Bioidentical hormones have a chemical structure similar to the hormones that the human body naturally produces They claim to create similar results to HRT and ERT, and are sometimes touted as more “natural.” The treatment involves individual doses of compounded (made specifically for you at a specialized pharmacy) or pharmaceutical-company-produced medications that come in patches, pills, gels, sublingual tablets, even suppositories Speak with your doctor about what this therapy involves Bummer alert: There’s no evidence that bioidentical hormone therapy is any safer or more effective than traditional HRT FAST FACT Big word but not a big help Plant-derived estrogens are called phytoestrogens They have been marketed as a “safer” alternative to hormones for women with menopausal symptoms The effectiveness of supplements containing phytoestrogens such as red clover, black cohash, or evening primrose oil is questionable, although some women claim anecdotally that they help Women who have had breast cancer should avoid supplemental phytoestrogens Because phytoestrogens are found in so many healthful foods such as soybeans, tofu and other dietary soy, chickpeas, lentils, flaxseed, grains, fruits and vegetables, I recommend making these foods part of your diet instead of taking processed supplements Proof According to the North American Menopause Society, more than 30 percent of women say they use herbal remedies and supplements such as evening primrose oil, black cohosh, and red clover to deal with their symptoms What About Phytoestrogens? H IS FOR HORMONES DURING MENOPAUSE A to Z for Your_001-192_12095.indd 83 A to Z for Your_001-192_12095.indd 83 Text 83 Job: 12095 Title: A to Z for Your V (Fair Wind) 14/2/17 9:19 AM 9:24 AM WSL Page:14/2/17 83 I IS FOR INTERCOURSE ISSUES All the Snafus That Can Happen to You and What You Can Do About Them Proof According to surveys, the Americans and Greeks top the list of intercourse frequency, doing it on average 124 and 117 times each year, respectively The Indians it only about 76 times a year, while the Japanese seem to be the least interested, clocking in at a lowly 36 times annually 84 THE COMPLETE A TO Z FOR YOUR V A to Z for Your_001-192_12095.indd 84 A to Z for Your_001-192_12095.indd 84 Text Job: 12095 Title: A to Z for Your V (Fair Wind) 15/2/17 10:27 AM 9:24 AM WSL Page:14/2/17 84 There are approximately 100 million acts of sexual intercourse around the world each day While sex can be one of the most loving and fun activities you can engage in, that doesn’t mean it always goes well When it’s great, it’s great, and there are countless ways it can be great On the other hand, there are a number of issues that can prevent you from having all the O’s you want The good news is that many of these snafus are totally treatable Rather than beat around the bush, if having sexual intercourse is no longer fun, or was never a bowl of cherries, read on to see what might be going on—and perhaps discover you’re far from alone Vaginal Dryness It can happen at any age, although it’s most common in postmenopausal women Some reasons why we go dry are: • declining estrogen levels • inhibited desire • inadequate foreplay • medications such as antihistamines, antidepressants, and antihypertensives • medical disorders such as diabetes, thyroid disorders, and heart disease Now for the Good News: Dryness Can Be Managed! Here are some tips on how to get wet: • Increase foreplay to give more time for natural lubrication • Hold off penetration until you’re driven mad by yearning • Try OTC vaginal moisturizers and lubricants Daily vaginal moisturizers like Replens don’t contain hormones OTC lubricants to use during sex include K-Y Jelly, Astroglide, and Überlube Follow directions • Give natural lubricants like coconut oil a whirl • Avoid perfumed body or hand lotions, which may be irritating to the vagina • Speak with your gyno about vaginal estrogen, which requires a prescription and may not be right for every woman, but can work very well for the right candidates Vaginal estrogen comes in various forms: creams, rings, and tablets Vagifem, for instance, is a small vaginal estrogen tablet inserted twice weekly There is also Estring, a flexible vaginal ring worn inside the vagina for three months at a time Both products claim minimal to no absorption of estrogen into the bloodstream—and therefore carry little risk of breast cancer and blood clots In fact, minimally absorbed vaginal estrogen has recently been given the goahead as a reasonable solution to painful sex due to dryness even for women with a history of breast cancer if non-hormonal options fail That said, each circumstance is different and very personal Remember, use it or lose it! Sexual activity, including masturbation, may help the vaginal tissues remain elastic and soft and prevent narrowing and discomfort I IS FOR INTERCOURSE ISSUES A to Z for Your_001-192_12095.indd 85 A to Z for Your_001-192_12095.indd 85 Text Proof And How Many of Those Acts Are Awesome? 85 Job: 12095 Title: A to Z for Your V (Fair Wind) 14/2/17 9:19 AM 9:24 AM WSL Page:14/2/17 85 No woman needs intercourse; few women escape it —Andrea Dworkin, Feminist Semen Allergy Sometimes sisters complain about itching, burning, swelling, and redness, and, much less commonly, shortness of breath, rash, and hives that last an hour or so after sexual intercourse On exam there’s no sign of infection, skin condition, or reaction to a particular product In these unusual cases, patients have asked, “Could I be allergic to my partner?” The short answer is yes—and most often women under 40 years old with a family history of major allergic reactions are complaining Talk about an intimacy obstacle! Since symptoms won’t occur if condoms are used, that’s one way to prevent them Another, in severe cases, is medication You could pre-dose with antihistamines The downside? You might fall asleep in the middle of making love So, speak with your doc Vaginismus Otherwise known as genito-pelvic pain/ penetration disorder (GPPPD), this is a tightening and spasming of the muscles at FROM THE NO-MATTER-HOW-YOU-SAYIT FILES: screw, bang, reproduce, copulate, Proof pork, boogie, hump your honey, get laid, lay some pipe, hide the salami, bury the bone, stuff the donut, dine in the Y, munch on the rug, or whack Willy Wonka into Wonderland 86 your V’s opening This involuntary muscle contraction can make it painful, difficult, or virtually impossible to have vaginal intercourse In fact, vaginismus can also make putting in a tampon or a pelvic exam tough going You can get your lady flower to blossom using a vaginal dilator that gradually increases the size, through physical therapy and relaxation exercises, and even by intravaginal Valium Vulvodynia This is an unprovoked stinging, burning, irritation, rawness, or pain anywhere on the vulva that occurs during intercourse, from tactile stimulation, or even spontaneously with no obvious cause Your gyno will diagnose your condition based on your history and a physical exam and by cancelling out all the other usual suspects Treatment is an individual matter It may include application of topical anesthetic jelly ten minutes before sexual intercourse (don’t forget to wipe away the excess prior to sex so that your partner’s penis doesn’t go numb!), meticulous hygiene, hypoallergenic lubricants, physical therapy, Botox injections, antidepressants, analgesics (pain meds), nerve blocks, and of course emotional support and counseling Vulvar ice packs are also soothing after sex You can find more helpful information through the National Vulvodynia Association at www.nva.org THE COMPLETE A TO Z FOR pain felt deep inside during sex might be due to your individual anatomy and can be managed by simply changing positions For example, if your uterus is tilted, missionary position may be uncomfortable but being on top could feel much better However, this type of pain could signal other issues, so discussing it with your gyno is essential It could be one of these conditions: SISTERS WITH V ABNORMALITIES FROM BIRTH or because of trauma or women who have undergone female circumcision may benefit from surgical treatment to enhance the vagina for better sexual function Sexual abuse is associated with chronic pelvic pain and sexual pain disorders If this is true for you, seek counseling and therapy There’s an excellent chance you can be helped PELVIC INFLAMMATORY DISEASE (PID) is an infection of the uterus, fallopian tubes, and nearby pelvic structures that can cause scar tissue and chronic pelvic pain including pain during sex Treatment involves antibiotics and at times surgery OVARIAN CYSTS are diagnosed through medical history, a physical exam, and an ultrasound Treatment may include expectant management (watching and waiting), medication, birth control pills (which can suppress ovarian cysts), or surgery for definitive removal FIBROIDS are benign muscular growths on the Proof uterus and are common They are a frequent cause of discomfort during sex For some women, fibroids are small and no problem Other women hold off on treatment because fibroids usually shrink in menopause In some I IS FOR INTERCOURSE ISSUES A to Z for Your_001-192_12095.indd 87 A to Z for and 77 to 155 calories for a 170-pound man every hour cases, however, fibroids cause unmanageable symptoms including heavy bleeding In these cases, options include surgical removal with myomectomy (removal of fibroids only), hysterectomy (removal of the uterus with fibroids), or embolization, an invasive procedure to diminish the blood supply to the uterus in an effort to shrink fibroids ENDOMETRIOSIS A condition in which cells that typically line the uterus are found on your ovaries, fallopian tubes, and other pelvic structures This tissue then bleeds monthly due to hormonal changes and can cause scarring and pain Diagnosis is made by history (the condition tends to run in families), physical exam, and at times laparoscopy GENITAL PROLAPSE When the cervix, uterus, vagina, bladder, or rectum is bulging inside or protruding from the vagina It can be brought on by childbirth, being overweight, or genetic predisposition While Kegel exercises may help in very mild cases, surgical intervention may be necessary in severe cases A common treatment option for prolapse includes insertion of a pessary, which is a firm rubber device with support, to hold the organs inside Unfortunately, such a device means sexual intercourse is O-U-T Fortunately, a new, totally cool alternative called the Elvie is now available This vaginal device, a sort of Fitbit for the pelvic floor, acts as a personal trainer for your pelvicfloor muscles It even comes with Bluetooth capabilities and a phone app that provides biofeedback so that you can check on your progress Hysterectomy and Sexual Intercourse Proof A hysterectomy, which is the surgical removal of the uterus, does not generally affect sexual function after you’ve healed But here’s a caveat: If your ovaries are removed as well, your estrogen will be depleted and then dryness will occur Some surgeons and patients feel that retaining the cervix during hysterectomy may make for better sexual function and pelvic support Here’s the conundrum: The reason for hysterectomy may impact one’s sex life more than the surgery itself It’s personal Women undergoing surgery for cancer may be depressed, fatigued, and facing further treatment Others are so happy and relieved to have treated heavy bleeding or large fibroids, for example, with a hysterectomy that they feel liberated and actually have a better sex life after to Take a Break from the Birth Control Pill at Least Once a Year Heck no Many women can safely stay on the pill until menopause without a break LIE: Using the Pill May Make It Harder to Get Pregnant in the Future Relax The pill won’t harm your fertility or prevent you from getting pregnant in the future A couple of caveats: If you went on the pill to control irregular cycles, you might still have irregular cycles when you get off the pill For this reason, you might have trouble conceiving because you may have a preexisting ovulation issue Also, if you’re over 35 when you stop taking the pill, your fertility is likely to have declined due to age Eggs only last so long!
