Ivy Tech Community College-Lafayette 2019 Chancellor’s Opportunity Scholarship General Information The Ivy Tech Community College-Lafayette Chancellor’s Opportunity Scholarship is a scholarship designed to recognize a student’s personal and scholastic achievements The scholarship amount of $1000 will be applied towards tuition costs and fees Scholarship recipients will be credited with $500 for the fall semester and $500 for the spring semester This scholarship is only valid for the 2019-2020 school year Application Information Applications are now being accepted for the 2019 Chancellor’s Opportunity Scholarship Please work with your high school counselor or appropriate educator (home schooled students) to complete all the application steps Students are required to submit a completed and signed application form, high school transcript, and the two recommendation forms provided in this application in order to be eligible PLEASE SUBMIT ALL ITEMS IN ONE COMPLETE PACKET (Incomplete packets are not accepted) Eligibility All high school seniors that meet the following criteria are eligible to apply: (1) Be a U.S citizen, permanent U.S resident, qualified alien, or have DACA (2) must have at least a 2.0/4.0 high school GPA (3) must have applied and be admitted to Ivy Tech Community College- Lafayette (including instructional sites in Crawfordsville, Frankfort, and Monticello) for the fall 2019 term (4) must plan to enroll full-time (at least 12 credit hours) for the fall and spring semesters of 2019-2020, and (5) must presently attend one of the high schools or be a home schooled senior in the Ivy Tech Lafayette Service Area Application Deadline: April 12, 2019 2019 Chancellor’s Opportunity Scholarship Application Form Deadline Friday, April 12, 2019 (Please make sure to answer all questions and sign your application) Name _ First Middle Last Ivy Tech Student ID# C Address _ City State ZIP Phone _ Email _ High School Graduation Date Ivy Tech Community College Program of Interest Please list and describe any community service or volunteering that you are currently involved with _ Please list any extra-curricular activities or hobbies in which you participate, including any offices held and other personal achievements _ What are your career goals and why you want to attend Ivy Tech Community College?
