Tổ chức phải thiết lập, thực hiện và duy trì một

(hoặc các) thủ tục để điều tra và phân tích các sự cố nhằm: sự cố nhằm:

a) xác định các thiếu sót cơ bản về ATSKNN và các nhân tố có thể gây ra hoặc góp phần gây và các nhân tố có thể gây ra hoặc góp phần gây ra các sự cố;

b) nhận diện nhu cầu đối với các hành động khắc phục; khắc phục;

c) nhận diện được các cơ hội đối với các hành

động phòng ngừa;

d) nhận diện các cơ hội cho việc cải tiến liên tục;

e) truyền đạt về kết quả của việc điều tra. Việc điều tra phải được thực hiện kịp thời. Việc điều tra phải được thực hiện kịp thời. Bất kỳ nhu cầu nào được nhận diện là cần có

hành động khắc phục hay cơ hội cho các hành động phòng ngừa phải được tiến hành hòan tắt phù hợp với 4.8.3.2

4.5.2.2 The organizaftion shall evaluate

compliance with other requirements to which it subscribes (see 4.3.2). The organization may

wish to combine this evaluation with the evaluation of legal compliance referred to in

4.5.2.1 or to establish a separate procedure(S).

The organization shall keep records of the

results of the periodic evaluations.

NOTE The ƒrequency oƒperiodic evaluation may vary ƒor difering other equiremenfs to which the organizafion subscribes.

4.5.3 Incidenf invesfigation, nonconformify, correcfive acfion and prevenfive acfion

4.5.3.1 Incident investigation

The organizafion shall establish, implement and

maintain a procedure(s) to record, investigate

and analyse incidenfs in order to:

a) determine underlying OH&S deficiencies and other factors that might be causing or

contributing to the occurrence of incidents; b) identify the need for corrective action;

c) identifý opportunifies for preventive action;

d identfý opportuniiles for continual

improvement;

e) communicafe the resuls of such invesfigations.

The investigations shall be performed in a timely

manner.

Any identified need for corrective action or (adsbygoogle = window.adsbygoogle || []).push({});

opportunities for preventive

action shall be dealt with in accordance with the

relevant parts of 4.5.3.2.
