Luận án tiến sĩ nghiên cứu sự biến đổi nồng độ hs CRP, hs troponin t, NT probnp huyết thanh và chức năng

1 ĐẶT VẤN ĐỀ Đau thắt ngực không ổn định gây nứt vỡ mảng vữa xơ động mạch vành, làm giảm đáng kể đột ngột dòng máu lòng động mạch vành, hậu cân cung cầu Oxy tim gây triệu chứng lâm sàng [1] Theo thống kê Anh năm từ 2006 đến 2007 có 69.971 bệnh nhân đau thắt ngực khơng ổn định nhập viện khoa cấp cứu, có 42.526 bệnh nhân nam giới 60 tuổi Tại Hoa Kỳ, đau thắt ngực không ổn định nhồi máu tim không ST chênh lên nguyên nhân tim mạch gây tử vong hàng đầu bệnh nhân 65 tuổi [1] Nhờ tiến điều trị nội khoa can thiệp tái tưới máu tỷ lệ tử vong giảm đi, nhiên tỷ lệ suy tim đau thắt ngực không ổn định lại tăng lên Đau thắt ngực không ổn định ổn định mảng vữa xơ động mạch vành, làm giảm lưu lượng mạch vành gây dãn thất trái với tăng áp lực đổ đầy thất trái, dãn nhĩ trái, phì đại thất trái, đồng tái cấu trúc điện học, hậu cuối dẫn đến suy tim [2] Nghiên cứu Sumeet D cộng năm 2019 266 bệnh nhân đau thắt ngực không ổn định cho kết có 21,1 % trường hợp có triệu chứng suy tim lâm sàng mà chức tâm thu thất trái giới hạn bình thường hay giảm mức độ vừa phải, trường hợp suy chức tâm trương thất trái [3] Nghiên cứu Chaowalit N cộng năm 2011 50 bệnh nhân đau thắt ngực không ổn định cho thấy rối loạn chức tâm trương thất trái xảy sớm rõ nét rối loạn chức tâm thu thất trái [4] Việc đánh giá chức tâm trương thất trái sớm có ý nghĩa dự báo biến cố tim mạch, tử vong giúp lựa chọn chiến lược điều trị đau thắt ngực không ổn định [5] Trong năm gần đây, dấu ấn điểm sinh học tim biết đến cơng cụ giúp sàng lọc, chẩn đốn, phân tầng nguy tiên lượng bệnh lý động mạch vành cấp mạn tính [6] Các dấu ấn sinh học tim sử dụng nhiều lâm sàng Hs – CRP đánh giá tiến trình viêm vữa xơ động mạch, HS – Troponin T đánh giá hoại tử tim NT – proBNP đánh giá suy giảm chức tim Nghiên cứu Klingenberg R cộng năm 2018 81 bệnh nhân đau thắt ngực không ổn định cho thấy nồng độ Hs – CRP, Hs – Troponin T NT – proBNP giúp cải thiện phân tầng nguy cơ, tiên lượng tử vong tốt kết hợp với thang điểm GRACE [7] Nghiên cứu Troughton R.W cộng năm 2009 cho thấy nồng độ NT – proBNP có tương quan với chức tâm trương mà không phụ thuộc vào tuổi, giới Mức NT – proBNP 140 pg/ml có giá trị chẩn đốn âm tính cao 90 % với suy chức tâm trương thất trái [8] Nghiên cứu Grufman H cộng năm 2019 524 bệnh nhân đau thắt ngực không ổn định cho kết nồng độ Hs – Troponin T tăng cao nhóm có nguy cao, có liên quan đến tình trạng tái cấu trúc thất trái suy chức tâm trương thất trái sau năm theo dõi dọc [9] Trên giới có số nghiên cứu mối liên quan qua lại nồng độ Hs – CRP, Hs – Troponin T, NT – proBNP chức tâm trương thất trái Đặc biệt nhóm bệnh nhân đau thắt ngực khơng ổn định riêng rẽ nghiên cứu với cỡ mẫu từ 40 đến 60 bệnh nhân cho nhiều kết khác nhau, chưa làm sáng tỏ mối liên quan qua lại dấu ấn sinh học tim với chức tâm trương thất trái Vì vậy, chúng tơi tiến hành đề tài “Nghiên cứu biến đổi nồng độ Hs – CRP, Hs – Troponin T, NT – proBNP huyết chức tâm trương thất trái bệnh nhân đau thắt ngực không ổn định” với hai mục tiêu: – Khảo sát đặc điểm lâm sàng, cận lâm sàng, biến đổi nồng độ Hs – CRP, Hs – Troponin T, NT – proBNP chức tâm trương thất trái bệnh nhân đau thắt ngực không ổn định có EF ≥ 50% – Xác định mối liên quan nồng độ Hs – CRP, Hs – Troponin T, NT – proBNP, chức tâm trương thất trái với số đặc điểm lâm sàng, cận lâm sàng bệnh nhân