sĩ DRP chung 41 MỞ ĐẦU Trong trình điều trị bệnh tật, thuốc ln đóng vai trị quan trọng để giảm nhẹ, chữa khỏi tình trạng bệnh Song, việc dùng thuốc không hợp lý gây bất lợi khiến bệnh nhân gặp vấn đề liên quan đến thuốc (drug related problems) Vấn đề liên quan đến thuốc định nghĩa cố liên quan đến việc sử dụng thuốc điều trị gây hại tiềm ẩn mối nguy hại cho sức khỏe người bệnh [60] Trên giới, nhiều nghiên cứu cho thấy tần suất xuất vấn đề liên quan đến thuốc đơn thuốc lớn xảy đối tượng bệnh nhân Tại Pháp (2020), nghiên cứu cho thấy có tới 77,4% bệnh nhân có đơn thuốc xuất vấn đề liên quan đến thuốc [62] Một nghiên cứu tương tự Indonesia (2020) ghi nhận khoảng 57,0% đơn thuốc có vấn đề liên quan đến thuốc, hầu hết đơn định thuốc không phù hợp [48] Các vấn đề liên quan đến thuốc thường xuyên xảy ra, làm ảnh hưởng đến chất lượng sống, tăng tỷ lệ mắc bệnh tử vong [77] Do đó, can thiệp dược sĩ để cảnh báo đề giải pháp làm giảm thiểu vấn đề liên quan đến thuốc nhằm nâng cao hiệu điều trị cho bệnh nhân thật cần thiết Hiện nay, vai trò dược sĩ việc can thiệp kịp thời cũng minh chứng nhiều nghiên cứu Từ nghiên cứu Budiastuti R.F (2019) cũng ghi nhận kết số lượng vấn đề liên quan đến thuốc trẻ bệnh bạch cầu lympho cấp tính Indonesia giảm sau có can thiệp dược sĩ lâm sàng [21] Các công việc kiểm tra đơn thuốc thường xuyên xác định ngăn ngừa vấn đề liên quan đến thuốc, tránh tổn thất tài gia tăng giá trị khơng thể đo lường để đảm bảo an toàn cho bệnh nhân [10] Ở Việt Nam, việc kê đơn thuốc tồn nhiều bất cập, không hợp lý việc nhiều sở khám chữa bệnh định nhiều thuốc đơn, lạm
