- We, all company members including suppliers and subcontractors will their best to prevent occupational accident by implementing the Safety and Health Management We evaluate the risk from the work and establish safety measures before performing every work PHUONG THANH TRANCONSIN VRAMP – CONTRACT PACKAGE:RAP/CP15 – MAINTENANCE OF NH6 FROM KM193÷KM233 IN SON LA - - We minimize the unsafe factors through making comfortable work environment, managing human resources (decrepitude, health, physical and mental state) and habitual danger prediction activities We take emergency measures and provide emergency medical treatment in case of an occupational accident occurs Also we investigate causes and take appropriate measures to prevent the recurrence of similar or other accidents In addition, Phuong Thanh Tranconsin pursues improving environment-friendly performance in whole stages of the clean business development and the execution as a task of green management considering pan-global environment situation: - - We consider environment – friendliness through whole stage (Planning, design, procurement, construction, operations, and disposal) of business We establish and implement our own environmental control criteria, taking into account the characteristics of project as well as compliance with environment-related laws and regulations We concentrate on conservation of ecosystem, resource saving, environmental pollution prevention and minimization of the construction wastes We regularly operate training, monitoring and auditing system responsibility of environmental management at site III SAFETY MANAGEMENT ORGANIZATION Organization Chart Project Manager Safety Officer Site Manager Safety Staff All Worker PHUONG THANH TRANCONSIN VRAMP – CONTRACT PACKAGE:RAP/CP15 – MAINTENANCE OF NH6 FROM KM193÷KM233 IN SON LA Duty – General Authority 2.1 Project Manager : - - In charge of the overall accountability for construction site health, safety and environment (HSE) Be accountable for achieving HSE goal Be responsible for ensuring subcontractors implement, administer, plan, train, and promote the HSE plan Set up site HSE Plan and be responsible for their implementation before commencing site activity Use only competent personnel to work on site Issue, where necessary, in accordance with the policy on high-risk activities written instructions setting out method of and safe work Plan and maintain housekeeping to high standards Inform management of contractor, including owner’s representative immediately to enter count any accidents, incidents with the potential of injury and illness consequences Ensure all activities to site with the identified HSE problem Immediately remove from site any people (staff, subcontractors, or third parties) who are not willing to comply with the HSE requirements Provide equality of HSE training opportunity to all employees Perform corrective actions for any identified unsafe conditions Qualifications and experience 2.2 Minimum 10 years of experience in project management of construction of similar project An understanding of relevant government requirements, Project Quality Plan, procurement procedure, scheduling, etc… Strong understanding of basic construction principl Good communication both written and verbal, and presentation skills in English are a must Sound leadership and management capabilities Good interpersonal skills with strong analytical and a team player Strong subcontractor management and decision making skills Site Manager: - - Reporting to the Project Manager, the Site Manager has overall responsibility for HSE implementation and results at site, including: Being familiar with applicable health, safety and regulations and requirements of the Project’s HSE documents Ensuring that all site personnel, visitors, COMPANY personnel, and subcontractors conform to Project HSE requirements Reporting concerns regarding the implementation of the HSE Management System to the Project Manager/Project HSE Manager Ensuring effective communication with CLIENT regarding HSE program and issues Ensuring effective HSE coordination and cooperation with vendors and subcontractors PHUONG THANH TRANCONSIN VRAMP – CONTRACT PACKAGE:RAP/CP15 – MAINTENANCE OF NH6 FROM KM193÷KM233 IN SON LA - - Participating in audits, inspections and monitoring programs to measure the effectiveness of the HSE Management System and assure requirements are being effectively communicated throughout the work force Ensuring that adequate welfare arrangements are in place for site personnel, Project visitors including sanitation, office, messing facilities, first aid, housing, transport, etc… where these are not provided by others Attending and participating in all weekly progress meetings and monthly Project HSE meetings Qualifications and experience 2.3 At least years working experience in related position Having experience in similar projects Having Supervision Certificate is preferred Having project management certificate is preferred Being active, enthusiastic, responsible for work Good English communication skills Able to work independently and under pressure Safety Officer: - - Assist to Project Manager in overall management and execution of HSE plan Conduct regular site inspections and prepare reports to Project Manager for corrective actions Check and advice on work plan or procedures for HSE prior to work commencement Coordinate with owner HSE section regarding to HSE activities Liaise with subcontractors HSE Supervisor regarding to HSE to all workers undertaken by subcontractors Act at the capacity of secretary to HSE committee Provide advice, guidance for accident prevention Coordinate with government authorities, especially Ministry of Labor on matters of pertaining to legal HSE requirement Establish and maintain proper HSE administrative system Investigate all incidents and accidents and submit reports completely and present remedial actions to HSE team of head office through Project Manager for corrective action Monitor and issue of Risk Assessment Qualifications and experience Degree qualified in Engineering 5+ years experience in site safety management Maintain an intermediate knowledge of safety regulations and compliance requirements Intermediate skills in ensuring compliance including the confidence to