Do là nghiên cứu hồi cứu nên nghiên cứu này chúng tôi có những hạn chế nhất định:
- Số lượng mẫu trong nghiên cứu còn nhỏ
- Qui trình chẩn đoán, biểu mẫu tường trình phẫu thuật, giải phẫu bệnh chưa được thống nhất.
KẾT LUẬN
Qua nghiên cứu 47 trường hợp ung thư bóng Vater được phẫu thuật cắt khối tá tụy kèm nạo hạch triệt để, từ tháng 3 năm 2012 đến tháng 4 năm 2015 tại khoa Ngoại Gan – Mật – Tụy bệnh viện Chợ Rẫy, chúng tôi có một số kết luận như sau:
1. Nhóm hạch bạch huyết di căn, đặc điểm giải phẫu bệnh và giai đoạn của ung thư bóng Vater:
- Ung thư bóng Vater đa phần có kích thước > 2cm, dạng sùi chiếm ưu thế, mô học của toàn bộ các trường hợp là ung thư tuyến, hầu hết ung thư có độ biệt hoá trung bình – cao.
- Đa phần ung thư bóng Vater ở mức T2 và T3, không ghi nhận khối u Tis hay T4. - Trong ung thư bóng Vater tỉ lệ và tỉ suất di căn hạch tương ứng là 40,4% và 0,11. Hạch bạch huyết bị di căn chủ yếu ở các nhóm 13, nhóm 14 và nhóm 17, không di căn nhóm 6, 12p và 16b1. T1 không di căn hạch, T2 chỉ di căn nhóm hạch số 13 và 14, T3 di căn chủ yếu nhóm 13, nhóm 17 và nhóm 14.
- Đa phần ung thư bóng Vater ở giai đoạn I và II theo phân loại của AJCC 2012.
2. Các yếu tố liên quan đến di căn hạch bạch huyết trong ung thư bóng Vater: nồng độ CA 19 – 9 huyết thanh > 37 UI/mL, kích thước u, dạng u, độ xâm lấn của u và độ biệt hoá tế bào ung thư
- Các yếu tố: CA 19 – 9 huyết thanh > 37 IU/mL, kích thước u, độ xâm lấn của u, độ biệt hoá tế bào ung thư đều không liên quan di căn hạch bạch huyết.
- Dạng đại thể của u bóng Vater có liên quan đến tỉ lệ di căn hạch bạch huyết, tỉ suất di căn hạch cũng như số lượng hạch di căn.
+ Ung thư bóng Vater dạng loét có nguy cơ di căn hạch gấp 9,8 lần ung thư dạng sùi.
+ Ung thư bóng Vater dạng hỗn hợp có nguy cơ di căn hạch gấp 6,1 lần ung thư dạng sùi.
