the works of Leak Test for Manholes and Pipe and detail the steps to be taken in meeting the technical standards and HSSE requirements of NSRP Complex Project SCOPE OF WORK This Method Statement covers the works related to inspection and leak detection (if any) for all the manholes which are located at the areas of C200 and C300 to ensure that the entire execution stages of work is done in source or be tested to the • satisfaction and approval of Contractor 6.1 Concrete Brick Concrete brick shall be used for construction of Temporary wall for plugging opening holes on Manholes, it shall be removed after completion of Leak test 6.2 Rebar (Reinforcement) Rebar for Concrete work (at the connection location between RC Pipes and Manholes) shall comply with ASTM A-615 Grade 60 and approved by JGCS (refer to D-000-13201001V Drawing) 6.3 Structure Concrete Concrete using for installation of Pipes (GRP Pipes or RCP Pipes) is concrete type Mix-A which comply with Specification S-000-1320-1001V and Drawing D-000-13101Q01V 6.4 Formwork The contractor shall use Plywood or steel formwork All the formwork surface need coated by dedicated oil for avoid adhesive to concrete element surface 6.5 Joint Materials All joint materials shall be approved by JGCS and the Employer prior to use MANPOWER - Prior to working any work at site, all workers will be trained safety and of communication between people inside the Confined Space, the Confined Space Entry Attendant, the CONTRACTOR'S Security Personnel, and the Medical Centre must be established and operational Any entry outside site normal hours shall only occur with the express permission of the HSSE Manager who shall ensure medical and rescue services are maintained on site The Entry Attendant shall ensure that communication with personnel inside the Confined Space is maintained at all times to allow a summons for assistance 12.3 PERSONAL PROTECTIVE EQUIPMENT Any person working in a Confined Space shall wear an approved safety harness where there is a potential for the environment to become immediately dangerous to life and health A lifeline shall be attached and secured outside the Confined Space and a harness shall be the means to assist in the retrieval, unless a greater hazard is created If the internal atmosphere has the potential to become oxygen deficient or contains toxic gases, appropriate respiratory protection shall be worn Protective clothing is to be worn where hot works or irritating substances may be encountered If supplied air is being used by persons in a Confined Space sufficient supplied air resources are to be available on standby for any rescue 12.4 RESPIRATORY PROTECTION In circumstances where the nature of the work to be performed in the confined space will introduce new contaminants, such as welding, grinding, use of epoxy resins, solvents,liberation of contaminants from wall, etc additional precautions may be required in regards to PPE 12.5 ELECTRICAL EQUIPMENT INSIDE A CONFINED SPACE Electrical tools and equipment shall be connected thorough a Residual Current Device (RCD) located outside the Confined Space All leads that must enter vessels and tanks shall be routed through a protective nozzle or over a protective shroud to prevent damage to the outer sheath of cables Wherever possible, all leads, cables, hoses etc shall be routed through openings separate from Page 27 of 41 NGHI SON REFINERY AND PETROCHEMICAL PROJECT Title: METHOD STATEMENT OF LEAK TEST FOR MANHOLE Contractor Doc No VV3120-008-A-137 Rev.0 those used for entry by personnel, and maintained clear of any water All electrical equipment shall be shutdown when the Confined Space is not attended, such as crew breaks etc The cables shall be located, suspended or guarded to minimise accidental damage All welding equipment and leads shall be inspected for cracks, cuts and breaks In insulation Where there is any doubt of safe condition they shall be checked by an authorised licensed person and faults rectified before use 12.6 GAS CUTTING AND BURNING Compressed gas cylinders, other than those used as part of a breathing apparatus, shall not be taken into a confined space Gas cutting equipment shall be inspected and leak tested prior to the introduction of hand pieces and hoses into a Confined Space All gas cutting and burning equipment shall be shutdown at the cylinders and removed when the Confined Space is not attended or when not in use, such as crew breaks etc All gas cutting and burning equipment shall be fitted with flashback arrestors at both the hand piece and regulators for oxygen and fuel gas 12.7 TRAINING IN RELATION TO WORK IN A CONFINED SPACE All persons who are to undertake work associated with Confined Space are required to complete approved training prior to any involvement in this work All personnel conducting work in relation to confined spaces shall have received training in accordance with the National Training Accreditation Standards and the training scope be recognised against AS 2865 Safe Working in a Confined Space (certification validation period 12 months from date of issue) The CONTRACTOR will make available a Site Specific Confined Space training course on the Permit and Procedural requirements of the project, however, it remains the responsibility of the Subcontractor or employer in the case of the CONTRACTOR’S personnel to verify the competency of all persons who enter, supervise or control entry to Confined Spaces in accordance with AS 2865, and the requirements of this procedure 12.8 a Confined Space, regular visits by the Confined Space entry supervisor or other nominated and responsible persons must be made They shall inspect and ensure that the requirements of the Permit are being complied with and that site HSSE requirements are being maintained 12.9 RESCUE The CONTRACTOR and Subcontractor shall ensure that prior to entry into any Confined Space, an Emergency Plan is in place, known by all personnel associated with the Confined Space and that rescue platforms, equipment and personnel are in place and available at all times during entry The details of the Emergency Plan shall be determined by risk assessment based on the nature and complexity of the Confined Space and discussed with the CONTRACTOR’S Emergency Response Coordinator Where there is a disagreement on the requirements of the Emergency Plan, the CONTRACTOR'S Site HSSE Manager shall determine the appropriate requirements, and this decision shall be final Page 29 of 41 NGHI SON REFINERY AND