strictly administer safety systems and close sites if necessary Intermediate attention to detail, particularly in respect to safety requirements of project and site contractors Intermediate written and verbal communication skills, with the ability to write clear reports Intermediate client and contractor management skills, including demonstrable negotiation skills and inter-personal skills PHUONG THANH TRANCONSIN VRAMP – CONTRACT PACKAGE:RAP/CP15 – MAINTENANCE OF NH6 FROM KM193÷KM233 IN SON LA Beside, Must be familiar with the commitments of the safety officer and in the event of the safety officer’s absence during an event be in a position to assume the responsibilities of the safety officer to the exclusion of all other duties In this case to have contingency plans in place to ensure all other deputy safety officer duties and procedures can go ahead with other staff - - - - Will be familiar with the principles of the General Safety Certificate, the Safety Policy, the Guide to Safety at HSP and assist the safety officer in ensuring that the provisions of these documents are complied with Will attend the pre-match and post match briefing/de-briefings with the safety officer Will be required to liaise effectively with HR and the safety officer and be heavily involved with all aspects of recruitment of stewarding roles During each event the deputy safety officer will assist in the safe and effective management of each event After each event the deputy safety officer will advise the safety officer of any lessons learnt which should be taken into account of in planning the next fixture The deputy safety officer will have a specific responsibility for monitoring the implementation of the Club Health and Safety Policy for employees during an event and bring to the attention of the safety officer any health and safety issues which occur The deputy safety officer shall keep the safety officer informed of how the Club Spectator Safety Policy is being implemented and make recommendations for amendments to the policy The deputy safety officer shall monitor the club grievance procedure policy and bring to the immediate attention of the safety officer any grievance or disciplinary issue affecting the event day staff Qualifications and experience Degree qualified in Engineering 5+ years experience in site safety management Maintain an intermediate knowledge of safety regulations and compliance requirements Intermediate skills in ensuring compliance including the confidence to strictly administer safety systems and close sites if necessary Intermediate attention to detail, particularly in respect to safety requirements of project and site contractors Intermediate written and verbal communication skills, with the ability to write clear reports Intermediate client and contractor management skills, including demonstrable negotiation skills and inter-personal skills 2.4 Safety Staff : The assigned Safety Supervisors, will report to the HSE Officer and have the following responsibilities: Review the results of daily inspections of the site HSE observation cards to identify safety issues and deficiencies and to advise Supervisors/HSE Officer of findings - Conduct HSE inspections and Audits of the work and facilities and document items of concern on HSE observation cards Participate in Job Hazard Analysis (JHA‟s) reviews PHUONG THANH TRANCONSIN VRAMP – CONTRACT PACKAGE:RAP/CP15 – MAINTENANCE OF NH6 FROM KM193÷KM233 IN SON LA - Liaise on HSE matters with construction supervision to ensure involvement in all site activities, including work method statements and risk assessments (JHAs) Investigate incidents in accordance with Project requirements Attend toolbox talks on a regular basis and ensure that they are being conducted in a professional and capable manner Carry out regular checks and issuance of permits to work Assist in controlling areas where critical tasks are being undertaken (e.g., pressure tests, heavy lifts) Set a personal example to all the site personnel and actively promote HSE on the Project Qualifications and experience Training: - HSE management certified HSE training certified First aid training certified Experiences: At least years HSE on site experiences 2.5 Worker : - IV Comply with HSE rules and regulations Work safely and shall not anything that can cause injury to him and the other Ensure work tools, equipment, materials, including Personal Protective Equipment (PPE) are used correctly and maintained in good serviceable conditions Reports any unusual occurrences and all defects of plant and equipment to your supervisor Attend all Tool Box Meetings, other meetings and training Observe all written and verbal HSE instructions issued from time to time by supervisor and HSE Supervisor Observe and obey all HSE notices Develop a personal concern Keep work place clean and tidy APPOINTMENT, AUTHORITY AND RESPONSIBILITY OF SAFETY STAFF - - - Safety Officer has been authorized with adequate power to take appropriate and direct measures to ensure the safety of the Site in case of emergency, prevent unsafe practices and unfavourable effect on the environment and correct the inappropriate and insufficient traffic control measures or behaviors against the safety plan and rules Safety Officer is responsible for reviewing whether the proposed procedures and practices in safety plans of Subcontractors are in accordance with Site Safety Plan and statutory regulations Safety Officer shall not be removed from the Site without the written consent of the Engineer Within fourteen (14) days of any such removal or notice of intent of removal, the Contractor shall nominate a replacement Safety Officer for the approval of the Engineer PHUONG THANH TRANCONSIN VRAMP – CONTRACT PACKAGE:RAP/CP15 – MAINTENANCE OF NH6 FROM KM193÷KM233 IN SON LA - V According to the requirements of the Contract provisions and Technical Specification, the Contractor will set strict criteria for the employment of Safety Officer and Safety Staff to satisfy the requirement of Project safety work First, Safety Officer must have a good knowledge on the laws and regulations etc of the Vietnamese government on the requirement of health and safety, or comprehend and have a good knowledge on the above laws and regulations by training; Second, every safety stuff shall be educated and trained on health and safety before undertaking any work to know the source of danger on site and the prevention measures; Finally, carry out work strictly in accordance with the approved Health and Safety Plan, warning and reporting problem if any HEATH& SAFETY CONTROL WORKFLOW AND COMMUNICATION AND REPORTING PROCEDURES - According to Organization Chart of the Project, Heath and Safety Control Workflow is shown as follows: Project Manager Safety Officer Site Manager Safety Staff Site Engineer Sub-Contractor or Worker PHUONG THANH TRANCONSIN VRAMP – CONTRACT PACKAGE:RAP/CP15 – MAINTENANCE OF NH6 FROM KM193÷KM233 IN SON LA Safety staff are authorized to implement solution if engineer is not available, including direct instruction to site employees to make safe the site or take urgent direct action for safety - Communication Procedures Interaction and communication between the construction personnel on site and safety assurance staff: According to the requirement of project, there is a billboard for every work item on which are telephone numbers of the related safety staff who can be connected in 24 hours of a day so that the construction personnel on site can contact with the safety staff at any time The telephone numbers of the Project Manager, the Safety Officer and Deputy Safety Officer who can be connected in 24 hours of a day shall be on the billboard to ensure the timely communication of safety matters A schedule of regular communications between the safety officer, safety staff, and site engineer shall be established Each site engineer will be readily accessible by the safety staff, and the safety staff shall be readily accessible by site engineer and workers Interphones shall be equipped for the vertical communication at height for implementing the main pylon and main beam of the main bridge to ensure the effective communication between construction personnel on site and safety staff - Reporting Procedures For the health and safety problem arising from site construction, Sub -contractors and their staff shall report timely to the site safety staff and Site Engineer if any If possible, the site safety staff and Site Engineer shall reach a solution, and the Site Engineers shall be responsible for implementing it immediately The site safety staff and Site Engineer shall respectively report to the Safety Officer and Site Manager provided that the site safety staff and Site Engineer can‟t solve the problem after communication If possible, the Safety Officer and Site Engineer shall arrive at a solution, and the Site Manager shall implement it immediately However, the Safety Officer is authorized independently to issue direct instructions to site employees and to take urgent action make safe the site and assure safety Furthermore, if the Safety Officer and Site Manager can‟t solve the problem either after communication, they can respectively report to the Project Manager After the Project Manager has had the solution, it shall be noticed and execute in reverse procedures The Contractor shall attach great importance to the safety construction of Subcontractors and low-tier subcontractors, knowing the means of communication between each Sub-contractor and ensuring all of them carry out construction in accordance with the regulations of the Safety Plan and all the other related laws PHUONG THANH TRANCONSIN VRAMP – CONTRACT PACKAGE:RAP/CP15 – MAINTENANCE OF NH6 FROM KM193÷KM233 IN SON LA VI PERSONAL PROTECTION EQUIPMENT (PPE) Introduction - - - - All international and domestic HSE law requires the use of PPE to reduce person’s exposure to hazards when engineering and/or administrative controls are not feasible or effective in reducing these exposures to acceptable levels If PPE is to be used to reduce the exposure of persons to hazards, a PPE program should be initialized and maintained The program should contain identification and evaluation of the hazards to determine the effectiveness of the PPE Other factors include “Selection-Issue-Training-Inspection-MaintenanceSupervision-Storage” The use of PPE must be NOT be considered as the “last resort”, it is THE FIRST STEP in protecting people Based on hazard evaluations (conducted by supervisors) employers shall select, and have each affected employee use PPE that will protect the employee from hazards Employers shall communicate PPE decisions to each affected employee and select PPE that properly fits each affected employee Employees shall use all PPE that may be required to maintain their exposure within acceptable limits The employer will make all reasonable efforts to accommodate employees with religious beliefs that may conflict with the PPE requirements contained within this plan However when reasonable efforts to accommodate the employees religious beliefs not provide the necessary safe working environment (without PPE), then the employer shall require the employees to use the appropriate PPE or the employee will not be allowed to work in the area where he/she will be exposed to the hazard requiring protection Personal Protection Introduction Responsibility The Contractor will take responsibility to assure that: - - - The Contractor/Subcontractor shall provide the standard PPE (e.g Safety Helmet, Safety Glasses, Safety Footwear, safety harnesses, fire extinguisher, stakerope, vibro-shock, first-aid equipment, rescue equipment as well as the additional requirements highlighted in the risk assessment) for their visitor and employees The Contractor/Subcontractor shall provide the construction personnel on site who work at height and installation work with safety equipment, scaffold, hand-rail, work platform, crane, staircase, and other traffic equipment, hoisting equipment, lighting, signal device and protection equipment Furthermore, all such equipment shall be tested, inspected, and maintained regularly by authorized staff to ensure them maintain a good working condition When carrying out work in water, near the water and on the water surface, the Contractor shall submit special safety protection measures before carrying out each work item, including but not limited to rescue boats, safety net, warning signs, warning lights, navigation lights, as well as search and rescue procedure, rescue PHUONG THANH TRANCONSIN 10
