STATE OF NORTH CAROLINA OFFICE OF STATE PERSONNEL POSITION DESCRIPTION FORM APPROVED CLASSIFICATION: _ EFFECTIVE DATE: _ ANALYST: (PD - 102A-92) (This Space for Personnel Dept Use Only Present Classification Title of Position: Physician Extender I Usual Working Title of Position: Physician Extender Requested Classification of Position: Name of Immediate Supervisor: Supervisor's Position Title & Position Number: PHN Supervisor II 522-50-400 Pres.15 Digit Pos No / Prop 15 Digit 522-50-207 Department, University Commission, or Agency: Person County Health Dept Institution & Division: Person County Health Department 10 Section & Unit: Personal Health Clinic 11 Street Address, City and County 355A South Madison Blvd Roxboro, NC 27573 Name Of Employee: 12 I Location of Workplace, Bldg and Room No.: 355A South Madison Blvd Roxboro, NC 27573 A PRIMARY PURPOSE OF ORGANIZATIONAL UNIT: The primary purpose of the Person County Health Department is to provide public health services that allow the promotion of optimal health and well being of the citizens of Person County The focus of service is health promotion and disease prevention B PRIMARY PURPOSE OF POSITION: Participates as a member of the health care team providing direct patient care to the adults, maternity, family planning , and STD Works in conjunction with program coordinators in all programs offered at the Person County Health Department C WORK SCHEDULE: Regular work hours are Monday through Friday, 8:00 am to 4:30 pm with one hour for lunch Assignments could include after hours and/or weekends on occasion A flex schedule may be utilized upon agreement with the PHN Supervisor D CHANGE IN RESPONSIBILITIES OR ORGANIZATIONAL RELATIONSHIP: None II A DESCRIPTION OF RESPONSIBILITIES AND DUTIES: Participates in Public Health Clinic Services days a week Performs a complete physical examination on both pregnant and non-pregnant women including all body systems; identifies any abnormalities and clearly documents findings Orders, interprets, manages routine and non-routine laboratory tests; recognizes implications of abnormal lab results, and orders appropriate plan of care Diagnoses medical conditions, formulates plan of care, and appropriately prescribes, and/or refers for patients’ total health care Manages all methods of contraception including emergency contraception; performs a wide variety of medical acts and renders emergency care if need arises (including CPR, treatment of shock, treatment of anaphylaxis) Manages complete ante partum care of pregnant women independently and renders emergency care if need arises (including CPR, fetal delivery, treatment of shock, treatment of anaphylaxis) Teaches patients and their families about their health needs and how to care for themselves to achieve improved health outcomes Works in conjunction with Maternity Program Coordinator, and serves as back-up, with maternity patient’s plan of care and changes as needed Schedules and case manages referrals for ultrasound, GTT, Rhogam Injection and 17P injection Monitors and follow-up of patients and plan of care as deemed necessary Provides maternal expanded role by assessing, instructing and physician communication for the maternity patient and family Documents accurately and in a timely manner according to agency protocol Coordinates care plan with the Pregnancy Care Management Program as needed Works closely with Pregnancy Care Management in developing and follow through with care planning in the Pregnancy Care Management Program Assists in carrying out care plan of care developed by appropriate Program Coordinators Prepares requisitions for lab work, updates documents on patient records, whether paper or electronic records 10 Assists in follow-up of clients who have had referrals to other Health Care Providers Provides direct patient care and assistance in all clinics as needed Takes histories, teaches patients/parents to understand cause, effect and control of health care Assists medical providers as needed when caring for patient 11 Administers Depo injections, preventive immunizations and antibiotic injections in Family Planning, Maternity, Adult Health, and STD Clinics Administers TB skin tests and accurately reads results Documents appropriately in NCEDDS and in EMR in accordance with policy 12 Reviews, revises and writes program policies 13 Collaborates with Health Education/Promotion in developing individual and community programs ex: Incentive Grants, Health Maintenance Programs Works in conjunction with Health Educator at events as needed 14 Teaches patients to maintain a state of wellness through regular pap exams, breast exams, mammograms, health maintenance exams, and programs Provides nutritional counseling and education 15 Performs Newborn Visits and Postpartum Visits as needed according to Program guidelines 16 Completes daily logs, travel, time sheets, request for leave forms and time studies in a timely manner 17 Serves on various in-house committees as deemed appropriate by PHN Supervisor and/or Health Director 18 Other duties as assigned by PHN Supervisor and/or Health Director II B OTHER POSITION CHARACTERISTICS: Accuracy Required in Work: a Documentation of patient care must be accurate and complete Using equipment must be according to specified procedures Accurate patient assessments are required to assure appropriate plan of care and/or referral Must practice safe/defensive driving Consequence of Error: a Consequence of error in work performance affects the life and health status of patients, loss of public confidence in the agency as a resource for safe, appropriate health education and care Instructions Provided to Employee: a Orientation to health department services, other service agencies in the county Participates in continuing education and staff in-services Reviews forms, records, and patient charts Reviews teaching materials; update as necessary Knowledge of Public Health principles, practice, laws, and ordinances Extensive knowledge of health issues, including women’s health issues related to contraception, reproductive disorders, healthy lifestyles, disease prevention, and primary care health issues Extensive knowledge of principles and practices of Natural Family Planning, Fertility Awareness, and evaluation of Level infertility Knowledge of advanced medical theory, practice, principles, and techniques/skills obtained through education and experience Knowledge of anatomy and physiology; physical assessment principles; techniques for collection of laboratory specimens and principles of interpreting results Knowledge of epidemiology and prevention of disease/illness 10 Knowledge of quality improvement and assurance standards 11 Knowledge of other health and social agencies and medical specialist 12 Knowledge of Current Procedural Terminology, International Classification of Diseases 10th Revision, Health Systems Information Services, and Health Care Finance Administration Common Procedure Coding System Ability to make decisions to comply with mandatory coding for medical services 13 Ability to establish and maintain effective professional relationships with public, co-worker, and other agencies 14 Ability to interpret and explain the value of the service and obtain cooperation from the client and the public 15 Ability to function independently and make accurate decisions based on sound principles of practice 16 Ability to perform physical assessments, perform medical acts, collect a wide variety of laboratory specimens with strict adherence to quality assurance and distinguish normal versus abnormal findings 17 Ability to make diagnoses, formulate plan of care, and appropriate treatment for patient’s medical condition 18 Ability to interpret public health laws and regulations 19 Ability to accurately document and communicate patient feedback from clients to verify understanding of oral instructions;
