JV CIVIL WORKS CONTRACT PACKAGE (KM 32+600-KM42+000)- DA NANG – QUANG NGAI SECTION I ASSURANCE METHOD FOR LABOR All works must be in accordance with Specifications Section 01500 Project Safety Safety & health policy of bidder Bidder strives to integrate the safety and health protection activities into every aspect for the best performance to become one of the leading major Bidder with environmental affinity to give prominence to environment, safety & heal for society, customer and labor With opinion: Safety and health protection is the most important element in construction process associate with business production effect The Bidder believes that all accidents are preventable and no undertaking is so important that would recognize the need to endanger the life of the person It is our company policy that safety and health of the public, clients and employees by all means throughout the execution of our business activities with the following principles: The Bidder will perform the work in full compliance with all safety and industrial health legislation and regulations of the Socialist Republic of Vietnam The Bidder shall take part in regional SH program and assist that on the basis of close cooperation with regional society The Bidder will conduct continuous safety measures of the public and all persons associated with the works The Bidder will also display the safety and industry health posters to rouse the caution behavior The Bidder will maintain the existing pedestrian routes in a safety condition or provide the alternative route in view of securing safety of local population The Bidder believes that time and effort shall be spent for the advancement of any operations and undertakings The Bidder will provide full cooperation and assistance in safety surveillance which will be carried out by the Engineer or the Employer Above SH Policy and the results of activities will be available to the public The Management Representative shall ensure that this SH Policy is fully understood, implemented and maintained at relevant functions and all levels of organizations by means of reviewing continuing suitability and improving the effectiveness of the documented SH management system All of our employees take part in the SH Management System and make efforts to enhance SH mind 3/ 29 PHUONG THANH TRANCONSIN – CIENCO6 – CIENCO8 – THANH PHAT JV CIVIL WORKS CONTRACT PACKAGE (KM 32+600-KM42+000)- DA NANG – QUANG NGAI SECTION Project safety organization and responsibility 2.1 Safety Organization Chart [Figure 0]: Safety organization chart 2.2 Responsibility Project Manager The Project Manager is responsible for the establishment of the Project Safety Plan This Plan shall be according to The Bidder’s policies with the recommendations and assistance of the Technical Board and the Safety Manager Accident prevention procedures will be included in all activities by the application of sound safety planning Specifically, the Project Manager shall set the pace for the project safety plan with the Safety Manager and shall act as follows: Direct the activities of the Safety observation Team as the Project Manager, Promote full support for the implementation of this Safety Plan, Delegate responsibilities for the various safety functions and authorities with safety promotion and observation at all levels of the Project, 4/ 29 Maintain active involvement in the Project’s safety status and safety morale, PHUONG THANH TRANCONSIN – CIENCO6 – CIENCO8 – THANH PHAT JV CIVIL WORKS CONTRACT PACKAGE (KM 32+600-KM42+000)- DA NANG – QUANG NGAI SECTION Ensure a prompt investigation action on any accident investigations with proper reports completed and follow-up action taken Construction management department (Plan & engineering department) The responsibility position of Construction management department is appointed by the Project Manager, to be in-charge of the Quality, Health and Safety aspects of all activities on site with the system implemented in this Project Construction management department will oversee the activities concerning these aspects by reviewing/discussing with the Safety Manager or even together with the Project Manager to ensure the compliance of the laws and regulations The specific safety responsibilities of the Chief of Construction include, but are not limited to, following: Responsibility for all safety activities on the Project and maintain strong liaison with the Project Manager and Safety Manager, Assist Safety Manager to establish and implement Safety Plan, Ensure and support Safety Manager to promote activities at the construction site to achieve the Project’s safety objectives and targets, Ensure the Safety Team is formed and actively involved in safe environment matters in the Project, Ensure compliance to the Safety Plan by all departments in this Project, To support Safety Manager to establish safety rules with special attention to unusual operations and safety requirements of the Employer, Ensure smooth information and effective follow-up action of Safety Manager after discussion with Project Manager, Enforce the disciplinary actions necessary to develop a good functioning safety program, Involve with the planning for the operations where the possible hazard may happen or the safety level is below the normal, Oversee the implementation and execution of Safety Plan, Program and their effectiveness Suggest/Support the Safety Manager on the implementation of Safety Plan or Program such as training program for approval from the Project Manager, Monitor/Check and Instruct to correct any unsafe acts during site patrols Safety Manager The Safety Manager is responsible for the administration of the implementation of Safety Plan and Program The Safety Manager reports to the Project Manager or Chief of Construction and