Find out more at http://www.billwilsoncenter.org/bwc_jobs/ February 16, 2017 JOB TITLE: Case Manager / Housing Specialist – Victims of Crime (VOCA) SUMMARY: The program provides intensive community-based services to young adults (ages 1825) identified as having the highest housing needs in San Clara County, and relies on a diverse staff and innovative program model to effectively meet the needs of this underserved and often marginalized population Specific supports and services offered by this team for TAY housing include: case management/referral to mental health services, housing search, skill building (independent living, vocational, community safety, and emotional/behavioral), socialization and recreational activities, peer and family relationship building, academic support and coordination, employment exploration, and permanent housing search support ESSENTIAL DUTIES AND RESPONSIBILITIES: Case Management • Complete case management assessment on all clients entering caseload, develops, implements and revises individual case plans and consistently monitors progress toward goals • Provide on-going supportive and/or case management functions in accordance with the problems, needs and strategies identified within the case plan in order to help the clients achieve the stated goals and objectives This includes communicating regularly with housing specialist, landlords, probation officers, social workers and family members, regarding client’s treatment progress, etc., when appropriate • Works with supervisory assistance to evaluate possible decisions regarding client status and makes decisions where there is no opportunity to seek supervisory assistance • Administer VI- SPDAT to assess needs • Participates actively as a team member in supporting clients and the facility as needed • Consults and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining treatment goals • Provides crisis management for clients; makes linkages for interventions as appropriate • • • • • Maintains client and program records in accordance with applicable standards and regulations, grant requirements, etc Maintains a high level of ethical conduct regarding confidentiality, dualrelationships, and professional stature Participates in continuing education activities, remaining knowledgeable in area(s) of expertise Attends weekly case consultation meetings and monthly trainings; meet regularly with supervisor to exchange pertinent information and receive supervision Other duties as assigned Housing Specialist • Searches for housing in the community to meet the needs of the program • Assists client to locate their own housing when it is time to transition out of the agency rented apartment • Engages landlords in the community to educate them about Bill Wilson Center and advocate for the housing needs of the TAY served by this program General • Participate in all Quality Assurance and Quality Improvement activities • Ensure that any BWC vehicles used are kept tidy and that any maintenance or safety issues are reported to the Facilities Manager immediately • Ensure that site is neat and tidy and report all maintenance or safety issues to manger or Facilities Manager immediately • Regular attendance is a requirement of the position • Other duties may be assigned QUALIFICATIONS: Bachelor’s Degree in Counseling, Social Work or related field from a four-year college or university or equivalent combination of education and experience Housing needs and assistance experience required Experience working with culturally diverse population Current CPR and First Aid Certification required (or complete within 30 days of hire) Valid California drivers license with access to a personal vehicle with current insurance that can be used for company business Fingerprinting/background check as required by the Department of Justice and/or Title XXII regulations TB test required SALARY RANGE: Benefits include 100% employer paid Medical, Dental, Vision, Life and AD&D Insurance, Long Term Disability, Employee Assistance Program (EAP), Travel Assistance Program as well as access to Flexible Spending Account for Health and Dependent Care, PreTax Commuter Benefits, Retirement Plan with discretionary Employer Contribution, generous Personal Time Off (PTO) Plan and 12 Paid Holidays In addition, we offer extensive training, mileage reimbursement, cell phone stipend, staff Wellness Program/Activities and much more!
