This method statement describes the procedure and sequence of activities to be carried out during the Temporary Tower Crane erection & dismantle for Fly Ash Silo in order to ensure that the work is planned and performed in accordance with the manufacture & supplier’s installation and manual at Song Hau 1 Thermal Power Plant.

Crane Specification
- This method statement describes steps to be followed before, during and after Tower
Crane erection to ensure compliance with manufacturer’s manual.
- The method statement assigns responsibility and defines procedures for controlling the
Temporary Tower Crane erection work by systematic actions
- After installing, must check, loading test according to regulated safety norm on lifting
equipment - Article 6.3 TCVN 4244 – 2005.

4.1 Project Manager:
The Project Manager shall ensure that the erection work is performing to the required
Project quality standard and Schedule The Project Manager is also responsible for
managing the erection work of consultants, civil and fabricating resources in an efficient manner The Project Manager is responsible for the following:
- Planning: looking ahead to implementation and determining the elements of the project for each element, scheduling durations, resources, costs and etc
- Organizing: selecting the project team and determining the team’s responsibilities;
- Managing: coordinating activities of the project team, contractors, consultants, clients, and/or financing agency;
- Supervising: implementing control techniques for design reviews, construction checks, milestone assessments, progress meetings, and reports, both informal and formal; and
Trang 4- Financial controlling: administering financial aspects such as invoice approvals, progress certificates, payment of accounts, etc.
- Etc.
4.2 Site Manager
The Site Manager has the responsibilities for the application of this Method Statement and detail technical aspects, which is defined in this procedure inclusive of ensuring that all safety measures are implemented Site Manager shall:
- Monitoring and implement the construction phase Health & Safety plan.
- Ensure works are executed within legislative and Project requirements.
- Manage the Subcontractors, Risk Assessments and Method Statements as applicable.
- Approve Method Statements and JSA.
- Prepare Risk Assessments and Method Statements for directly operatives
- Maintain a working knowledge of H&S legislation including changes to the legislation.
- Carry out audits of H&S as required by Project HSSE procedures.
- Advising others of H&S requirements as required.
- Determining the scope of the package in conjunction with the Project Manager.
- Monitoring and controlling the progress of the subcontractors by means of established control methods (progress meetings, short‐term programmes etc).
- Monitoring progress of works based on Contract Programme and record as necessary.
- Ensuring Company operating systems are adhered to in all respects.
- Setting up the required operating systems at project commencement.
- Maintenance of daily records.
- Providing records to support Project Manager.
- Attending meetings as required.
- Report on changes which are affective to works and comment on day works.
- Identification and discussing with Project Manager possible commercial opportunities arising from client information / changes or subcontract information /changes.
- Etc.
4.3 Construction Manager
Construction Manage shall overall take construction management of the Project.
- Planning, developing and organizing to the construction plan and schedule of erection work to implement the execution in accordance with the approved Project Schedule.
- Be responsible for implementation of the scope of work related to construction/
fabrication and erection in accordance with the approved Project Schedule.
- Monitoring and reporting to Project Manager project details, including progress, risks and opportunities in a timely manner.
- Ensuring that all changes to specifications, work scope and drawings are documented
- Defining clear roles & responsibilities and deliverable requirements in terms of both
scope and schedule to all the team members.
- Reviewing man-hours and duration forecasts for completion onshore construction and man-hours, duration and manning forecasts for offshore hook-up and offshore installation durations and resource requirements.
Trang 5- Monitoring construction productivity and schedule performance and investigate reasons for less than satisfactory performance Provide recommendations and institute measures for improvement by modification to operating procedures/work instructions.
- Etc.
4.4 HSSE Manager/ Officer
HSSE Manager takes responsibility of all ESSH (Environmental Safety Security and Health) activities at site
HSSE Manager/ Officer shall:
- Ensure that work is carried out in safe manner and also ensure that tool box talks, pre talk ESSH meetings are conducted by concerned supervisors and shall ensure proper mode of communication in case of emergency
- Ensure personnel adhere to method statement, hazards and sequence of work
- Ensure that all the activities are carried out in accordance with the relevant safety
regulations.
- Practice safe working techniques and re-enforce the safety policies, guidelines, and
procedures as setting out by the Company.
- Provide daily guidance and direction in EHS programs for the employee / supervisor
level to ensure a common and effective approach;
- Be responsible for assisting the identification of possible losses; evaluating the risk such
as losing exposure; developing and implementing plan to correct or prevent loss and audit the plan to ensure its effectiveness.
Ensure that contractor personnel working on the property are in compliance with Avalon policies and safety requirements.
- Maintain and update as needed SOP’s for all safe work practices, equipment and utilize them in the day to day training.
- Provide advice, assist, and audit line management to ensure that effective EHS is
implemented and working.
- Perform and contribute to the Site Training Plans by conducting employee orientations; emergency response training; and coordination of modular training programs for the
Operation.
- Ensure inspections by regulatory agencies which are coordinated and deficiencies
minimized and corrected.
- Coordinate with VP, Operations and General Manager on facility deficiencies and
monthly safety inspections
- Maintain all logs and documentation associated with permits and regulatory
requirements;
- Assist with maintaining the Site Preparedness and Emergency Plans & Procedures.
- Be responsible for coordinating regulatory programs and its applications and help support the compliance
process; supporting and providing relief for the environmental department.
- Be accountable for researching and writing permit applications and work with
environmental group and be compliant in completing permit reporting requirements.
Trang 6- Review accidents and incidents which occur in the area of responsibility and conduct
field audits, reporting and managing the corrective actions.
- Maintain filing systems regarding worker training and safety;
- Maintain working relationships with Regulatory Agencies and chair or co- chair and lead the Safety Committee for the Operation.
- Manage Team Base Risk Assessments and the process and regularly conduct training needs analysis.
- Lead the Emergency Response Team and comply with all Company policies and
procedures.
- The above duties and responsibilities are representative of the nature and level of work assigned and are not necessarily all-inclusive.
- Travel to site including cooperating and operational offices and perform any additional duties as required.
4.5 QA/QC Manager
The QA/QC Manager shall be responsible for ensuring that these activities complied with the relevant standard procedures and designer’s recommendations Quality control engineer shall take responsible for the documentation and inspection in accordance with the QA/QC manual and specifications.
QA/QC Manager shall:
- Lead and manage the QA/QC team and expertise to the overall Quality Management
System, policies and standards to support properly and smoothly execution of all Quality related to activities in meeting of the Project which specified standards and requirements
- Ensure all work meets or exceeds the Project specifications and standard
- Ensure all materials meet or exceed the contract specifications.
- To be able to document, monitor, track, and submit laboratory data.
- Coordinate and schedule field testing and performance QA/QC inspections.
- Monitoring daily construction activities and materials delivered to the site.
- Review all special inspection and material testing perform by third parties.
- Provide documentation all inspection and testing in QC reports.
- To implement preventive measure and corrective action for non-conforming works.
- Be responsible for production testing of raw material and other related testing and ensure testing is in compliance to internationally accepted guidelines.
- Be able to demonstrate implementation, documentation & monitoring experience in the development & application of ISO 9001, ISO 14001, OHSAS 18001 and other.
- Etc.
4.6 QC Supervisors
- QC Supervisors are responsible to directly observe/ control the quality of each working team Detail of organization diagram shall be determined in construction stage.
- QC Supervisors instruct to working team about technical requirement, to internal
inspection and report to QA/QC manager for final inspection.
- QC Supervisors provide the requests for inspection (RFI) of work, and attend the
inspection at site in corporate with DHI/ LLM/ Owner/ PMC.

4.7 Construction Supervisor (Engineer)
- Supervisor shall supervise closely the activities in accordance with method statement and project specification and ensures that required instructions are followed and all necessary work permits are placed prior to commencing the work and safety procedures of project are strictly adhered to the requirements of the project.
- Taking necessary precautions to protect the safety and health of workers under their
supervision;
- Ensuring that workers comply with safety and health procedures and use safety
equipment, clothing, and devices;
- Advising workers of safety and health hazards in the work area;
- Cooperating with the workplace safety and health committee or representative;
- Cooperating with other people on workplace safety and health matters
Comply with the of Project HSSE Regulation
4.7.1 Crane and Equipment Operator
The crane and equipment operator shall be qualified and certified (Where is available for any certification from the authority in Vietnam) and fully trained to operate the crane and equipment before starting works and also getting approval as per related site regulation at site And keeping the related site requirements such as ESSH requirement.
4.7.2 Foreman
Foreman shall be fully experienced on the related works who has the responsibility of supervising a team at a construction site A foreman is in charge of making sure that
projects are completed safely and comply with building regulations and legal
requirements They also monitor the progress of projects and ensure that the work is
completed to the highest standard And keeping the related site requirements such as
ESSH requirement
4.7.3 Workers
Workers shall be properly carried out on the related works who has the responsibility of doing the work as per supervisor’s, foreman’s and any other’s instructions and related technical specifications such as drawing, document and etc also, any kind of worker
groups shall be as per related code/specification and site regulations And keep the related site requirements such as ESSH requirement.
