CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION PROJECT SAFETY PLAN 1/ 41 PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION 2/ 41 PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION CONTENT Preface Safety Organization and Responsibility Safety Instruction and Training Implementation of the Daily Work Meeting and Weekly Progress Meeting Recording of safety and reporting of accident Personal Protective Apparel & Other Site Safety Protection Measures Safety Standards and Inspection for Plant and Equipment Safe Operation 10 Chemical Management First Aid 11 HIV-AIDS Prevention 12 Fire Prevention 13 Emergency Preparedness 14 Access Control on Site 15 Penalties for Safety Violations 3/ 41 PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION 4/ 41 PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION Preface The safety is of paramount importance at all times and consideration of such safety shall take precedence over any and all other consideration The contractor shall throughout the execution and completion of the wok, having full regard for safety of all persons entitled to be upon the site and keep the site and the work in an orderly state appropriate to the avoidance of danger to such person The primary objective of the Safety Plan is to avoid human distress and financial loss during the performance of all work on field and to ensure effective control of them It is also expected that every employee does his best effort to create a safe environment to work The Safety Program outlines the procedures essential for the protection of men and materials in work Every staff that supervises or directs the work of craftsmen shall pursue his best efforts to assure the safety of each employee under his supervision or direction In addition, he shall be accountable for the protection of equipment and property within his area of responsibility Within 30 days of the award of the Contract, the Contractor shall submit to the Project Manager for his approval, a complete safety program The Program shall contain the following as a minimum: Administration Section  Identification and C.V of the Contractor's proposed Safety Officer  Administrative responsibilities for effecting the Accident Prevention Plan (identification and accountability of Contractor personnel responsible for accident prevention)  Local requirements, if any, which must be compiled with, i.e., noise control, traffic regulation, etc  The method the Contractor proposes to control and coordinate work of his Subcontractors  Safety Plans for layout of temporary construction buildings and facilities, including how the Contractor plans to control those of his Sub-contractors (include sanitary facilities) 5/ 41 PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION  Plans for initial and continuing safety education, and training for the Contractor's employees  Plans for traffic control and marking of hazards to cover haul roads, highway intersections, utilities, bridges, restricted areas, etc  Methods for pedestrian road closures and alternate routes, foot bridges and barricades; directional signs and warnings and pedestrian traffic control This plan shall be coordinated with appropriate local People's Committee officials  Plans for maintaining continued job clean up and safe access  Plans for fire protection and dealing with emergencies (ambulance service, fires, first aid, medical evacuation)  Plans for inspection of the job sites by competent persons including reports to be kept, results of the inspection, and corrective actions taken Particular attention being paid to the utilization of a "Safety Violation Report Form" (ii)  Procedures to be used for accident investigation  Description of safe clearance procedures  Description of office trailer anchoring system  Contingency plans for severe weather Activity Hazard Analysis Section  An Activity Hazard Analysis shall be developed for each contract activity and operation occurring in each major phase of work  The Contractor shall develop the plan to identify the sequence of work, the specific hazards anticipated, and the control measures to be implemented to minimise or eliminate each hazard The Activity Hazard Analysis shall be job specific and shall address the following major points:  Activity being performed (identify major phase), to include but not be limited to the following :  earth moving (equipment operations, backing signals, etc.)  trenching operation (shoring)  lifting equipment operations (crane certification, rigging, etc.)  6/ 41 welding and cutting operations (P.P.E.) PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION  blasting operations (regulations)  Sequence of work  Hazards to be controlled in each activity, and means of control, including, but not limited to :  initial and continuing training  personal protective equipment (hard hats, boots, safety vests, eye/ear protection, etc.)  drugs and alcohol control  The Plan will be reviewed by the Project Manager and a meeting held within two weeks of submittal to discuss and approve the plan 7/ 41 PHUONG THANH TRANCONSIN – GRBCC JV 2.1.1 Project Director and Vice Project Directors Establishing the Safety Control Plan  8/ 41 PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION Taking a measure of accident prevention by coordination throughout the  works Site Patrol   Instruction and Support for Safety Education of Workers  Scheduling and Planning for an Installation of Equipments  Liaison for Authorized Parties  Promote full support of the Safety control Plan  Delegate responsibilities for the various safety functions at all levels of the Project Maintain active involvement in the Project’s safety status and safety  morale Ensure prompt investigation action on any accident investigations with  proper reports completed and follow-up action taken Be responsible for training and instruction for site engineers and office  engineers about their safety responsibilities Be equipped to take over the implementation of safety program during  the time safety manager is absent Keep main role in Safety team Construction manager will check the  technical standards and newsletters serving safety method obedient and attaching responsibilities with this safety team Cooperate  with safety manager in evaluating the operation of subcontractors and make necessary actions to ensure safety Support safety manager in relating-to-technique issues  2.2.2 Safety manager Safety manager is a competent and experienced person who is suitable  for investigation and checking the Safety Plan obedient, and especially in detail but not restrictedly check the Safety Plan Activities in a rolling-up program that depending on different time will be submitted to Consultants for approval The contractor will make no action on site until the Safety manager does  unless Consultants release an official approval document 9/ 41 PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION The contractor must not command the Safety manager to go quit site  unless there is permission in word from Consultants Within 14 days from the notice of transfer of that employee, the contractor will choose an alter Safety manager and consultants will give final decision The contractor authorizes the Safety manager and officers to indicate the  contractor’s or sub-contractor’s employees to end all of their work and carry out the urgent and reasonable actions to ensure the site safety and prevent unsafe actions or violations to Safety plan or the Law Evaluate the needs and propose for safety equipments, first-aid and fire  prevention Make sure that the safety equipments are in good maintenance conditions Making and maintaining a daily site diary, this recording all relevant  matters concerning site safety, inspections and audits, related incidents and the like The site diary shall be available at all times for inspection by the Consultants Safety manager reports to the project manager and informs the decision  on Safety control plan to workers Safety manager makes workers list and indicate the leader of workers’  groups and implements the workers labour safety training plan 2.2.3 Vice Safety manager Vice Safety manager is indicated by CEO under the approval from Consultants and will offer help manage, labour safety monitoring during construction time In case Safety Manager does not present on site, Vice Safety Manager will alter, monitor and decide on matters relating to labor safety In case serious problem happens, Vice Safety Manager reports to Project manager, Safety Manager can solve the problem Project manager will indicated a new employee being in charge of site nurse, carrying out the training of handling health problems for workers/officers on site Site nurse must have knowledge of medical, health safety, first-aid and problems handling 10/ 41 PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION Safe Operation 8.1 Lookout men and flagmen Whether lookout men and flag men are required for the protection of such traffic the contractor shall provide such lookout men and flagmen that are necessary for the safety 8.2 Maintenance of facilities Where the construction work interferes with road traffic the contractor shall provide and maintain all necessary barricades, suitable and sufficient light, danger signal and other sign, provide a sufficient number of watchmen and take all necessary precautions for the protection of the works and the safety of the public 8.3 Safety Housekeeping Daily Safety Housekeeping Cycle Keeping the site in a clean, tidy and sanitary condition Rubbish shall be cleared daily and not allowed to accumulate on site and checking of safety facilities at the end of the day on daily basis before leaving the site, shall be performed by all workers and checked by their Site Engineers The night shift work following the earlier day work requires the concluded work done schedule with the daily safety duties carried out Weekly Safety Housekeeping Cycle Before the start of the work on each Monday morning, safety information of the previous week carried out and its activities with the planned schedule for the current week shall be informed to all staff and workers Cleaning of work places by all workers shall be carried out after the group discussion on Saturday morning before the start of that day’s work Monthly Safety Housekeeping Patrol/Cycle Monthly Safety Patrol attended by representatives of VEC