informs Site Managers and shall actively promote the safety program at the construction site to all levels within the Project As Administrator of the safety program, he shall function as the Safety Coordinator, 5/ 29 PHUONG THANH TRANCONSIN – CIENCO6 – CIENCO8 – THANH PHAT JV CIVIL WORKS CONTRACT PACKAGE (KM 32+600-KM42+000)- DA NANG – QUANG NGAI SECTION Advisor and Consultant for the Project Manager, Chief of Construction, and Site Managers and supervise project staff, sub-Bidders in establishing and implementing safety programs Safety Manager shall be a suitable qualified and experienced person with complete involvement with the safety activity throughout the period of this Project The Safety Manager’s specific responsibilities include, but not limited to, the followings: Develop and supervise the implementation of the Safety Plan for this project Coordinating the site safety efforts with the Project Manager, Construction Managers, Team leaders, Site Engineers and follow the Employer’s health and safety requirements Keeping the Project Manager, Construction manager, Team leaders, informed of the progress of the Safety Plan, Program and making recommendations for site related safety improvements Conduct or support Team leaders, Sub-Bidders conduct Safety training for Bidder’s staff, prior the work at site Propose program to implement the accomplishment of short and long range of site safety objectives and to motivate all employee participation to obtain these objectives Preparing, submitting and distributing the required safety and injury reports with records kept in his file Maintaining a continuing inspection program and reporting the findings and recommendations to the Project Manager/ Construction manager/ Team leaders, Sub-Bidders for prompt corrections Conducting and/or participating in job safety meetings Monitoring safety trainings in accordance with the site requirements Coordinating the first aid and rescue services on site to ensure the appointed employees are equipped with proper first aid know-how in the absence of a nurse Establishing a fire prevention program with training on the actions and participation of employees for designated duties such as informing the Fire Brigade Investigating all accidents, injuries, fires, property damage and other safety related incidents and prepare/submit the required reports Identify the classification of injuries if they are occupational or time losing, disabling injuries and take the follow-up actions on the control of injury cases with appropriate accident forms to be filled 6/ 29 PHUONG THANH TRANCONSIN – CIENCO6 – CIENCO8 – THANH PHAT JV CIVIL WORKS CONTRACT PACKAGE (KM 32+600-KM42+000)- DA NANG – QUANG NGAI SECTION Assisting Construction teams with their safety programs to meet the various site safety requirements Notify the Project Manager and Construction manager, Team leaders of all safety inspections with any action to be taken Evaluating the needs and then, request of safety, first aid and fire protection equipment needed on the job Ensure that safety equipment is maintained in good condition at all times Check/Provide safety approval for new tools and equipment, including tools needed for specific job Effective display and maintain publicity of news of safety on the site bulletin boards, posters, safety signs, banners and distribution of safety literature to insist a good and positive attitude on health and safety environment amongst the people of all levels on site at all times Administer and/or monitor the site hazardous program Administer and/or monitor the Occupational Safety Act and the hazardous materials information system regulation on worksite Attend pre-job meetings with Construction teams, Sub-Bidders and ensure the Construction teams, Sub-Bidders comply with Bidder’s safety policies and procedures during their work in the Project Plan the site safety program timely to achieve the required site safety policies and procedures accordingly Maintain an updated knowledge of all safety policies and procedures with knowledge of state provincial and local safety laws and regulations Maintain updated knowledge of state-of-the-art concepts of accident prevention and continually contributing to upgrading the site safety effort Actively support Team leaders, Site Engineers to resolve any emergency issues related to safety at the construction site Team leaders Team leaders are key figures when it concerns the monitoring of the contractual obligations of the various Bidder in the field of Safety at the construction site Team leaders is responsible for the training and directing of site engineers their safety responsibilities Team leaders shall support Safety Manager to conduct Safety Training for labors prior the construction works Team leaders shall be prepared to accept the responsibility for administering the site safety program during the absence of the Safety Manager from the project 7/ 29 PHUONG THANH TRANCONSIN – CIENCO6 – CIENCO8 – THANH PHAT JV CIVIL WORKS CONTRACT PACKAGE (KM 32+600-KM42+000)- DA NANG – QUANG NGAI SECTION Team leaders shall take an active part in the Safety Team He shall check safety preparedness and ensure safety for the upcoming work He shall assist the Safety Manager on problems of a technical nature Site Engineer (Site Safety Checkers) Site Engineers are directly responsible for the control of safety for all activities at the construction site of this Project They are the key personnel in the implementation and maintenance of effective actions of the project safety program in time manner, assisting the Safety Manager and Site Managers The standards for a good safety program are established by Safety Manager, Construction manager and Team leaders Site Engineers will be directly responsible for the success of the safety