đau thắt ngực không ổn định CHƯƠNG TỔNG QUAN 1.1 Tổng quan bệnh đau thắt ngực không ổn định Thuật ngữ hội chứng vành cấp dùng để nhóm triệu chứng thiếu máu cấp tính tim Trong hội chứng vành cấp chia nhóm: Nhóm có ST chênh lên biểu nhồi máu tim có ST chênh lên nhóm khơng ST chênh lên bao gồm nhồi máu tim không ST chênh lên đau thắt ngực không ổn định Đau thắt ngực không ổn định nhồi máu tim không ST chênh lên xếp chung vào nhóm có chế bệnh sinh thái độ xử trí [10] Đau thắt ngực không ổn định thường gây ổn định mảng vữa xơ động mạch vành, làm tăng nguy tử vong và/hoặc dẫn tới nhồi máu tim có ST chênh lên [11] Nghiên cứu Sakaguchi M cộng năm 2016 mối liên quan mảng vữa xơ ổn định với đặc điểm lâm sàng, kết cho thấy tỷ lệ ổn định màng vữa xơ huyết khối tăng lên nhóm bệnh nhân bệnh nhân đau thắt ngực không ổn định [12] Nghiên cứu Seifarth H cộng năm 2014 368 bệnh nhân hội chứng mạch vành cấp theo dõi 24 tháng cho thấy có liên quan nồng độ Hs – CRP Hs – Troponin T với tiến triển mảng vữa xơ động mạch vành [11] Đau thắt ngực không ổn định nhồi máu tim không ST chênh lên khơng có khác biệt lâm sàng điện tim đồ Khác biệt chủ yếu mức độ thiếu máu tim vùng xa giải phóng dấu ấn sinh học hoại tử tế bào tim mà định lượng [13] Do đó, bệnh nhân có nhồi máu tim cấp khơng ST chênh lên coi trải qua đau thắt ngực khơng ổn định, có tăng dấu ấn sinh học hoại tử tim, kết hợp với biến đổi điện tim đồ thường bền vững so với đau thắt ngực không ổn định [6] Nghiên cứu Vogiatzis I cộng năm 2016 nồng độ Troponin T NT – proBNP 193 bệnh nhân nhồi máu tim không ST chênh lên so với 30 bệnh nhân đau thắt ngực không ổn định Kết luận nồng độ Troponin T NT – proBNP nhóm bệnh nhân đau thắt ngực khơng ổn định thấp có ý nghĩa so với nhóm bệnh nhân nhồi máu tim khơng ST chênh lên [14] 1.1.1 Sinh lý bệnh đau thắt ngực khơng ổn định Sự hình thành cục máu đơng mảng vữa xơ bị vỡ làm lộ lớp nội mạc, tiếp xúc với tiểu cầu, hoạt hoá thụ thể GP IIb/IIIa bề mặt tiểu cầu làm khởi phát trình ngưng kết tiểu cầu [15] Hậu trình làm giảm lưu lượng máu tới vùng tim mà động mạch vành chi phối biểu lâm sàng đau thắt ngực khơng ổn định, điện tim đồ hình ảnh thiếu máu tim cấp với đoạn ST chênh xuống T âm nhọn, dấu ấn sinh học tim loại Troponin tăng nhẹ có thiếu máu tim nhiều gây J.K cộng (2004) [16] Đau thắt ngực không ổn định gây nứt vỡ mảng vữa xơ làm giảm lưu lượng động mạch vành gây triệu chứng lâm sàng cân cung cầu Oxy tim Nghiên cứu Tian X cộng năm 2019 284 bệnh nhân đau thắt ngực không ổn định cho kết phân xuất dự trữ lưu lượng vành (FFR) giảm 0,8 chiếm tỷ lệ 73,12 % nhóm bệnh nhân có hẹp động mạch vành nặng 75 % [17] Cơ chế bệnh sinh đau thắt ngực khơng ổn định bao gồm năm nhóm chế bệnh sinh [2]: - Sự nứt mảng vữa xơ làm lộ lớp nội mạc, khởi phát trình ngưng kết tiểu cầu hình thành huyết khối Đây chế thường gặp - Cản trở mặt học: Do co thắt động mạch vành - Lấp tắc mặt học: Do tiến triển mảng xơ vữa - Do viêm liên quan đến nhiễm trùng: Sự hoạt hóa thành phần tế bào viêm để gây phản ứng co thắt động mạch vành - Đau thắt ngực không ổn định thứ phát: Do tăng nhu cầu Oxy tim bệnh nhân hẹp động mạch vành sẵn có như: sốt, nhịp tim nhanh, cường giáp…[13] 1.1.2 Yếu tố nguy đau thắt ngực không ổn định 1.1.2.1 Các yếu tố nguy khơng thể thay đổi - Giới tính: Nam giới có nguy bị đau thắt ngực khơng ổn định nhiều nữ giới Tuy nhiên nữ giới thời kỳ mãn kinh, nguy tăng cao sau tuổi 65 nguy mắc bệnh tim mạch nam nữ giới [2] - Tiền sử gia đình: Trong gia đình có cha mẹ anh chị em ruột mắc bệnh lý tim mạch (đặc biệt bệnh lý mạch vành) trước tuổi 55 có nguy mắc bệnh tim mạch nhiều người có tiền sử gia đình bình thường [13] - Tuổi cao: Thành tim dày lên, động mạch cứng lại dẫn đến trình co bóp tim hạn chế nên nguy mắc bệnh mạch vành gia tăng theo tuổi [2] 1.1.2.2 Các yếu tố nguy thay đổi - Thừa cân béo phì - Nghiện thuốc - Ít vận động thể lực - Tăng huyết áp - Rối loạn chuyển hóa Lipid - Đái tháo đường 1.2.2.3 Một số yếu tố nguy khác xác định qua nghiên cứu gần - Ngừng thở ngủ: Mức Oxy máu giảm xuống xảy thời gian ngưng thở ngủ làm căng thẳng hệ tim mạch dẫn đến bệnh lý mạch vành - Protein C phản ứng - Homocysteine: Homocysteine Acid amin để thể sử dụng xây dựng trì mơ Q mức Homocysteine làm tăng nguy bệnh mạch vành - Fibrinogen: Tăng nồng độ Fibrinogen huyết làm tăng kết dính tiểu cầu hình thành cục máu đơng dẫn đến nguy bệnh mạch vành - Lipoprotein (a) [2] 1.1.3 Chẩn đốn đau thắt ngực khơng ổn định 1.1.3.1 Lâm sàng Đau thắt ngực điển hình biểu lâm sàng [6]: - Hồn cảnh xuất hiện: Chủ yếu xảy gắng sức - Tính chất đau: Cảm giác đè ép bóp chặt lấy ngực - Vị trí đau: Đau từ sâu lồng ngực, sau xương ức vùng trước tim - Hướng lan: Xuyên sau bả vai tới cánh tay trái theo mặt cẳng tay tới ngón út, tới cổ, hàm lan xuống thượng vị - Thời gian: Từ 10 – 30 phút Đau chấm dứt giảm nghỉ ngơi sau dùng Nitrat tác dụng nhanh - Tiêu chuẩn phân loại đau thắt ngực điển hình đau thắt ngực khơng điển hình theo ACC/AHA 2007 [18]: + Vị trí đau sau xương ức đau kiểu bóp nghẹt (có lan khơng lan) + Xuất gắng sức + Cơn đau giảm chấm dứt sau ngừng gắng sức hay dùng thuốc nhóm Nitrat Đau thắt ngực điển hình có ba tiêu chuẩn Đau thắt ngực khơng điển hình có hai ba tiêu chuẩn Không phải bệnh động mạch vành khơng có tiêu chuẩn - Trong đau thắt ngực khơng ổn định có hình thái: + Cơn đau thắt ngực xuất nặng từ độ III theo phân độ CCS (Hội tim mạch Canada) trở lên + Đau thắt ngực tăng lên tần xuất và/hoặc thời gian đau (trên sở bệnh nhân có tiền sử đau thắt ngực ổn định, tăng mức theo phân độ CCS tới mức III trở lên) + Đau thắt ngực xảy sau biến cố thủ thuật (sau nhồi máu tim, can thiệp động mạch vành, phẫu thuật bắc cầu nối chủ vành) [13] Bảng nhẹ thường gây đau thắt ngực xảy nghỉ * Nguồn: Theo Anderson J.L cộng (2011) [18] 1.1.3.2 Cận lâm sàng * Điện tim đồ: Điện tim đồ bề mặt 12 chuyển đạo: Trong đau ngực có biến đổi đoạn ST, sóng T sóng Q tạo nên dấu hiệu gọi [19]: + Sóng T âm, nhọn, đối xứng: Thiếu máu thượng tâm mạc + Sóng T dương, cao, nhọn, đối xứng: Thiếu máu nội tâm mạc - Tổn thương: + Đoạn ST chênh lên cong vòm gộp sóng T tạo nên sóng pha tổn thương thượng tâm mạc + Đoạn ST chênh xuống, thẳng đuỗn tổn thương nội tâm mạc - Hoại tử: Xuất sóng Q sâu, rộng Holter điện tim đồ 24 giờ: Theo dõi thay đổi đoạn ST đặc biệt có ý nghĩa trường hợp có rối loạn nhịp kèm [20] * Các dấu ấn điểm sinh học tim - Creatine Kinase: Cho tới gần đây, CK – MB dấu ấn điểm sinh học tim chủ yếu chẩn đoán hội chứng vành cấp [21] - Các dấu ấn sinh học Troponin tim: Bao gồm Troponin T Troponin I đặc hiệu cho tim, có giá trị chẩn đốn phân biệt đau thắt ngực không ổn định nhồi máu tim không ST chênh lên [22] - Myoglobin: Myoglobin tăng khoảng sau bắt đầu có hoại tử tim đào thải nhanh qua thận nên Myoglobin trở bình thường sớm sau đến [23] Cũng Troponin, Myoglobin có giá trị chẩn đốn phân biệt đau thắt ngực khơng ổn định nhồi máu tim không
