Project Manager No2 (Health and safety Construction Managers and Site Engineers Health and Safety officer Safety checker (From Km0+000 to Km1+000) Technical Schedule Safety checker (From Km1+000 to Package 7: chainage from 0+000 to The project Manager is responsible for the establishment of the Project Health and Safety Plan programme This Programme shall be according to the Bidder target with the recommendations and assistance of the Deputy Project Manager No2, Contruction Managers and the safety Officer Accident prevention procedures will be included in all activities by the application of sound safety planning Specifically, the Project Manager shall set the pace for the project health and safety programme with the safety Officer and shall act as follows: Direct the activities of the safety Team as the Project Manager Promote full support of the health and safety programme Delegate responsibilities for the various safety functions and authorities with health and safety promotion at all levels of the Project Maintain active involvement in the Project’s safety status and safety morale Ensure prompt investigation action on any accident investigations with proper reports completed and follow – up action taken 1.2.2 Project QEHS Manager The project QEHS Manager (in this Project he is Deputy Project Manager No2) is appointed by the Project Manager, to be in-charge of the quality, environmental, health and safety aspects of the activities on site with the system implemented in this Project He will oversee the activities concerning these aspects by reviewing/discussion with the Safety Officer or even together with the Project Manager to ensure the compliance of the laws and regulations The specific health and safety responsibilities of the Project QEHS Manager include, but are not limited to, the following: Responsibility for all safety activities on the Project and maintain liaison with the Construction Manager/Safety Officer Establish and direct the site Health and Safety Plan programme Ensure and guide Safety Officer to promote activities onsite to achieve the Project’s objectives and targets Ensure the Safety Team is formed and actively involved in the health and safe environments matters in the Project Technical Schedule Health & safety plan, Security assurance of 32 NORTH SOUTH EXPRESSWAY CONSTRUCTION project-bidding document HO CHI MINH CITY – DAU GIAY SECTION 2+000 Package 7: chainage from 0+000 to Ensure compliance to the health and safety programmes by all departments in this Project Establish safety rules with special attention to unusual operations and safety requirements of the Employer Ensure information and follow-up action of Safety Officer after discussion with Project Quality Manager Involve with the planning for the operations where the possible hazard may happen or the safety level is below the normal Oversee the implementation and execution of Safety Programmes and their effectiveness 10 Suggest/Support the Safety Officer on health and safety programmes such as training programme for approval from the Project Manager 11 1.2.3 Monitor/Check any unsafe acts during site patrols Construction Manager The Construction Manager is the key figure when it concerns the monitoring of the contractual obligations of the various contractors on the job One of these obligations within our total program is safety The Construction Manager is responsible for the training and directing of site engineers and office engineers which includes their safety responsibilities The Construction Manager shall be prepared to accept the responsibilities for administering the site safety programme during the absence of the Safety Officer from the Project Manager The Construction Manager shall take and active part to the Safety Team He shall review the specifications and prints for safety compliance and relate his findings to this team The Construction Manager will co-ordinate activities with the Safety Officer regarding construction teams and subcontractor performance and take whatever steps are necessary for proper safety compliance He shall assist the safety Officer on problems or a technical nature As administrator of the health and safety programme, he shall function as the Safety Co-coordinator advisor and consultant to the Project Manager and supervisory personnel in establishing and maintaining compliance with the health and safety programme Safety Officer shall be a suitable qualified and experienced person with complete involvement with the safety activity throughout the period of this Project Technical Schedule Health & safety plan, Security assurance of 32 NORTH SOUTH EXPRESSWAY CONSTRUCTION project-bidding document HO CHI MINH CITY – DAU GIAY SECTION 2+000 Package 7: chainage from 0+000 to The Safety Officer’s specific responsibilities include the followings: Coordinating the site safety efforts with the Project Manager and the Employer’s health and safety requirements Keeping the site management and the Project QEHS Manager informed of the progress of the health and safety programmes and making recommendations for site related safety improvements Administer site health and safety policies and procedures within the framework of the Bidder Health and Safety Plan