and the Engineer shall be carried out with reports prepared by the Safety Manager from these patrols These reports shall be issued to all attendees at the Safety Patrols and also to discuss any issues at the Daily Meeting for follow-up actions The monthly patrol shall be held at end of each month as part of the monthly schedule meeting whereby discussion on suggestions for any 27/ 41 PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION site safety /work schedule relating to the patrol can be taken into action for the following month 8.4 Working at Height Where works are to be carried out at height, scaffolding erection for works will be carried out at elevated area Safety concerning the erection, maintenance and dismantling of these scaffoldings is mandatory to ensure safe operation on site All scaffoldings to be inspected/checked at regular intervals as required by the manufacturers or the legal requirements for wellmaintained condition Trained and competent workers with supervision for this work Any deflective parts of the scaffolding materials shall be removed from the storage area Working platform shall be sufficiently strong, properly secured and not overloaded with toe-boards to prevent falling objects Erect barricades or overhead shelter wherever practicable to prevent person from being struck by falling objects Open-sides/working platforms higher than safe allowable height may be properly barricaded All personnel working at heights have to wear their personal safety gear at all times Routine checks on erected scaffoldings e.g Daily site check, Monthly safety patrol, etc shall inspect such scaffolding to ensure the tag of records of checks conducted as per requirements Safety nets should be around the scaffolding if exceed the safe allowable height to prevent any falls of materials 8.5 Working around Equipment/Machineries Limitation of working space for works to be carried out within construction site may affect movements of workers As a result, accident may happen when the machineries accidentally knocked/hit workers working too close to them If such situation arises, the followings must be observed by the Contractor: Keep a safe distance depending on the type of work to be carried out by the workers within close vicinity of machineries Site Engineer should inform such requirements to the workers and operators of machineries Appoint signalmen/riggers to control the movement of the machineries during these periods 28/ 41 PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION Do not store too many unnecessary items at this congested area Housekeeping is required If this situation is dangerous and congested, review work schedule and carry out work in sequence 8.6 Reflective personal vest/clothing provided for better visibility Pressurized Equipment All pressurized equipment and systems shall be approved, tested and equipped with safety or relief valves and pressure gauges All pressurized equipment or systems confirmed to be unsafe shall be tagged “OUT OF SERVICE”, “DO NOT USE” Use of such unsafe equipment is prohibited Pressurized equipment shall be operated and maintained only by qualified and authorized personnel Safety devices shall not be removed except to make repairs Compressors and related equipment shall be correctly placed in order to provide safe access to workers to operate, maintenance or repair Safety valve setting shall not exceed 110 % of working pressure Discharge of any safety valves, relief valves will blow off and to direct away from the personnel All air compressors shall automatically stop before the discharge pressure exceeds the maximum working pressure allowable on the weakest portion of the system A stop valve shall be installed on each outlet for air hoses The stop valve on the air hose shall be closed when a tool is being changed or repair work is in progress A drain valve on the air received shall be opened to drain the accumulated oil or moisture before commencing work 8.7 Hand Tools General a) All personnel who use hand tools must be trained properly to ensure safe operation and use at all times b) Hand tools shall be kept in well-maintained condition/places and use them for the work which they are designed for the purposes c) Inspections at periodic interval shall be carried out to assure safe operating conditions d) Defective tools must be placed away for repair and not to be used e) Tools that are not required at elevated position shall be secured or placed in holders 29/ 41 PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION f) Transfer tools/materials from one location to another by throwing or dropping to lower levels are prohibited g) Tools of non-sparking nature shall be used where hazardous vapor exist as it may initiate a fire or explosion Power tools shall be operated only by appointed personnel h) Flexible rubber cable shall be used for all portable electric tools Waterproof connectors shall be used for cable connections Grinding Tools Cracked or Damaged grinding wheels of tools shall not be used Pneumatic Tools a) Compressed air shall be exhausted from the line before disconnecting tools from the line b) Air hose shall be labeled with maximum safety working pressure