program hence they are in direct contact with labors Their abilities to achieve good safety attitudes to labors will reflect the success of the safety program at site Site Engineers must plan and implement necessary activities to ensure safety for all labors under his supervision prior, during and after construction activities The specific safety responsibilities of Site Engineers include, but are not limited to the following: Review all work and all new personnel for safety needs Conduct training of safety for labors, which will execute up-coming works Complete familiarization with safety requirements is an integral part of the total job responsibility Continually monitor safety attitudes of labors and their responsibilities towards safety in the job safety effort Enforce on compliance with all established safety regulations before and during performance of the work Conduct safety inspections and support labors make corrections on a daily basis Ensure that proper safety training is provided Initiate plans prior to the start of work to supply the tools, materials, and equipment necessary to the job safely Assist with accident investigations to ensure proper reporting and documentation with prompt corrections made after accident and to prevent any recurrences Coordinate planning and work activities with other supervisory personnel performing work within, or adjacent to, their assigned work areas Operators / Workers 8/ 29 PHUONG THANH TRANCONSIN – CIENCO6 – CIENCO8 – THANH PHAT JV CIVIL WORKS CONTRACT PACKAGE (KM 32+600-KM42+000)- DA NANG – QUANG NGAI SECTION Operators or workers working with tools, plant, equipment, etc are directly responsible for their health and safety with others working around them Therefore, there should be sufficient instructions provided to them They shall be responsible for but not limited to the followings: Use proper, correct and well-maintained tools and equipment for the jobs, Properly use protective equipment such as safety helmets, safety boots, goggles, safety belts, etc which must be worn whenever required Maintain and keep tools in good condition at all times Report defects in plant or equipment or any potential risk to health and safety to their Supervisor or Team leaders, Site Engineers or Project Safety Manager Avoid improvising work which entails unnecessary risk, Refrain abuse of welfare facilities, Suggest ways for eliminating hazards if he can Accident preventive measures The best way to minimize expenditure on the treatment of the accident victim is to supply labors adequate knowledge and equipment for avoiding possible accident Therefore, Bidder focus on providing adequate safety education and personal protective equipment to all of its labor At least, following activities shall be provided to labors prior and during construction period of the project: 3.1 General No employee shall be required to work in surrounding or under working conditions which are hazardous or dangerous to his/her life, health and safety The Bidder shall be responsible for initiating and maintaining a safety program which will comply with the Contract, Main Bidder, The Engineer’s representatives and statutory requirements Safety Program shall provide frequent and regular safety inspections of the worksites, material and equipment by competent person designated by The Bidder Results of findings and correction measure shall be submitted to the Main Bidder, the Engineer’s Representative 3.2 New Employees Indoctrination Each employee shall be provided initial indoctrination and such continued training will enable him to perform his work in a safe manner Indoctrination and training shall be based on the safety program of The Bidder, government degree for safety training program (08/1995/TT-LDTBXH) and shall include but not limited to: General safety policy and pertinent provisions of The Bidder’s Project Safety Plan, 9/ 29 PHUONG THANH TRANCONSIN – CIENCO6 – CIENCO8 – THANH PHAT JV CIVIL WORKS CONTRACT PACKAGE (KM 32+600-KM42+000)- DA NANG – QUANG NGAI SECTION Requirements for employees and project safety, Employee’s responsibilities for property and the safety of others, Employee’s responsibilities for reporting all accidents, Medical facilities and required treatment, Procedures for reporting or correcting unsafe conditions or practices, Safe clearance procedures, Fire and other emergency procedures 3.3 Regular Education Object : Time To reinforce safety issues for all site employees : Instructor : Contents : One time per month during “Tool box meeting” Chief of Section/ Safety Manager Dangerous factor of each work process Safety/ health performance and accident prevention Example of accidents & prevention of accident movement Other items about site safety control 3.4 Instructions and Training on the Job Safety meetings scheduled on regular basis shall be held at least once a month for all Site Engineers on the Project to review past activities, to plan ahead for new or changed operations and establish safe working procedures for anticipated hazards Everyone required entering confined or enclosed spaces or atmospheres with immediate danger to life, shall be instructed as to the hazards involved and precautionary measures to be taken Training by qualified person on the use and care of such emergency and protective equipment shall be conducted All Project staff shall comply with all specific regulations in dangerous or potentially dangerous areas All employees who have to handle poisons, caustic or other harmful substances, are required to receive instruction regarding the safe handling and use Information 10/ 29 PHUONG THANH TRANCONSIN – CIENCO6 – CIENCO8 – THANH PHAT JV CIVIL WORKS CONTRACT PACKAGE (KM 32+600-KM42+000)- DA NANG – QUANG NGAI SECTION Stoppers are required on hook of crane to prevent