particularly emphasis on potential hazardous operations Proposing programmes to implement the accomplishment of short and long range of site health and safety objectives and to motivate all employee participations to obtain these objectivies Preparing , submitting and distributing the required safety and injury reports with records kept in his file Maintaining a continuing inspection programme and reporting the findings and recommendations to the Project Manager/Managers/Constractors for prompt corrections Conducting and/or participating in job safety meetings Supervising safety trainings in accordance with the site requirements Coordinating the first aid and rescue services on site with certification to ensure the appointed employees are equipped with proper first aid know-how in the absence of a nurse 10 Establishing a fire prevention programme with training on the actions and participation of the employees for designated duties such as informing the Fire Brigade 11 Investigating all accidents, injuries, fires, property damage and other safety related incidents and prepare/submit the required report 12 Identify the classification of injuries if they are occupational or time losing, disabling injuries and take the follow-up actions on the control of injury cases with appropriate accident forms to be filled 13 Assisting Sub-Contractors with their health and safety program to meet the various site health and safety requirements 14 Notify the Project Manager and Project QEHS Manager of all health and safety inspections with any action to be taken Technical Schedule Health & safety plan, Security assurance of 32 NORTH SOUTH EXPRESSWAY CONSTRUCTION project-bidding document HO CHI MINH CITY – DAU GIAY SECTION 2+000 Package 7: chainage from 0+000 to 15 Evaluating the needs and then, request of safety, first aid and fire protection equipment needed on the job Ensure that safety, equipment is maintained in good condition at all times 16 Check/Provide safety approval for new tools and equipment, including tools needed for specific job 17 Effective display and maintain publicity of news of health and safety on the site bulletin boards, posters, safety signs, banners and distribution of safety literature to instill a good and positive attitude on health and safety environment among the people of all levels on site at all times 18 Administer and/or monitor the site hazardous waste program 19 Administer and/or monitor the Occupational Health and Safety Act and the hazardous materials information system regulation on worksite 20 Attend pre-job meetings with Sub-Contractors and ensure the Sub-Contractors comply to the Bidder safety policies and procedures during their work in the Project 21 Plan the site health and site safety program timely to achieve the required site health and safety policies and procedures accordingly 22 Maintain an updated knowledge of al health and safety policies and procedures with knowledge of state provincial and local safety laws and regulations 23 Maintain updated knowledge of state-of-the-art concepts of accident prevention and continually contributing to upgrading the site safety effort 24 Execute effectively on any other assignments delegated by the Project Manager Construction Manager The Site Engineers will be directly responsible for the success of the health and safety programmes as they are in direct contact with the SubContractors Their abilities to instill good health and safety attitudes to the SubContractors will reflect the success of the health and safety programmes on site Site Engineers are responsible for a wide range of activities They must plan their health and safety activities with care and good effort as they have to carry out other portions of their work programme such as cost, manpower allocation and scheduling of the respective work within this Project The specific health and safety responsibilities of Site Engineers include, but are not limited to, the following: Review all work and all new personnel for safety needs Complete familiarization with health and safety requirements is an integral part of the total job responsibility Technical Schedule Health & safety plan, Security assurance 10 of 32 NORTH SOUTH EXPRESSWAY CONSTRUCTION project-bidding document HO CHI MINH CITY – DAU GIAY SECTION 2+000 11 Record must be kept for all inspections carried out 6.2 Work schedule and safety instruction Package 7: chainage from 0+000 to Site Engineer with the assistance from Safety Officer shall review the Safety performance of Sub-Contractor with their safety standard or practice on site If it shows that the standard is below the requirements set forth in the Project Management Plan and Work Schedule, Instruction will be issued to the management of Sub-Contractor for their necessary follow- up action of rectification Safety officer will then check on the completion of the remedial actions/work and report to the Project Manager Safety devices Protective guards to be fitted to gears, shafts, pulleys, lifting devices, etc to prevent contact to any person when operated Proper ratchets or other positive looking devices for drums on load-hoisting equipment are required Stoppers are required on hook of crane to prevent rope from slipping