as recommended by the manufacturer and this stipulated pressure shall not be exceeded Deflective hoses shall not be used c) Hoses shall not be left on ladders, steps, scaffolds or walkways d) Compressed air to blow dirt from hands, face, or clothing is prohibited 8.8 Welding and Cutting General a) All welding and cutting apparatus shall be inspected daily by personnel in-charge Defective apparatus or equipment shall be replaced or repaired b) Combustible materials, equipment such as gas cylinders, rubber hoses and debris shall be removed or shielded from heat, sparks and slag from welding or cutting c) Welding or cutting in an area where flammable vapor may be present shall be carried out carefully, under the supervision of Safety Manager after confirming with the person in-charge of the gas detector that the area is safe Welding Works a) Welding machines to be placed at least 150 mm above the ground to keep them in dry condition b) Welding machines shall be placed at level surface and secured with a suitable wedge to keep them in a horizontal position c) All electric welding machines shall be effectively grounded The ground lead for the welding machines shall be mechanically strong and electrically adequate for the service required d) Approved connectors shall be used to connect the welding cable between the welding machine and electrode holder 30/ 41 PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION e) Only electrode holders that have passed the inspection shall be used f) Welder has to leave the work site whenever his welding work is suspended He has to turn-off the power and disconnects the welding rod from the holder g) All personnel located within 30 m at electric welding operations shall be screened from the welding flash Gas Works a) Cylinders shall be stored in well-ventilated location/area and shield from direct sunlight with steel plates or incombustible canvas Cylinders in service shall be secured in portable racks or handcarts b) Cylinders shall be transported by crane, hoist or derrick in cradles, nets or skid pans but not in slings or chains or by magnets c) Cylinder caps shall be placed in the proper area where they are in storage, or during transfers and whenever the regulators are not in place d) Acetylene cylinders shall be in an upright position during in use and secured to prevent displacement e) Cylinders for use underground must be equipped with flask back arrestor Otherwise, it is prohibited to use them in underground f) Oxygen cylinders and fittings shall be kept away from oil or grease No oil-greased hands or gloves are allowed to handle these cylinders Tags indicating FULL or EMPTY must be done Empty cylinders shall be returned immediately to storage areas g) All connections between hoses, hose and torch and hose to regulator shall be tightened rigidly with hose bands h) Pressure gauges with cracked glass or damaged regulators shall not be used and will be replaced i) Before start of work, leakage test shall be made at all connections with soapy solution j) Torch valve shall be closed and gas supply shall be shut-off whenever work is suspended k) Cylinder’s block valve shall be closed after work is completed Regulators and hoses shall be removed and put in the storage location and cap to be replaced properly 8.9 Lifting and hoisting equipment Prior to use its use on site all lifting and hoisting equipment shall be tested to an approved safety margin The operation of these equipment shall be noted as follows 31/ 41 PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION Lifting of loads over public areas is prohibited Clear workers below the lifting operating area Do not operate lifting and hoisting works during inclement weather Do not overload beyond the allowable safe working load Outriggers are fully extended Any sudden movements are to be avoided Loads to be securely slung and no falling/dislodgement will happen Chemical Management 9.1 Chemical Inventory The contractor will prepare inventory for chemical materials and material safety data sheet for the each chemical material such as solvents, lubes oils Inventory will be updated once a month or when the considerable amount of change occurred with stock quantity 9.2 Handling Procedures As for the handling of Chemical materials, adequately trained personnel available will be assigned for material control Also, the storage area will be secured for prevention of intrusion In case of spillage, spilt material will be properly absorbed with unwoven cloth or similar material and treated properly in accordance with relevant regulations 9.3 Storage Chemical material shall be stored or separately in accordance with characteristic of each substance such as flash point, fire pint, melting point, boiling point, soluble characteristic In addition, to prevent the unsuitable mixture of each substance, each substance will be properly segregated in appropriate containers when stored Especially for storage of flammable substances extinguishers shall be arranged proper 10 First Aid 10.1 First Aid Base The Contractor shall provide, maintain, and move when required, to locations directed or approved by the Project Manager, a mobile emergency medical facility