rope from slipping off 3.12 Tools and Equipment Inspections All tools (electric, explosive, hydraulic, pneumatic, gas) and equipment, especially for electrical distribution, welding, oxyacetylene cutting, grouting machine, girder launching, jacking, hoisting, drilling, barge, floater, will be inspected by safety department while on site After a successful inspection an inspection tag those records the: date of test, name of tester, name of company, expiry date, will be attached to the equipment Tools and equipment that fail inspections will be removed from service 3.13 Site Inspections of Tools and Equipment During the daily and weekly safety inspections on site by the Safety Department, special attention will be given to: the checking of equipment tags, the observation of correct use, (including use of safety gear), the condition of hand tools, and to the condition and use of non-mechanical equipment such as ladders and scaffolding Any equipment in poor condition will be removed from service by the Safety Department, without any further warning to the SubBidder Larger equipment that cannot be easily removed will be shut down if found to be in poor condition, for example material and personnel hoists, bar cutting machines 3.14 Safety Housekeeping Daily Safety Housekeeping Cycle Cleaning of work places and checking of safety facilities at the end of the day on daily basis before leaving the site, shall be performed by all workers and checked by their Site Engineers The night shift work following the earlier day work requires the concluded work done schedule with the daily safety duties carried out Tool Box meetings or discussion group meetings shall be held before the start of the night shift work so that the jobs to be carried out are instructed accordingly Weekly Safety Housekeeping Cycle Before the start of the work on each Monday morning, safety information of the previous week carried out and its activities with the planned schedule for the current week shall be informed to all staff and workers Cleaning of work places by all workers shall be carried out after the group discussion on Saturday morning before the start of that day’s work Monthly Safety Housekeeping Patrol/Cycle Monthly Safety Patrol attended by representatives of Safety Managers, Specialized Construction Work Managers, Main Bidder and team leaders shall be carried out with reports prepared by the Safety Managers from these patrols These 15/ 29 PHUONG THANH TRANCONSIN – CIENCO6 – CIENCO8 – THANH PHAT JV CIVIL WORKS CONTRACT PACKAGE (KM 32+600-KM42+000)- DA NANG – QUANG NGAI SECTION reports shall be issued to all attendees at the Safety Patrols and also to discuss any issues at the Daily Meeting for follow-up actions The monthly patrol shall be held at end of each month as part of the monthly schedule meeting whereby discussion on suggestions for any site safety /work schedule relating to the patrol can be taken into action for the following month 3.15 Working at Height Scaffolding erection for works will be carried out at elevated area Safety concerning the erection, maintenance and dismantling of these scaffoldings is mandatory to ensure safe operation on site All scaffoldings to be inspected/checked and recorded properly at regular intervals as required by the manufacturers or the legal requirements for wellmaintained condition Trained and competent workers with supervision for this work Any deflective parts of the scaffolding materials shall be removed from the storage area and recorded Working platform shall be sufficiently strong, properly secured and not overloaded with toe-boards to prevent falling objects Erect barricades or overhead shelter wherever practicable to prevent person from being struck by falling objects Open-sides/working platforms higher than safe allowable height may be properly barricaded All personnel working at high risk of falling places have to use their personal safety gear properly, especially safety belt at all times Routine checks on erected scaffoldings e.g Daily site check, Monthly safety patrol, etc shall inspect such scaffolding to ensure the tag of records of checks conducted as per requirements Safety nets should be under I-Girders to prevent any falls of labors All work on high places shall be stopped if the wind speed exceeds 35 m/second 3.16 Work around Heavy Equipment/Machineries Limitation of working space for works to be carried out within construction site may affect movements of workers As a result, accident may happen when the machineries accidentally knocked/hit workers working too close to them If such situation arises, the followings must be observed: Keep a safe distance depending on the type of work to be carried out by the workers within close vicinity of machineries, especially around cranes or excavators Site Engineer should inform such requirements to the workers and operators of machineries 16/ 29 PHUONG THANH TRANCONSIN – CIENCO6 – CIENCO8 – THANH PHAT JV CIVIL WORKS CONTRACT PACKAGE (KM 32+600-KM42+000)- DA NANG – QUANG NGAI SECTION Appoint signalmen/riggers to control the movement of the machineries during these periods Do not store too many unnecessary items at this congested area Housekeeping is required If this situation is dangerous and congested, review work schedule and carry out work in sequence Reflective personal vest/clothing provided for better visibility Carefully check cranes, lifting wires, accessories, ground condition prior lifting operation 3.17 Electrical Works Electric wires should be lifted up from the earth surface or properly isolated Any damaged electric wire shall be immediately replaced or repaired properly Electric cable joint will be properly connected with using vinyl tape Before starting use, The Bidder will check the condition, and cannot use without