Accidental scald burn b • History of hot liquid spill or flowing hot water that matches burn location/pattern • Irregular margins • May involve head/face, neck, trunk, upper body • If involves lower extremities typically asymmetric distribution • Usually does not involve perineum/genital region in young children Accidental contact burn b • Burn in location consistent with child accidentally touching hot item • Blurred margins/not clearly demarcated • Single burn Bullous impetigo • Honey crusted lesions that vary in size and may spread Phototoxicity/photoallergy • See description in bruise section above Senna-containing laxative • Ingestion of senna-containing laxative blistering dermatitis • Diamond-shaped lesion on buttock with linear borders that line up with diaper edge • Sparing of perianal tissue and gluteal cleft a Includes some but not all of the medical conditions that can mimic cutaneous injuries from abuse Evaluation for additional conditions may be indicated in some cases b The pattern and distribution of bruises and burns can help in identifying lesions associated with an increased likelihood of abuse, but are not diagnostic for abuse and must be interpreted within the context of the history, physical examination, and results of laboratory and radiographic evaluation