off Looking devices for electrical panel, boxes, etc … must be provided Safe Operation 7.1 Safety Housekeeping 7.1.1 Daily Safety Housekeeping Cycle Cleaning of work places and checking of safety facilities at the end of the day on daily basis before leaving the site, shall be performed by all workers and checked by their Site Engineers The night shift work following the earlier day work requires the concluded work done schedule with the daily duties carried out Tool Box meetings or discussion group meetings shall be held before the start of the night shirt work so that the jobs to be carried out are instructed accordingly 7.1.2 Weekly Safety Housekeeping Cycle Before the start of the work on each Monday morning, safety information of the previous week carried out and its activities with the planned schedule for the current week shall be informed to all staff and workers Cleaning of work places by all workers shall be carried out after the document HO CHI MINH CITY – DAU GIAY SECTION 2+000 7.1.3 Package 7: chainage from 0+000 to Monthly Safety Housekeeping Patrol/Cycle Monthly Safety Patrol attended by representatives of the Bidder and the Engineer shall be carried out with reports prepared by the Safety Officer from these patrols These reports shall be issued to all attendees at the Safety Patrols and also to discuss any issues at the Daily Meeting for follow-up actions The monthly patrol shall be held at end of each month as part of the monthly schedule meeting whereby discussion on suggestions for any site safety/work schedule relating to the patrol can be taken into action for the following month 7.2 Working at Height Scaffolding erection to be carried out, when working at elevated area Safety concerning the erection, maintenance and dismantling of these scaffoldings is mandatory to ensure safe operation on site 7.3 All scaffoldings to be inspected/checker at regular intervals as required by the manufacturer’s or the legal requirements for well-maintained condition Trained and competent workers with supervision for this work Any deflective parts of the scaffolding materials shall be removed from the storage area Working platform shall be sufficiently strong, properly secured and not overloaded with toe-boards to prevent falling objects Erect barricades or overhead shelter wherever practicable to prevent person from being struck by falling objects Open-sides/working platforms higher than safe allowable height may be properly barricaded All personnel working at heights have to wear their personal safety gear at all times Routine checks on erected scaffoldings e.g Daily site check, Monthly safety patrol, etc shall inspect such scaffolding to ensure the tag of records of checks conducted as per requirements Safety nets should be around the scaffolding if exceed the safe allowable height to prevent any falls of materials Equipment/ Machineries Limitation of working space for works to be carried out within construction site may affect movements of workers As a result, accident may happen when the machineries accidentally knocked/hit workers working too close to them If such situation arises, the followings must be observed; Technical Schedule Health & safety plan, Security assurance 19 of 32 NORTH SOUTH EXPRESSWAY CONSTRUCTION project-bidding document HO CHI MINH CITY – DAU GIAY SECTION 2+000 Package 7: chainage from 0+000 to Keep a safe distance depending on the type of work to be carried out by the workers within close vicinity of machineries Site Engineer should inform such requirements to the workers and operators of machineries Appoint signalmen/riggers to control the movement of the machineries during these periods Do not store too many unnecessary items at this congested area Housekeeping is requited If this situation is dangerous and congested, review work schedule and carry out work in sequence Reflective personal vest/clothing provided for better visibility Daily inspection on construction machinery, equipment and tools shall be conducted The inspection records shall be kept for designated period Heavy machines such as cranes, bulldozers shall be inspected by qualified personnel or relevant authorities Inspection records shall be kept Safety officer is responsible to confirm the inspection status of subcontractors’ machines Moving parts of machines shall be covered to prevent human bodies from getting caught Electric machines shall be properly earthed to prevent electrification accidents Properly insulated electric cables shall be wired in good order and indefinable as to the connection with each electric machine Proactive measures shall be taken to prevent foreseeable accidents, e.g setting barricades and handrails, etc Signals toward the crane operators and riggers shall be determined 7.4 Pressurized Equipment All pressurized equipment and systems shall be approved, tested and equipped with safety or relief valves and pressure gauges All pressurized equipment or systems confirmed to be unsafe shall be tagged “OUT OF SERVICE”, “DO NOT USE” Use of such unsafe equipment is prohibited Pressurized equipment shall be operated and maintained only by qualified and authorized personnel Technical Schedule Health & safety plan, Security assurance 20 of 32