approval of The Site engineer/Safety checker The Bidder will attach sticker for confirmation of checking All the electric machines must equip the earths, and be suitably connected And electricity shut-off switch will be equipped for each machine Before starting use, The Bidder will check the machine condition and the earth All the workers must take on suitable personal protective equipment (PPE) such as rain boots, gloves, and helmet Workers who don’t take on proper PPE will be kicked out from the site When machines such as welding machine, bending machine, and cutting machine, need to be shifted, the switches must be turned off The Site engineer/Safety checker will instruct labor When machines are not in use, switch should be turned off Electric machine will be covered when it is raining If it is found that the electric machine condition is bad such as electricity leakage, the machine cannot be used until repair finish and get approval of Site engineer/Safety checker All electric facilities and machines will be periodically checked by The Bidder together with Construction teams 3.18 Pressurized Equipment All pressurized equipment and systems shall be approved, tested and equipped with safety or relief valves and pressure gauges All pressurized equipment or systems confirmed to be unsafe shall be tagged “OUT OF SERVICE”, “DO NOT USE” Use of such unsafe equipment is prohibited 17/ 29 PHUONG THANH TRANCONSIN – CIENCO6 – CIENCO8 – THANH PHAT JV CIVIL WORKS CONTRACT PACKAGE (KM 32+600-KM42+000)- DA NANG – QUANG NGAI SECTION Pressurized equipment shall be operated and maintained only by qualified and authorized personnel Safety devices shall not be removed except to make repairs Compressors and related equipment shall be correctly placed in order to provide safe access to workers to operate, maintenance or repair Safety valve setting shall not exceed 110 % of working pressure Discharge of any safety valves, relief valves will blow off and to direct away from the personnel All air compressors shall automatically stop before the discharge pressure exceeds the maximum working pressure allowable on the weakest portion of the system A stop valve shall be installed on each outlet for air hoses The stop valve on the air hose shall be closed when a tool is being changed or repair work is in progress A drain valve on the air received shall be opened to drain the accumulated oil or moisture before commencing work 3.19 Counter actions against storm and possible lightning in rainy season In order to ensure safety and quality of works, at least following countermeasures shall be taken against four main groups of impacted factors namely: (i) Raising of the surface water during flood and storm season, (ii) Raising of the ground water level during flood and storm season, (iii) Heavy rain and strong wind occur when there are storms, (iv) Thunder and lightning strikes in heavy rain and storm i) In case of the increment of the surface water during flood and storm season, we shall apply at least following countermeasures: -Monitoring the water precipitation and rise of flood water level inside the dyke and inform to manager timely -Equipment, Machineries, Materials and Facilities nearby flood and storm area shall be shifted, removed and kept to a sufficiently higher elevation and high level location Materials that are not affected by flood or storm water (such as steel H-Beams and steel sheet piles for temporary purposes only) may remain at their respective location Other materials such as Reinforcing Steel Bars shall be shifted and kept at sufficiently high position with plastic cover protection and shall be properly underlay -Fuel, oil and lubricants inside drum shall be transported and kept at safe location -Electrical line supplying power to nearby flood area shall be installed with a separate circuit breaker where it can be shut off in case where water level will rise up to the tapping points 18/ 29 PHUONG THANH TRANCONSIN – CIENCO6 – CIENCO8 – THANH PHAT JV CIVIL WORKS CONTRACT PACKAGE (KM 32+600-KM42+000)- DA NANG – QUANG NGAI SECTION -Standby Generators shall be utilized in case when the electric power is deem and necessary at those locations -Make appropriate drainage system to minimize surface water going into excavated substructure holes and prevent surface water to stay on the access road ii) In case of the increment of the ground water level during flood and storm season, we shall apply at least following countermeasures: -Reduce the slope of the walls of excavated sub-structures, -Strengthen the walls of excavated sub-structures by appropriate materials, -Soils excavated from substructure holes shall be removed from the substructure excavated areas or placed at least on the distance of equal to the height of the excavated substructures, iii)In case of heavy rain and strong wind occur when there are storms, we shall apply at least following countermeasures to prevent damages: -Keep close contact with relevant agencies to get information timely, -Request all labors to go to safe places before occurring of storms, strong wind or heavy rain, -Place equipment and materials on places, at least higher than surrounding area 0.5 m -Make appropriate drainage system to minimize surface water going into excavated substructure holes and prevent surface water to stay on the access road iv) In order to keep safety condition for people and property at this Project in rainy season and protection from lightning, all people working for the project shall fulfill following instructions: -Put equipment on the solid and firm ground; -Do not put equipment or machines on the edge of hole or soft ground in order to avoid falling of the equipment or machines; -Put equipment or machines at least on the distance of 1.5 time of the height of