Long Thanh Expressway Project Package A5 Civil Works Km 32+450 to Km 35+900 I HEALTH AND SAFETY PLAN It is the prime concern of the Contractor that the works to be carried out at the construction site are of the highest safety standard The Contractor's management level will be responsible for ensuring that safety is given top priority which it deserves and that the safety objectives which are identified are, so far as is reasonably practicable Project Health and Safety Organisation and Responsibility 1.1 Layout of management Please see as bellows 1.2 Policy of Safety The Contractor will closely monitor the safety performance of the Project and will take all practical steps to maintain safety in all fields of works In order to achieve the safe performance of any interface work areas, the Contractor will cooperate with the Employer in solving safety problems arising from respective areas and will coordinate with any other contractors on the Project construction site The policy of the Contractor's management level is to accept and implement the safety rules and regulations, standards and codes of practice of the local Government and the Employer in addition to such internal policies as may be in existence within the Contractor's organization And to achieve aforesaid policy, the management will be properly considered and appropriately developed and implemented the following: - Identify the common and serious hazards and resolve them - Conduct safety audits to evaluate the safety performance - Compliance with the Employer's safety policy at all levels of management - Plan and implement accident prevention programmes and measures - Plan and implement fire safety and emergency control measures and procedures Operational success will be achieved in conjunction with a good and effective accident prevention practice based on internationally accepted codes and practices All local Government's legislation will be considered in establishing the obsolete minimum requirements for policy The management will take all reasonable steps within its power to meet its responsibilities and shall pay attention to the provision of the following: PHUONG THANH TRANCONSIN GMS Ben Luc - Long Thanh Expressway Project Package A5 Civil Works Km 32+450 to Km 35+900 - Plant, equipment and systems of work that are safe and suitable for the purpose intended - Safe arrangement for the handling, transportation and storage of equipment, materials, etc - Monitor the safety activities programme - Acquaint management with the legal aspects of the safety rules and regulations - Equip managers with the necessary tools for the successful execution of safety activities in order to prevent injury to personnel and/or damage to property - Keep records in compliance with the requirements of all local safety rules and regulations - Provide all workmen with safety provisions and ensure the correct usage of all necessary protective clothing and safety equipment while carrying out the work - Conduct in-house training through: i) on the job training; ii) on the job corrections, and iii) tool-box meeting - Promote safety at all levels through safety meetings, safety promotion activities and campaigns, and safety competitions with incentive awards - Provide safety banners, safety signboards, posters, etc and display them at conspicuous points or location as constant reminders - Establish a safety committee which will meet regularly to review all existing safety measures - Appoint a safety officer to assist the management team to identify and solve problems and to work out methods of establishing and maintaining a safe and healthy working environment PHUONG THANH TRANCONSIN GMS Ben Luc - Long Thanh Expressway Project Package A5 Civil Works Km 32+450 to Km 35+900 PHUONG THANH TRANCONSIN GMS Ben Luc - Long Thanh Expressway Project Package A5 Civil Works Km 32+450 to Km 35+900 1.3 Responsibility a) Project Manager The project Manager is responsible for the establishment of the Project Health and Safety Plan programme This Programme shall be according to the Bidder target with the recommendations and assistance of the Deputy Project Manager, Contruction Managers and the safety Officer Accident prevention procedures will be included in all activities by the application of sound safety planning Specifically, the Project Manager shall set the pace for the project health and safety programme with the safety Officer and shall act as follows: b) - Direct the activities of the safety Team as the Project Manager - Promote full support of the health and safety programme - Delegate responsibilities for the various safety functions and authorities with health and safety promotion at all levels of the Project - Maintain active involvement in the Project’s safety status and safety morale - Ensure prompt investigation action on any accident investigations with proper reports completed and follow – up action taken Project QEHS Manager The project QEHS Manager is appointed by the Project Manager, to be in-charge of the quality, environmental, health and safety aspects of the activities on site with the system implemented in this Project He will oversee the activities concerning these aspects by reviewing/discussion with the Safety Officer