RISK MANAGEMENT Dear UW Student: Recognizing the importance of the physical and mental health of our students, the University of Wyoming has developed a two-part Student Health Program: (1) Student Health Services and (2) a student injury and sickness insurance plan (Student Insurance) underwritten by UnitedHealthcare Insurance Company and administered by UnitedHealthcare StudentResources Student Health Service is an outpatient care facility, which is designed to meet the primary health care needs, preventative and therapeutic, of UW students Participating in the Student Insurance is not required to utilize Student Health Service Student Health Service is one of many benefits provided to full-time students Part-time students are charged for the Optional Student Fee Package, which provides them access to Student Health Service Graduate students who are otherwise eligible for the Optional Student Fee Package are eligible to enroll if they desire Graduate students taking less than 4.5 hours may petition to participate in the insurance program by completing an Optional Fee Petition and submitting it to their department and the Office of the Registrar within the first two weeks of the semester The University requires all domestic students to make a Student Insurance selection each semester as part of their registration process in WyoWeb (on-line registration) The student must answer “yes” or “no” to the Student Insurance question A “no” selection will waive participation in the insurance program If the student is eligible for insurance, a “yes” selection will enroll the student in the insurance program and assess the fees associated with participation The student is required to make a selection each semester A summer school selection is not required, but the student may elect to participate in the Student Insurance during summer school if enrolled for at least one (1) credit hour If the student was enrolled during the spring semester and participated in the Student Insurance for that semester, he/she is covered during the summer whether enrolled for the summer term or not If the student did not participate in the program during the spring semester but enrolls for the summer term and wishes to participate in the insurance program, he/she may complete an insurance enrollment form for the summer term electronically or in person by contacting the Student Health Insurance Advocate Brett Selmer at 307-766-3025, stuins@uwyo.edu, or in Room 248, Knight Hall We encourage you to review the attached Student Insurance flyer and the online brochure (www.uhcsr.com/uwyo) for a description of coverage including costs, benefits, exclusions, any reductions and limitations, and the terms under which the coverage may remain in force If you have any questions please contact the Student Health Insurance Advocate Brett Selmer at 307-766-3025, stuins@uwyo.edu, or in Room 248, Knight Hall Sincerely, Risk Management and Insurance 1000 EAST UNIVERSITY AVENUE, DEPT 4300 LARAMIE, WY 82071 FAX: (307)766-3024