the nearest electric line in order to avoid possible electric shock; -Properly connect equipment or machines with the earth by suitable cable to avoid damage by lightning; -Run to nearest sturdy building or other similar shelters and get inside; -Do not hide under trees or open structure; -Stay away from the tallest trees or objects (such as light poles or flag poles), metal objects (such as metal fences or metal bleachers), individual trees, 19/ 29 PHUONG THANH TRANCONSIN – CIENCO6 – CIENCO8 – THANH PHAT JV CIVIL WORKS CONTRACT PACKAGE (KM 32+600-KM42+000)- DA NANG – QUANG NGAI SECTION standing puddle of water, and open fields Avoid being the highest object in a field, and not take shelter under a single, tall tree; -Do not use the mobile phone or telephone, except in emergency situations; -Do not use electric equipment and stay away from window, doors; -Allow half an hour to pass after the last sound of thunder or flash of lightning before resuming any activity 3.20 Chemical Inventory The Bidder will prepare inventory for chemical materials and material safety data sheet for the each chemical material such as solvents, lubes oils Inventory will be updated once a month or when the considerable amount of change occurred with stock quantity 3.21 Chemical Handling Procedures As for the handling for Chemical materials, suitably trained personnel will be assigned for material control Also, the storage area will be secured for prevention of intrusion At the handling area of storage, concrete slab with concrete dyke may be provided for spill control In case of spillage, spilt material will be properly absorbed with unwoven cloth or similar material and treated properly in accordance with relevant regulations 3.22 Chemical Storage The Bidder also ensure that chemical material will be stored appropriately in accordance with characteristic of each substance such as flash point, fire pint, melting point, boiling point, soluble characteristic In addition, to prevent the unsuitable mixture of each substance, each substance will be properly segregated in appropriate containers when stored Especially for storage of flammable substances, The Bidder will provide fire/ high temperature protection system for the storage area utilizing concrete block, plasterboard or similar nonflammable material For such storage area, The Bidder will also provide fire extinguishers for emergency case 3.23 Control Access to the Construction Site As access into work site by outside parties due to wide range of construction activities is accessible easy, The Bidder shall control the access to these working areas as much as possible to prevent unexpected accidents 3.24 Boundary Demarcation Right of Way This Right of Way shall be demarcated the boundary of the worksite on existing roads with yellow flags at intervals and safety board, informing the public/outside parties on The Bidder’s worksite Confined Area 20/ 29 PHUONG THANH TRANCONSIN – CIENCO6 – CIENCO8 – THANH PHAT JV CIVIL WORKS CONTRACT PACKAGE (KM 32+600-KM42+000)- DA NANG – QUANG NGAI SECTION These confined areas where the construction, mechanical plant, material stockyards, etc are located, shall be clearly marked with red flags for the attention of the public/outside parties Handrails will be installed along the boundary of these confined areas to prevent unforeseen/unexpected accidents such as falling Lighting system shall be equipped at the handrails for identification of these confined and dangerous areas from the public Fencing shall be erected along the boundary of the camp to ensure good security 3.25 Security Check Guards Guards shall be employed to check/inspect on all accesses into the worksites for security purposes They shall be stationed at the designated locations to patrol the site security Security Gate Gate shall be built at the designated position of the boundary to control access in and out of the site Notification of accidents and safety reports 4.1 Emergency Communication The emergency communication system shall be established and posted at the convenient access-ways, in all site offices, in all security towers/ gates, medical center, in order for speedy rescue of the accident victims, timely communicate with local fire brigade office, prompt response from first aid staff, ambulance services and the soonest possible evacuation of the workforce if necessary in a serious emergency First Aid station shall be maintained during all working hours This will be 24 hours if necessary First Aid Center will be equipped with telephone line and mobile phone Medical Staff will be well informed as to the project layout and be taken on regular site visits as the project changes so that they always know the access routes and stairs to ensure quickest access to injured persons The witness of the accident shall immediately inform Safety Manager, Team leader, Site Engineers or related Bidder’s person in charge to get their instruction and support to rescue labors or equipment 4.2 Bidder’s Emergency Call out Procedure Introduction The procedure is intended to be used for The Bidder’s activities and must be observed by all employees working in this project Priorities 21/ 29 PHUONG THANH the situation The First Aid Staff shall provide lifesaving assistance including CPR (Cardio Pulmonary Resuscitation) must perform the first aid treatment, if required Immediately, after being released, the Emergency Call out Procedure shall be initiated Trigger Definition Triggering must be done to conform to the following conditions: Fatal accidents Any accident that causes serious injury including loss of vital organs or severe bleeding Any accident that cause injury to the employee(s), public Any severe blockage of public traffic or passageway Any important report incidents/accident to be reported Any