Level: Elementary of English Time : 45 minutes

5 Aims

 Topic: filling in a form

 Help Ls know how to fill in a form and some new words and develop

writing skill

 At the end of lesson students can know on what occasions they have

to fill in a form and how to fill in it

6 Exponents

7 Anticipated problems:

 Some words in a form can cause difficulty to Ls

Eg: nationality, occupation…

 Ls may not be familiar with form and language use in form

8 Teaching aids

 Textbook, board, chalks, handout (some forms), pictures

 Picture used in warm-up and handout used in pre-writing and

while-writing

Time Teacher’s activities Learners’ activities 5’ 5’ 5’ I. PRE-WRITING 1.Warm-up

 T uses a picture of a

famous person- Marie Curie and gives handout about her personal information

 T asks Ls to read it and

then asks some questions: Eg: What’s her name? Where was she born? What did she do? …………

2 Introduce the topic: filling in a form

3 T gives a modal form_ CV

 T asks Ls to discuss about

2 questions in Task in pairs

1 On what occasions you have to fill in a form?

2 What sorts of information you often have to provide when you fill in a form?

 T analyses the modal form

o It is used to ask for a job

o It consists of

personal information such as: name, occupation…

o This information is clear and exact

 Ls listen to T and look at

picture and handout

 Ls answer T’s questions

Eg: Marie Curie

She was born in Warsaw She was a physicist ………

 Ls listen

 Ls read it quickly  Ls discuss in pairs and

then some Ls answer these questions Eg: When you asks for a job, open an account in a bank …

Your name, address, date of birth, occupation…

 Ls listen to T ad look at

 T gives some new words

such as:

1 Surname ( n) : họ Fist name: tên

3 Nationality (n): quốc tịch

4 Occupation (n): nghề nghiệp

4 T asks Ls to make questions for some information in CV

Eg: surname, fist name, date of birth…

5 Ask Ls to give their questions T asks Ls to task in

textbook in pairs Then ask some Ls to give answers

7 T asks Ls to work in pairs so that they can ask other’s

information to fill in a CV like the modal form

II While – writing  T asks Ls to write this

information in a form

 T asks Ls to task in

the textbook individually

 Ls listen and note down

 Ls make questions

+ What’s your surname? +When were you born? + What’s your fist name? ………

 Ls read and task in

pairs

Eg: 1-d, 2-f, 3-e……

 Ls work in pairs

Eg:

A: What’s your surname? B: My name is Phạm A: Where were you born? B: I was born in Thai Nguyen
