, altering, protecting and/or improving;