significant utility damage or impact The emergency situations in this Project shall include: Fire Failure and collapse of structure Failure and collapse of heavy machinery and equipment Adverse weather and flooding Procedure Initiation Site with staff working on day and night works, shall initiate the Emergency Call out Procedure The Site Engineer and Safety Manager (2nd Stage) will be called, and if they cannot be contacted, their level will be skipped and the 3rd Stage will be alerted In addition, the initiator will take prompt action to mitigate the situation When he is released he will report on: The situation in general in compliance with the trigger definition The Number of people killed / injured / involved Action taken by responsible site supervisory personnel Other vital information in brief The Site Engineer/Team leader will also report the matter to the Safety Manager and contact the duty telephone on the situation Second Stage (2nd Stage) 22/ completeness and assess the situation If necessary, he will travel to the Site to check himself Upon verification of all information involved, he will initiate the Third Stage (3rd Stage) with given telephone number(s) Third Stage (3rd Stage) The persons at the 3rd Stage (Project Manager) shall assess the situation i.e severity of accident / incident and decide whether they will proceed with Emergency Call Out or not If the situation required, he will call 4.3 First Aid Service In order to provide quick and best services to victim of possible accident, time to time Bidder will contract with Medicinal Centers at nearby communes to serve as First Aid Base for the project hence: They agreed to cooperate with the project to provide First Aid Service to the project labors on humanitarian spirit They have qualified medical staff, treatment room with suitable equipment such as emergency medical boxes with sterilizing equipment, treatment beds and sufficient medicines, stretchers, etc., They are familiar with this work because it is their daily work, They are located near the construction site The First Aid Base will be open whenever there is work being carried out at construction site The function of the First Aid Base in any emergencies is to provide first aid services to any person injured, providing first aid treatment both on the site and in the First Aid Base, and communicate with the Safety Personnel and nearest Hospitals If there is a need of immediate attention by the medical practitioner for injury/illness, the Site Engineer in-charge shall immediately report to Safety Manager and then send the injured/sick person to nearby hospital or polyclinic accordingly The First Aid Base staff will keep a record of all persons who receive treatment in the center This record will include: name of person, company, ID No., treatment received, cause of accident / illness, location of accident, medicines issued, further treatment required, ambulance use, hospitalization, and any time off work All records to be passed to Safety Manager, each week on Monday morning for the preceding week Construction team’ first aid / medical staff will be required to communes, Districts or City to provide best service to the victim of possible accident 4.4 Notification of Accidents Any injury or accident should be investigated and reported to the Employer’s and the Engineer’s Representative, Main Bidder if it involves any of the followings: Injury or accident that resulted in > days unable to work million VND Damage to property/equipment or any dangerous occurrence resulting in harm to people or properties Near miss accident Accident resulted in loss of life, public’s notice and awareness or press coverage shall be reported immediately to the Employer’s and the Engineer’s Representative, Main Bidder by telephone Further to informing them by telephone as mentioned in item No 2, The Bidder has to provide by facsimile the report in advance on the relevant issue within hours to the Employer The complete detailed report must be submitted by The Bidder to the Employer’s and the Engineer’s Representatives within 24 hours Injured person shall be responsible to report the injuries to their immediate Site Engineer as soon as possible The respective Supervisor shall decline any report submitted by another person instead of the injured person Any accident shall be immediately reported to the Safety Manager These accidents will be investigated in depth to identify all causes and to recommend hazard control measures To prevent re-accident First Aid Team came to the accident place and give necessary first aid service, Based on his assessment, Head of First Aid Team can call ambulance car and send victim of accident to Hospital for further treatment Head of First Aid Team reports Safety Manager about existing situation and his decision Safety Manager also can inform related Team leader about accident and request them to support to rescue labors If possible Safety Manager shall go to Emergency Unit at Hospital Safety Manager shall update health condition of victim and timely report to Project Manager or Construction manager Project Manager will decide and report to the Engineer and/or employer 4.7 Safety Reports Site Safety Reports shall be submitted to the Employer’s and the Engineer’s Representative, Main Bidder on Quarterly basis where the site safety inspection results should be summarized with details on accident/injury or any dangerous occurrence of that period reported These reports also shall point out measures to prevent the occurrence of similar incident or accident in the next phase Safety Manager shall keep an accurate record of the report on the man-hours exposure and inform the Engineer’s Representatives, Main Bidder upon its request Penalties for safety violations / failure to comply and