This unit shall be manufactured for it's intended purpose and shall not be a site fabricated facility The facility will be either vehicle mounted or mounted on skids capable of being loaded onto a flat bed trailer for the purpose of relocating It shall be blocked up and levelled at the contractor's 32/ 41 PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION camp or other such site as agreed by the Project Manager It shall be approximately 15m2 in area, well lit, weather tight, insect proof, with lockable door and windows, and shall include the following : (a) Dedicated electrical generating system, plus ability to be connected to public supply if available (b) Potable water supply (c) Sanitary facilities (d) Air conditioner/heater (e) Radio transceiver on the Contractor's frequency Furnishings, including: - Desk - Chair - Medical couch with adjustable head and foot ends - Instrument Table - Stainless Steel sink - Examination Chair - Stretcher - Storage cabinet (2)/lockable (g) Four electric outlets A qualified nurse and nurses aid shall be in attendance at the First Aid Base during all times when work is being undertaken on the Site, including work by any Subcontractor and periods when only emergency activities are being undertaken, such as during periods of inclement weather 10.2 First Aid Boxes First Aid Boxes will be provided at First Aid Bases at the offices with full necessary things The medical center will have a substantial first aid kit, including stretcher, for serious medical emergencies when the medical center staff are called out to attend 33/ 41 PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION Medical Equipment and Materials including but not limited to the following - First aid bandages and dressings - Nursing implements - Emergency medicines - Emergency medical instruments - First aid bag with shoulder strap - Soaps - Disinfectants - All other emergency medical equipment, supplies and materials which may reasonably be anticipated as necessary, given the nature of the work and the location 10.3 Medical arrangement The contractor shall have adequately trained personnel available for the on site treatment of casualties and sick person in first -aid unit and shall provide the means whereby injured or sick personnel may be expremoved editiously to hospital or receive the necessary medical attention The Contractor shall employ and provide the services, for the duration of the contract, of one Certified Emergency Medical Technician (EMT) The EMT shall be based at, and utilise the facility specified above He shall be available during the Contractor's working hours, and shall have available to him at all times one vehicle capable of providing emergency transportation to injured individuals The Contractor shall submit the qualifications of the proposed EMT to the Project Manager for his approval 11 HIV-AIDS PREVENTION This clause is provided to supplement the requirements provided in the Conditions of Contract While mobilized for work the Contractor shall conduct an HIV/AIDS Information, Education and Consultation Communication (IEC) campaigns via an approved service provider, and shall undertake such other measures as specified in this Contract to reduce the risk of transfer of the HIV virus between and among the Contractors Personnel and the local community, to promote early diagnosis and to assist affected individuals The Contractor shall not discriminate against people found to have HIV/AIDS as part of the campaign 34/ 41 PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION The Employer shall provide to the Contractor a list of approved service providers which shall include recognized local health providers which shall and/or recognized local health departments From the list the Contractor shall contract one service provider to undertake the HIV/AIDS IEC campaign The IEC campaign shall be conducted while the Contractor is mobilized at least every months, addressed to all the Site staff and labour (including all the Contractor's employees, all Sub-Contractors and Consultants' employees, and all truck drivers and crew making deliveries to Site for construction activities) and to the immediate local communities, concerning the risks, dangers and impact, and appropriate avoidance behaviour with respect to of Sexually Transmitted Diseases (STD) or Sexually Transmitted Infections (STI) in general and HIV/AIDS in particular At the time of first mobilization, the approved service provider shall prepare the HIV/AIDS Prevention Program which will be submitted to the Engineer for approval This will clearly indicate the (i) the types of education activities to be done; (ii) whether condoms shall be provided; (iii) whether STI/HIV/AIDS screening, diagnosis counselling and referral to a dedicated national STI and HIV/AIDS program, (unless otherwise agreed) of all Site staff and labour should be provided 12 Fire Prevention Safety Engineer is accountable to ensure the availability and quality of fire prevention and fighting equipment allocated on the Site Each office block and canteen is allocated one CO2 fire extinguisher and one Dry powder fire extinguisher One CO2 fire extinguisher and one Dry powder fire