or even together with the Project Manager to ensure the compliance of the laws and regulations The specific health and safety responsibilities of the Project QEHS Manager include, but are not limited to, the following: - Responsibility for all safety activities on the Project and maintain liaison with the Construction Manager/Safety Officer - Establish and direct the site Health and Safety Plan programme - Ensure and guide Safety Officer to promote activities onsite to achieve the Project’s objectives and targets - Ensure the Safety Team is formed and actively involved in the health and safe environments matters in the Project - Ensure compliance to the health and safety programmes by all departments in this Project - Establish safety rules with special attention to unusual operations and safety requirements of the Employer PHUONG THANH TRANCONSIN GMS Ben Luc - Long Thanh Expressway Project Package A5 Civil Works Km 32+450 to Km 35+900 c) - Ensure information and follow-up action of Safety Officer after discussion with Project Quality Manager - Involve with the planning for the operations where the possible hazard may happen or the safety level is below the normal - Oversee the implementation and execution of Safety Programmes and their effectiveness - Suggest/Support the Safety Officer on health and safety programmes such as training programme for approval from the Project Manager - Monitor/Check any unsafe acts during site patrols Construction Manager The Construction Manager is the key figure when it concerns the monitoring of the contractual obligations of the various contractors on the job One of these obligations within our total program is safety - The Construction Manager is responsible for the training and directing of site engineers and office engineers which includes their safety responsibilities - The Construction Manager shall be prepared to accept the responsibilities for administering the site safety programme during the absence of the Safety Officer from the Project Manager - The Construction Manager shall take and active part to the Safety Team He shall review the specifications and prints for safety compliance and relate his findings to this team - The Construction Manager will co-ordinate activities with the Safety Officer regarding construction teams and subcontractor performance and take whatever steps are necessary for proper safety compliance - He shall assist the safety Officer on problems or a technical nature As administrator of the health and safety programme, he shall function as the Safety Co-coordinator advisor and consultant to the Project Manager and supervisory personnel in establishing and maintaining compliance with the health and safety programme Safety Officer shall be a suitable qualified and experienced person with complete involvement with the safety activity throughout the period of this Project The Safety Officer’s specific responsibilities include the followings: - Coordinating the site safety efforts with the Project Manager and the Employer’s health and safety requirements - Keeping the site management and the Project QEHS Manager informed of the progress of the health and safety programmes and making recommendations for PHUONG THANH TRANCONSIN GMS Ben Luc - Long Thanh Expressway Project Package A5 Civil Works Km 32+450 to Km 35+900 site related safety improvements - Administer site health and safety policies and procedures within the framework of the Bidder Health and Safety Plan particularly emphasis on potential hazardous operations - Proposing programmes to implement the accomplishment of short and long range of site health and safety objectives and to motivate all employee participations to obtain these objectivies - Preparing , submitting and distributing the required safety and injury reports with records kept in his file - Maintaining a continuing inspection programme and reporting the findings and recommendations to the Project Manager/Managers/Constractors for prompt corrections - Conducting and/or participating in job safety meetings - Supervising safety trainings in accordance with the site requirements - Coordinating the first aid and rescue services on site with certification to ensure the appointed employees are equipped with proper first aid know-how in the absence of a nurse - Establishing a fire prevention programme with training on the actions and participation of the employees for designated duties such as informing the Fire Brigade - Investigating all accidents, injuries, fires, property damage and other safety related incidents and prepare/submit the required report - Identify the classification of injuries if they are occupational or time losing, disabling injuries and take the follow-up actions on the control of injury cases with appropriate accident forms to be filled - Assisting Sub-Contractors with their health and safety program to meet the various site health and safety requirements - Notify the Project Manager and Project QEHS Manager of all health and safety inspections with any action to be taken - Evaluating the needs and then, request of safety, first aid and fire protection equipment needed on the job Ensure that safety, equipment is maintained in good condition at all times - Check/Provide safety approval for new tools and equipment, including tools needed for specific job - Effective display and maintain publicity of news of health and safety on the site PHUONG THANH TRANCONSIN GMS Ben Luc - Long Thanh Expressway Project Package A5 Civil Works Km 32+450 to Km 35+900 bulletin boards, posters, safety signs, banners and distribution of safety literature to instill a good and positive attitude on health and safety environment among the people of all levels on site at all times - Administer and/or monitor the site hazardous waste program - Administer and/or monitor the Occupational Health and Safety Act and the hazardous materials information system regulation on worksite - Attend pre-job meetings with Sub-Contractors and ensure the Sub-Contractors comply to the Bidder safety policies and procedures during their work in the Project - Plan the site health and site safety program timely to achieve the required site health and safety policies and procedures accordingly - Maintain an updated knowledge of al health and safety policies and procedures with knowledge of state provincial and local safety laws and regulations - Maintain updated knowledge of state-of-the-art concepts of accident prevention and continually contributing to upgrading the site safety effort - Execute effectively on any other assignments delegated by the Project Manager Construction Manager The Site Engineers will be directly responsible for the success of the health and safety programmes as they are in direct contact with the SubContractors Their abilities to instill good health and safety attitudes to the SubContractors will reflect the success of the health and safety programmes on site Site Engineers are responsible for a wide range of activities They must plan their health and safety activities with care and good effort as they have to carry out other portions of their work programme such as cost, manpower allocation and scheduling of the respective work within this Project The specific health and safety responsibilities of Site Engineers include, but are not limited to, the following: - Review all work and all new personnel for safety needs Complete familiarization with health and safety requirements is an integral part of the total job responsibility - Continual monitoring of the safety attitudes of Sub-Contractors and their responsibilities towards the health and safety of their trade and participations in the job safety effort - Enforce on compliance with all established health and safety regulations before and during performance of the work - Conduct health and safety inspections and make corrections on a daily basis PHUONG THANH TRANCONSIN GMS Ben Luc - Long Thanh Expressway Project Package A5 Civil Works Km 32+450 to Km 35+900 1.4 - Ensure that proper trade health and safety training is provided - Initiate plans prior to the start of work to supply the tools, materials, and equipment necessary to the job safety - Assist with accident investigations to ensure proper reporting and documentation with prompt corrections made after accident and to prevent any recurrences - Co-ordinate planning and work activities with other supervisory personnel performing work within, or adjacent to, their assigned work areas - Site Engineers who have responsibility for contractor’s operations will monitor Sub-Contractor’s compliance with the site health and safety programme This activity will be coordinated with the safety Officer Health and safety checker The health and safety checkers are key people on construction site to prevent accident from happening They are the personnel at the front line, directing work activities where by they are in control over the workers for the respective works which have the likelihood of victims from potential accident Therefore, the Safety Checkers have to know and be responsible for the followings: - Check their workers at the start of work daily to make sure these workers are ready to carry out their assigned tasks from their knowledge /information of their workers under their charge and also the physical and mental state of each worker daily - Observe and monitor continuously all workers on their knowledge and capabilities on their works and health & safety issues - Follow-up continuously with site inspections to ensure health and safety instructions are carried out such as personal protective equipment required to be used and in proper order - Give specific health and safety instructions with each job assignment according to each worker’s knowledge of job with related impacts to the environment, health and safety - Ensure workers under their charge use the proper well-maintained tools and equipment for each job - Check for any incident at the end of the day daily and report to safety Officer whenever necessary - Inform Safety Officer if unsafe acts or conditions found which they are not responsible PHUONG THANH TRANCONSIN 10 GMS Ben Luc - Long Thanh Expressway Project Package A5 Civil Works Km 32+450 to Km 35+900 5.2 Insulating personal safety gears such as insulating gloves, insulating line hose, etc to be provided for electrical staff Specification Specification for the common personal protective gears is as shown below: - Any headgear:
