Responsibilities

The Contractor sets up a Project Management Unit to oversee the project, which includes an executive board, an assisting department, and direct construction teams responsible for executing specific construction tasks.

Relationship between the Board of Management and the site management department

The Company's Board of Directors oversees the departments and the Site Management Unit, ensuring that each department is assigned tasks aligned with their expertise They provide direct guidance and supervision to the site manager to ensure effective contract execution, adherence to appropriate construction methods and regulations, quality control, technical management, and safety measures Additionally, they are responsible for the supply of materials and human resources.

The site management department, led by the Site Manager, is responsible for providing regular reports to the Board of Directors regarding the construction status and management aspects of the site The Site Manager must coordinate with relevant departments to ensure that all requirements are met and that construction proceeds in accordance with the design documents, maintaining the project's technical quality Additionally, the Site Manager is tasked with addressing issues in each construction area promptly and effectively.

A site engineer plays a crucial role in managing human resources, techniques, quality, and project progress They conduct daily checks and monitor the construction process to ensure adherence to the design specifications Additionally, site engineers coordinate effectively with the owner, design consultants, and the owner's engineer, as well as other relevant parties, to address any issues that arise on the construction site.

In the absence of the site manager, fulfill all assigned tasks and responsibilities to effectively oversee construction site operations on their behalf.

Department of Progress, Technical and Quality Management

- The head of this department is the construction technical manager - responsible to the Site Management Unit for the technique, progress and quality of the work

The technical department oversees project progress and construction techniques, ensuring quality in every job They assist the site manager and on-site technicians while maintaining communication with Party A and engineers Their responsibilities include consulting on construction technical solutions, conducting acceptance work, creating as-built documents, and maintaining a daily monitoring diary.

Safety, security and environment management department

The safety department is responsible for gathering, researching, and disseminating legal documents related to labor protection across teams Additionally, it organizes training courses on labor protection for employees at the construction site, ensuring compliance and safety standards are upheld This department is managed by a dedicated full-time officer alongside part-time site technicians.

Officers play a crucial role in maintaining safety, security, and environmental standards on construction sites They assist the Site Manager and technicians, ensuring their constant presence on-site Their primary responsibilities include implementing measures to protect both personnel and equipment while fostering a positive working environment.

The medical department is dedicated to ensuring the health and safety of staff and construction workers by providing care for minor illnesses and dispensing necessary medications In the event of an accident, they perform first aid and assess whether the patient needs to be transported to a hospital for further treatment Additionally, the department regularly reports to the site Management Unit and the company's board of directors regarding the status of occupational accidents occurring on the construction site.

The department tasked with overseeing and managing personnel is essential for ensuring timely worker payments and allocating funds for materials and equipment This department plays a critical role in coordinating with construction teams and equipment operations, fostering effective collaboration for project success.

The technical management department plays a crucial role in overseeing the supply of materials and equipment, which significantly impacts product quality and construction progress.

- Follow the direction of the technical management department for each work area

- Includes teams: Construction motorbike, formwork + scaffolding + reinforcement + concrete, masonry + finishing, mechanical group, aluminum and glass, plaster + paint, electricity and water team, construction team

5 INSTALLATION OF EXTRA LOW VOLTAGE (ELV) SYSTEM

ELV underground conduit installation

- Describe construction works relate with installating of ELV underground conduit

Works are execute base on construction schedule and detailed approval drawing a Material

- ELV underground conduit must be ordered according to the approved type and manufacturer b Preparation

- Prepare the excavator to excavate ditches to lay conduit

- The work shall be start when installation schedule allowed

- Check ELV underground conduit location follow to approved shop drawing are ready for installation

- Check the route to transport the ELV underground conduit to the installation location

- Determine actual, correct size, type of conduit as per location need to install follow to approval drawing

- The Safety Officer will check the area to ensure no potential incidents before install c Installation work

- Mark the route of underground conduit the ground before excavate

- Re-check collisions with other systems

- Use the excavator to excavate underground duct until the required depth, wide and length

- Check the depth of trench for proper elevation

- Clean to ensure the trench is free from debris and foreign materials

- To execute install ELV underground conduit

- Invite main contractor and consultant to inspect

- Back-fill until the depth 300mm below finish grade then install warning tapes

- After inspect with consultant or take verification piecture, back-fill until finish grade

- Use conduit cap and ELV tape to seal the exposed terminals on the ground to avoid rocks and foreign objects falling into the conduit

- The work shall be start when civil works of Rack panels location have completed, clean, handed over and approval of installation schedule

- Check Rack panels location follow to approved shop drawing are ready for installation

- Visually inspect the device for warping, dents, scratches, etc Read the installation instructions of the equipment supplier

- The Safety Officer will check the area to ensure no potential incidents before lifting c Installation work

- Determine actual, correct type of rack panel as per position need to install follow to approval drawing

- Before construction, it is required to clean the construction site area

- For Rack panels on the floor, use means such as rollers, cranes, and lifts etc

- Making of position for Rack panels on foundation or wall

- Secure panel base to the floor/wall using anchor bolts

- Check the leveling before tighten bolt

- After installation, fix the electrical cabinet, proceed to connect the ground to the cabinet

- Determine Install modulies and equipment into rack panel

To ensure optimal safety and functionality, regularly clean the electrical cabinet using a vacuum cleaner Inspect all connections for secure bolts, verify the insulation on colored cables, and check the markings on the cables Additionally, connect the ground wire from the cable tray ladder to the electrical cabinet cover and seal any openings for both input and output cables.

ELV cable tray and trunking installation

- The ELV cable tray and trunking will be standard manufacturer’s galvanized structural steel & with all standard accessories

- As per approved catalogue manufacture

- Prepare angle bars or c-channel or unistrut as supporting

- Prepair anchor bolt, thread rod, nut M10, M12 b Preparation

- The work shall be start when civil works of Cable tray and trunking location have completed, clean, handed over and approval of installation schedule

- Check Cable tray and trunking location follow to approved shop drawing are ready for installation

- Check the route to transport the Cable tray and trunking to the installation location

- Determine actual correct size & type of ladder, tray as per location need to install follow to approval drawing

- Determine actual, correct & type of support(angle bars, c-channel,unistrut, thread rod) as per type of ladder, tray and installation location

- The Safety Officer will check the area to ensure no potential incidents before lifting c Installation work

- Making of position for Cable tray and trunking support on foundation or wall or ceiling

- Use a laser machine to mark the cable ladder, tray according to the approved drawing

- Re-check collisions with other systems

- Drilling hole & setting of anchor

- Setting thread rod and tghiten by hand tool (at ceiling location)

- Install the support bars for the cable tray/trunking with distance 1,2m – 1,5m

- Tighten bolt to fix support after verify correctly elevation of support

- Move each tray/trunking to the support

- Secure ELV tray/trunking by bolts and nuts

- Fix 2 adjacent ELV tray/trunking by connection of cable tray/trunking and bolt, nut

- Between 2 cable trays connected by grounded braided copper wire

- Aligning cable ladder/tray route correctly in elevation, vertical, horizontal

- Determine actual, correct & type of conduit as per location need to install follow to approval drawing

- The Safety Officer will check the area to ensure no potential incidents before lifting c Installation work:

* PVC conduit embedded in concrete floor

- Install PVC conduit exaclly of switch, Joint box in the wall/ floor follow shop drawing approved

- Use painting or pen colour marking location Switch, Junction box…

- After finish layer 1 rebar, and then install conduit exaclly area Fix conduit strong on rebar

- After install conduit finish we wiring iron fix all conduit Between 2 wiring fix conduit under 800mm- 1000mm

- Install conduit must follow area equipment and structure on floor

To optimize sound distribution, position two sound boxes 10 to 15 meters apart and install an additional intermediate sound box Ensure that the pipes connecting the sound boxes are joined with appropriate joints and secured using plastic pipe glue Additionally, reinforce the setup by firmly anchoring it to the floor with iron supports.

Pipelines with two or more parallel pipes must maintain a minimum distance of 20mm between the inner edges of adjacent pipes to facilitate proper concrete insertion, excluding the areas where the pipes connect to an intermediate box.

To ensure optimal functionality, it is crucial that the pipes are securely reinforced on the cabinet and the equipment ends Additionally, there must be a minimum distance of 20mm maintained between the inner edges of adjacent pipes.

When conducting a deployment cut for a wall, it is essential that the cut is precise and compatible with the pipe installation The width of the wall cut should exceed the pipe diameter by at least 20 mm, while the depth of the cut should be approximately 5 to 10 mm greater than the pipe size.

- Marker the position of the switch and box in brick as approved drawings

- After finishing layer of bricks, the contractor will pipe marking on brick wall with 2 lines marked as pipe size, distance & number installed

- Use cutting tool or specialized cutters to cut brick wall exactly with 2-way marking and around box or cabinet

- Installation PVC conduit with sound box & room cabinet by threaded connection as approved drawing

- The conceal box must twist screw and filled with foam in box and taped to prevent sticking motar on the box

- Location as buit of pipe must be saved before to plastering

- The pipe must be fix to steel to prevent movement during plastering

- Use specialized stuccos and ciments wall to phaster where installed PVC conduits

* PVC conduit floating on ceilings, walls:

Conduit size Maximum distance between 2 saddle ỉ20 ≤1.0 m ỉ 25 ≤1.2 m ỉ 32 ≤1.5 m

Maximum distance from connection box to saddles : 0.3 m

• Check position, route of ELV conduit follow to approval shop drawing

• Check the physical condition of the conduit after installation (no distortion, damage )

• Check the inside of the pipe: there are no obstructions (bricks, mortar, etc.) to avoid blockages during cable pulling

ELV cabling

Describe construction works relate with ELV cabling

Works are execute base on construction schedule and detailed approval drawing a Material

EVL system cable include : Lan cable cat 6, audio cable 18AMG, lan cable cat 5, fiber optic cable, etc

- Make storage for gathering construction materials

- Gather materials to site or necessary location

- Request the representatives of main contractor and owner to inspect input materials b Preparation

- Prepare specific tools for pulling and laying cables: cable drum rack, rotation shaft, roller, bait wire, cable winch machine (if necessary), cordless crimper, etc

- The work shall be start when cable ladder, tray, underground conduit, conduit have completed and cleaned base on installation schedule

- Check cable route location follow to approved shop drawing are ready for pulling, laying

- Check the route to transport the cable to pulling, laying location

- Based on detailed schedule, approvalled cable route shop drawing, cable schedule to determine actual, correct type of cable as per location need to pull, lay

- The Safety Officer will check the area to ensure no potential incidents before lifting

- Deploy personnel depending on the details progress of construction:

 Each team of 20 to 25 people

 Each crew of 2 to 3 people

- Note: combine drawings of HVAC system, water system, electric system, fire fighting system with each other so that the installation will not be overlapped c Cable pulling, laying work:

* Pulling cable on tray & trunking

- Before putting up wide cable conduct discharge each cable wires in cable drum, using specialized cable discharge arm with large cross section

- Conduct scattered each cable wire by circulating method, prohibits use drag-end scratch and stretch cables

- When all of the cable in a process have finished, conduct rearrange and positioning them in ladder by nylon cable ties, ensure that the parallel cables and no cross,confusion

To install cable across the floor, an electric winch is mounted on the roof of the technical box to facilitate vertical hooking This process is initiated after securing the cable into the pre-installed cable ladder.

To ensure labor safety, it is essential to apply the technical box for cleaning the working platform on all floors To facilitate synchronized operations among the upper and lower levels and the electric winch operator, contractors will provide radios for effective communication during this process.

The Project Manager is responsible for establishing the project’s Health, Safety, and Environment (HSE) organization This includes appointing a registered Health and Safety (H&S) Manager who will have clearly defined roles, responsibilities, and authority The H&S Manager ensures the effective establishment, implementation, and maintenance of the HSE Management System in compliance with applicable regulations and standards.

The Project Manager provides an organizational chart that outlines the site management team responsible for executing the Works This project management team is adequately sized and possesses the necessary experience and competence to effectively implement the Health and Safety management system.

The Project Manager has the ability to delegate specific tasks or functions to a member of their management team, granting them the necessary authority to fulfill these responsibilities However, it is important to note that the ultimate accountability for the project still lies with the Project Manager.

The Project Manager is responsible for establishing effective communication channels to inform employees about Health, Safety, and Environmental (HSE) matters Additionally, he will ensure that all staff members receive the necessary training relevant to their specific roles.

The Project Manager will spearhead the development and promotion of Health, Safety, and Environment (HSE) initiatives, utilizing the management system to set necessary standards He will consistently monitor the workforce's performance to ensure that the project meets its HSE targets.

The Project Manager plays a crucial role in upholding the Health, Safety, and Environmental (HSE) commitment of the project management team This responsibility entails a comprehensive understanding of the key objectives and requirements outlined in the HSE policy, standards, and procedures, particularly when assigning responsibilities across various levels of supervision.

- Specifically, the Project Manager shall:

- Assure that approved HSE procedures are adopted for defining methods and systems to be applied

- Assure that all relevant laws, regulations and other requirements in matters of HSE are respected

- Maintain HSE relations with Owner, Sub-contractors, Vendors and with Governmental Authorities and organizations for all items regarding HSE

- Overall charge of the Project HSE organization, including medical care and first aid, HSE Training, etc

6.2.2 Responsibilities of the HSE Manager

The HSE Manager will report directly to the Project Manager regarding all health, safety, and environmental (HSE) activities on site This role involves supporting the Project Manager in health and safety matters by promoting HSE programs and ensuring compliance with all HSE requirements.

- Set up an adequate and strong HSE management system

- Ensure the development and implementation at site on activities described in this plan and other procedures

- Organize site so that work is carried out to the required standard with minimum risk to men, equipment, and materials

- Provides H&S consultancy to project management team

- Report any HSE issues/incident/accident to VAPCO

- Lead and ensure HSE incidents and non-compliances are investigated and corrected;

- Set up an site emergency response plans and procedure

- Leads the development and maintenance of Site Emergency Response Plans and

- Participates in the development of Site Security Plans and Procedures

The development of a robust Health, Safety, and Environment (HSE) culture is essential for achieving outstanding HSE performance This involves creating comprehensive coaching and training plans, overseeing the preparation of relevant training materials, and ensuring effective delivery of these programs throughout the project.

- Leads the investigation of accidents and incidents

 Conduct and chair the site HSE Monthly Committee Meetings;

- Conduct and chair the weekly, monthly site HSE tours

- Update HSE plans and procedures

- Review and approve subcontractors’ HSE plans/procedures/permit to work

- Review Risk Assessment & JSEA’s when required

- Conduct and chair incident response drills

- Ensure construction methods minimize risk to public & workers e.g through adequate Traffic Management

All equipment delivered to the site must be safe, fully operational, and equipped with appropriate safety devices It is essential that all plant machinery is tested in compliance with HSE Management Plans, and that evidence of all necessary certifications is maintained on-site.

6.2.3 Responsibilities of H&S Officers (Engineers & staffs)

- The H&S officers shall assist the H&S Manager by ensuring that the provisions of the H&S plan and other H&S related requirements are correctly complied with by HCA

CONTRACTOR personnel and Sub-contractors in each working area

- They ensure direct supervision and coordination of the Sub-contractors’ activities and day to day contacts with them to deal with any H&S matters

6.2.4 The H&S officers’ general responsibilities include, but not limited to:

- Verifying that all workforce under HCA CONTRACTOR’s direct or indirect responsibility receive appropriate H&S information, induction and training, including supervision and control on the H&S training program by Sub-contractors

- Advising the H&S Manager of any necessary required actions to improve working and living conditions in the field under their individual responsibility

- The H&S officers shall attend tools box meetings

- The H&S officers shall inspect unsafe conditions and acts

- Organizing, managing and recording of meetings with sub-contractors as per the H&S relevant meetings program

- Ensuring adequate levels of working and living conditions by means of regular inspections on housekeeping, hygiene, potable water system, kitchens and canteens, sewer system and waste disposal etc

- Leading accident, incident investigations and attending any major accident investigation in areas under their responsibility and reporting on them to the H&S Manager as per the Accident Reporting Procedure

- Organizing and managing the medical and first aid facilities in areas under their control

- Daily check and regular inspection of PPE

- The H&S officers shall report to the H&S Manager on H&S matters

6.2.5 Responsibilities of Construction Manager, Section Managers and Supervisors

It is essential to ensure that all operatives and laborers receive appropriate training for their specific tasks, particularly for new employees and subcontractor personnel, to promote the adoption of proper health and safety precautions.

- Checking that all plant and equipment, including power and hand tools are maintained in a good condition and remove from use any defective items

- Maintaining organized, tidy and safe working areas

- Maintaining all welfare facilities in a clean and safe condition

- Ensuring that all operatives/labor are provided with and use approved personal protective equipment

- Ensuring that instructions are given in the use of any potentially hazardous materials

- Ensuring that other persons including Owner representatives, other contractors and members of the public are protected from the work activities of HCA CONTRACTOR and its subcontractors

It is essential to discourage horseplay in the workplace and to take decisive action against individuals who repeatedly neglect their own health and safety, as well as that of their colleagues and those nearby Prioritizing a safe working environment is crucial for everyone's well-being.

6.2.6 Responsibilities of Subcontractors and Vendors Personnel

Every Sub-contractors / Vendors Personnel working on the Project Site shall:

- Comply with all H&S rules and other requirements

- Adhere to this H&S plan and comply with the H&S requirements and procedures These are to be observed as minimum requirements

To ensure compliance with health and safety regulations, the site organization must include a Health and Safety (H&S) supervisor who directly reports to the subcontractor's site manager This supervisor will assist in overseeing that all activities within the subcontractor's or vendor's scope of work adhere to project-specific H&S requirements, the established H&S plan, and safe working procedures.

To maintain effective health and safety (H&S) standards, organizations must ensure that an appropriate number of H&S supervisors are present on-site, relative to the total workforce of subcontractors These supervisors are essential for supporting the H&S officer and ensuring proper enforcement of H&S procedures throughout the execution of the work.

- Ensure that all of their own personnel are aware of H&S requirements arising from laws, regulations and the contract and observe them by planning appropriate training of their personnel

To ensure the safety of their workforce, it is essential to identify and analyze all potential job hazards, implement necessary preventative and protective measures, and provide personal protective equipment (PPE) as required for the tasks at hand.

General

HCA CONTRACTOR will ensure the availability of medical facilities on-site to provide immediate assistance to workers in case of incidents Trained first-aid personnel, along with appropriate first-aid equipment and facilities, will be present at both the Site and camp A doctor and a nurse will be assigned to these locations, with their CVs submitted to Vapco for approval Comprehensive medical resources will be provided to ensure worker safety and health.

The contractor will adhere to local standards for medical support services and is considering entering into a medical service contract with Ky Anh General Hospital Evidence of this agreement will be submitted to Vapco by the specified deadline.

Each work site will ensure effective communication with the Occupational Medical Service, whether it be a hospital or clinic A comprehensive list detailing the nearest hospitals, along with their locations and contact numbers, will be prominently displayed in all Site Offices, including those of subcontractors and camp offices.

- Prior to the start of work, arrangements will be made for medical facilities and medical personnel to be available at the construction Site and the camp

- Effective communication and transportation to care for injured workers will be provided

- Where professional medical services are available, each work Site will set up terms and conditions for use and maintain close communication with them

- A list of the available hospitals including locations and their phone numbers will be displayed at all Site and camp offices including Subcontractors’

Medical Facility

The medical facility will provide:

- A waiting / reception area, consultation / examination area, treatment area, sick bay / observation area, sanitary facilities, secure storage for drugs and medical equipment and confidential storage of medical records;

A thorough inventory of essential medical equipment and supplies is crucial for delivering first aid, immediate treatment, and stabilization of significant illnesses and injuries that may occur during work activities This inventory should at least comprise a bed, stretcher, chair, table, heating and lighting facilities, a sink with a water supply, cabinets for storage, and first aid supplies, including a portable defibrillator.

- A detailed inventory of medicines and drugs shall be kept and checked regularly

Medical Staff

The Contractor shall appoint at least one nurse with a college degree or higher

- Notify the HSE Manager of any trends with illness or injuries, especially for infectious disease

- Periodically check hygiene conditions of the canteens/restaurants, including all food

- Provide all possible medical assistance and care to improve the condition of persons suffering injury or illness

- Be available to attend accidents/incidents on Site to give medical treatment as required

- Carrying out training programs for construction personnel (First aid training, etc.)

- Dispense pharmaceutical items as prescribed by the doctor and maintain records of prescription drugs

- Keep stock records of pharmaceutical/clinic items, and order replacement items to maintain minimum stock levels

- Periodically check the hygiene conditions of all camps

- Keep accurate records of all medical matters and ensure they are kept strictly ‘Medical in Confidence’ and restrict access to Authorized Persons only
