important thing: Your work normally done at high places with heavy loads is generally considered to be dangerous and in danger of fatal accidents However, fatal accidents and injuries can be Wear the specified work clothing Keep it clean and neat all time Fasten hook and buttons correctly • Always wear a hard hat, safety shoes, and a safety belt • Follow instructions on safety sign • Keep work areas, rest areas, and their surroundings well arrange, cleaned, and neat • Keep a first-aid kid at the place where all the workers can recognize • Periodically check if first-aid outfit, such as bandages, tweezers, antiseptic solution is not being short 2.Precautions before starting work • New workers have to take prior training When entering a site, they also have to training for new workers co-workers Repeat the chief installer’s instructions Part 3: Precautions During Work When you act carelessly Accidents may occur in such situations When you make decisions on your own POINTS TO PREVENT ELECTRIC FATAL ACCIDENTS • • • • • • • • • • • • • • Install ground-fault interrupter to prevent an electric shock Prohibit wiring which has contac with steel beams, reinforcing bars, and steel pipe scaffoldings, etc Prohibit wiring on the ground Replace broken switches (cover, etc) Install the earth completely Waterproof measures for outdoor switch Waterproof measures for outdoor equipment 8.Maintain proper wiring height Tape connecting and terminal parts 10 Install a fence for high voltage facilities, indicate “Keep out” and “Danger” sign, and assign the person in charge 11 entrance opening 2-Install a protective cover to traction sheave and governor in machine room 3-Install a 90cm or more high safety handrail with intermediate handrail bar and kick plate to the upper/lower work platforms and install a removable gateway to the area facing to the hall 4-Install the ceiling to prevent any objects flying to upper work platform 5-Install a wire netting or curtain to the area facing to the CWT in the upper/lower work platforms to prevent the contact with CWT 6-Install a red warning lamp in the machine room to indicate that the power for elevator operation in on 7-Install an alarm buzzer in the machine room and car to inform the car operation 8-Install a key switch for the operation so that other workers cannot operation the car without permission 9-Use wire ropes which are cut into the predetermined length and inserted into the rope socket at both ends 10-Install a stopper on the CWT to prevent the CWT block from deflecting from the CWT guide rail 11-To install rail brackets in order with moving the gondola upward, install a contact segment for the overrun stop switch the the guide rail to prevent the bottom guide shoe from moving upward over the installation position of rail bracket is set 12-Install a nameplate to safety handrail in which the cautions such as maximum load, and the prohibition of work in upper/lower worker platforms at the same time are written 13- A chief installer using this method shall be the trained worker who participates in the classroom lecture and practical skill education held by the elevator manufacturer, and shall supervise and administrate the work SAFETY WORK STANDARD FOR ELEVATOR OPERATION • • • • • • • • • 1) Turn off before work Incase of elevators set side by side, check that the power of the appropriate elevato is turn off + Fatal accident case During the cleaning of sheave, a worker was pinched his finger by sudden movement of elevator because he turned off the power for the wrong elevator set side by side There is also a danger of electric shock 2) Do not touch the operating traction machine, governor, hydraulic pump motror, hydraulic V belt ect Fatal accident cases When one worker operated the car in low speed ridden on the car and stopped it, the other woker who cleaned ropes was caught by the sheave with a waste cloth Worker’s fingers ware caught between V belt and pit switch for machine-room-less elevators SAFETY WORK STANDARD FOR ELEVATOR OPERATION • • • • • • • • • • Before break releasing, change the operation mode to manual and turn off the power 6) In case the work is perfomed in machine room and the car seperately, use a comunication device such as intercom to give signals or instruction Check the elevator number, and receiver shall repeat signals signals and instruction Do not operate the elevator until receiver’s repetition or respond 7) After turning off the power, arrgane wires the inside the panel A worker got an eletric shock by touching the charging section during the arrangement of wires inside the panel 8) Do not perform the adjustment of hydraulic pipes during the work in hoisway When the oil leaks from pipes, the car goes down 9) During troubleshooting, not touch the main circuit for at least 30 seconds even if the power is turned off Check voltage by a tester and such before touching it Even of the power is turned off, the main circuit, espacially around inverter, is still charging electricity for a short time 10) In case of power failure, turn off the power after switching to manual mode and wait for the recovery of power supply After recovery of power supply, the car with emergency landing device or under automatic operation starts moving suddenly CAUTIONS FOR THE WORK ON CAR TOP • • • • • • • • 1) Do not assume that the car is always in expected position where the door opens when getting on the car top Fatal accident case When the door was fully opened at one push, a worker fell off by closing a balance Open the door a little and check if the car is on right position when a worker opens the door with the unclocking key Before getting on the car top, turn switch in car operating panel to manual and turn off all the switches on the car top from by keeping the hall door open Turn off the operation switch, door switch, and switch the mode from automatic to manual - Turn the switch in car operating panel to manual - Open the hall door 3) Do not stick out any body part from the handrail during operation After checking that all the body is inside of handrail, operate the car - Fatal accident case A moving without any calls in automatic operation The top clearance of the machine-roomless and hydraulic elevators is short 6) Keep in mind that if car moves upward, CWT moves downward Check the crossover point to prevent a worker from being pinched by CWT 7) Do not operate the car in case a worker on the car top cannot hear the signal from the machine room, pit and inside of the car - In case a worker cannot hear the signal, hypothetical judgment may cause a fatal accident In case he feels any danger, stop the work and restart the work after checking the safer way with other worker 8) For the signals for the low speed operation low speed operation “slow up” “slow down” For high speed operation, use “high speed up” “high speed down” 9) Do not jump to accident car directly In case moving to adjacent car, get out to the hall once and then ghet on the adjacent car A worker was almost to pinched when the car moved suddenly Also there is danger of falling 10) In case of elevators set side by side, not lean over to other elevator side 11) In case of parallel work, adjust the car position so that wokers on the car top can check each other by sight 12)During inspection, if a operate the car 14) In case of long term shutdown, move the car in low speed to the position out of the door zone of the bottom floor, 200~300 mm above the level, turn off all the switch inside the