award for safety success 5.1 Removal from Site For a number of actions that are mentioned throughout this Safety Plan, person that engage in such activities as: drinking on site during working time, stealing, vandalism, repeated failure to abide safety instruction, threatening behavior himself or others immediate danger This includes workers found to be not using helmet, shoes and the provided toilets and going “wherever they feel like” 5.3 Major Offences A major offence being a violation of safety rules which places the workers or other personnel within immediate danger Examples of this would be workers smoking near flammable material, workers at high risk of falling places without proper use of safety belt, carrying out welding without mask, etc In all such cases the worker will receive a verbal instruction to immediately rectify the situation, but their ID card and nature of offence will be recorded / passed to Safety Manager, and a penalty of 5,000,000.00 (five million) VND/time will be deducted from related Construction team Companies that commit major offences / allow poor safety to continue without any effort to improve / who ignore safety instructions passed to them both verbally and in writing/ or who fail to attend Safety Meetings and/or Inspections without adequate reason, shall be penalized 5,000,000.00 (Five million) VND from related Construction team 5.4 Safety Award Based on success of the Construction team in keeping safety for the work at the construction site, Project Manager will evaluate and award Construction team for its success in a range from one million VND (1) to five (5) million/ time Supervision of safety management system Review of the Safety Management System implemented by Bidder in this Contract shall be carried out when ever needed to ensure its effectiveness and conformity to the objectives of the Project Contract’s Safety Manager will evaluate suggestion, if any, which can be provided by relevant parties at the Safety Committee Meeting or via mails, letters, etc., and then report to the Project Manager and get approval from him to take the necessary follow-up action II SECURITY PLAN Security plan and site protection 1.1 Organization and cooperation of forces Before commencing construction, the Bidder will contact with relevant agencies including the Employer, the Authority and local police to agree and prepare plans for cooperation on security and explosion and fire access of the staff and vehicle at the site A group of guards will be established to control the internal security for 24/24h of the day Before construction, a list of Bidder’s staff working for the Project will be prepared and submitted to the Employer, local police and the site guard division for purposes of general management and checking The staff will be trained on regulation of working on the Site before start working All of the staff working at site will be issued badges for purposes of security and internal regulation of the Site All access to the Site will be checked by the guards and recorded Workers are not allowed to access to the areas not belong to construction areas The Bidder will solely be responsible for security on Site and other areas used for Contract’s execution The Bidder will solely be responsible for security of the visitors and will have necessary preventive method for safety and security of visitors The Bidder will define security requirements of the local government which is in construction and comply with them Before commencing of construction, the Bidder will place temporary fences, gates, barriers to separate the construction site with neighboring areas Fencing will be made of round steel piles, caped with tole and will be maintained during the construction Guards will work interchangeably 24/24 h all day around at the site gates, material storages, areas for construction plant and other important areas Cooperating with the local authority and local police in order to preserve on site security Before the construction the Bidder prepares a list of officers and workers participating in the Project The list shall be submitted to the Employer, local police, and site guardians for general inspection All officers and workers must be educated in working regulations before coming to the site All of the staff working at site will be issued badges for purposes of security and internal regulation of the Site All access to the Site will be checked by the guards and recorded The working table shall be prepared on site There should be regular inspection, remind of disciplined and timely implementation Bidder’s staff may be fined, division and shall be recorded for review Protection of Utilities The Bidder commits to cooperate with the SubBidder and on- site participation to ensure the convenient implementation of relocation work Before commencing construction work the Bidder shall undertake a survey to confirm the detailed location of all utilities affected by the Project including the trial drilling to confirm the actual location The detailed locations shall be recorded in plan form and a copy provided to the Engineer The Bidder shall be responsible for the temporary protection and support of the utilities located for the duration of the contract Appropriate markings, barriers and warnings shall be erected to ensure the utilities are not damaged during the Construction of the Works Particular attention should be paid to the overhead structures such as high voltage cables Barriers and warning signs shall be erected to ensure such structure are not damaged by high vehicles such as cranes or the raised bodies of dump trucks to ensure the health and safety of personnel on the site Fence, sign boards, gates Bidder shall take necessary measures to minimize the negative influences on the construction,
