Tetralogy of Fallot (TOF) is the most common form of cyanotic congenital heart diseases and can cause complications. Nowadays, complete repaired surgery (CRS) is the optimal treatment for this disease. However, cardiac surgery with cardiopulmonary bypass (CBP) can result in systemic inflammatory response syndrome (SIRS) that may contribute to postoperative complications including multiple organ dysfunction syndrome (MODS). SIRS is considered as a major cause of postoperative morbidity and mortality.
Balance of cytokines is an important factor in the development of SIRS in which the impact of pro-inflammatory interleukin(IL)-6 and anti-inflammatory IL-10 seems to be significant regarding the immunologic status and outcomes.
Amongs anti-inflammatory strategies, glucocorticoids (GCs) including dexamethasone (Dex) and methylprednisolone (MP) are most commonly studied. Despite controversy, the preoperative administration of GCs can attenuate inflammatory response and improve organ function and outcomes.
In fact, patients with TOF after complete repaired surgery still have excessive bleeding, prolonged stay in intensive care unit (ICU) and even death. Therefore, we conducted this study with two objectives as follows:
1. Determine the change of serum IL-6 and IL-10 levels and their relationship with some clinical and laboratory characteristics in patients with TOF after complete repaired surgery.
2. Evaluate the effects of GCs in the phase of systemic inflammatory response (SIR) in patients with TOF after complete repaired surgery.