extinguisher are allocated at each side of the parking lot Following working process, some mobile dry powder fire extinguishers (35 kg) shall be located in the working areas Locations of these mobile extinguishers are decided by Safety Officer depending on the specific works of each stage of the project Special attention is given to welding work places, storage of woods, chemical, paper and cloths and electrical hazard areas List of equipment for fire prevention and fighting is given as below 35/ 41 PHUONG THANH TRANCONSIN – GRBCC JV CIVIL WORKS CONTRACT PACKAGE A1 (KM 65+000-KM81+150)- DA NANG – QUANG NGAI SECTION All areas that have fire hazard are posted "No Smoking", “No Fire” and regulation board of fire prevention and fighting Welding, burning and electrical works are only permitted through the Permitto-Work system Safety Engineer is responsible to make arrangements with the Fire Brigade of Local province for its support and assistance in training, auditing the fire prevention system and in case of fire Equipment List for Fire Prevention and Fighting No CO2 Fire Extinguisher – Type kg Dry Powder Fire Extinguisher – Type kg 13 Description of Equipment Dry Powder Fire Extinguisher – Type 35 kg (Mobile) Set of Regulations of Fire Protection (Tin board) Emergency Preparedness 13.1 General Emergency procedure will be prepared and displayed in a prominent position in the site office The emergency plan will include the following: The method to evacuate from the incident area will be given The person who is in-charge of such emergency Location of mustering areas and directions Instructions for people to walk instead of running during must be reported to Project Management and Safety Manager 13.2 Emergency Call Out Procedure Introduction The procedure is intended to be used for the contractor ‘s activities and must be observed by all employees and Sub-Contractors working in this Project Priorities The Site Engineer in-charge / Construction Manager shall take full control of the   situation The Contractor’s duty personnel who have first aid training and can provide life saving assistance including CPR (Cardio Pulmonary Resuscitation) must perform the first aid treatment, if required Immediately, after being released, the Emergency Call Out Procedure shall be initiated Trigger Definition Triggering must be done to conform with the following conditions: a) Fatal accidents b) Any accident that causes serious injury with staff working on day and night works, shall initiate the Emergency Call Out Procedure The Site Engineer and Safety Manager (2nd Stage) will be called, and if they cannot be contacted, their level will be skipped and the 3rd Stage will be alerted In addition, the initiator will take prompt action to mitigate the situation When he is released he will report on: i) The situation in general in compliance with the trigger definition ii) The Number of people killed / injured / involved iii) Action taken by responsible site supervisory personnel iv) Other vital information in brief The Site Engineer will also report the matter to the Construction Manager and contact the duty telephone on the situation b) Second Stage (2nd Stage) Safety Manager, upon receiving information will check the completeness and assess the situation If necessary, he will travel to the Site to check himself Upon verification of all information involved, he will initiate the Third Stage (3rd Stage) with given contractor shall bear all responsibility and necessary arrangement for convenience and safety for access 14.2 Boundary Demarcation Right of Way This Right of Way shall be demarcated the boundary of the worksite with yellow flags at intervals and safety board, informing the public/outside parties on worksite Confined Area These confined areas where the construction, mechanical plant, material stockyards, etc are located, shall be clearly marked with red flags for the attention of the public/outside parties Handrails will be installed along the boundary of these confined areas to prevent site 15 Penalties for Safety Violations 15.1 Removal from Site: For a number of actions that are mentioned throughout this Safety Plan, personnel that engage in such activities as: drinking on site, stealing, vandalism, repeated failure to abide safety instruction, threatening behavior toward any staff, Safety Managers, security officers, will result in their removal from site, confiscation of ID card, and if deemed necessary - escorted to the local police station All the above offences will additionally incur a fine to the company concerned of VND 300.000, for each offence 15.2 Minor Offences: For personnel committing minor offences, it most circumstances they will receive a verbal warning to rectify the situation immediately, however, if they refuse to take action, or ignore the Safety Managers request their ID card No will be recorded, along with their offence, and a penalty will be imposed on the workers company of VND100.000 A minor offence is one where a worker is in violation of the rules, but not causing himself or others immediate danger 15.3 Major Offences : A major offence being a violation of safety rules which places the workers or other personnel within
