2021 Scholarship Program LMH Health Foundation is pleased to offer donor-funded scholarships in 2021 The program will be administered in partnership with LMH Health Human Resources Scholarships available for:  LMH Health employees (General)  LMH Health employees (Nursing)  Children of LMH Health employees  LMH Health Volunteers  Nursing students at o Baker University o Emporia State University o Washburn University Complete applications must be returned to: Human Resources before 4:30pm on Friday, April 30, 2021 Please contact HR with any questions: HumanResources@lmh.org 785-505-3004 Applicants must: • Have a GPA of 3.0 or higher • Submit one application to be considered for all scholarships for which they qualify • Include with their application: o Current school transcript (official transcript is not required) o Personal statement that addresses the following (limit 250 words):  What is your career interest and why?  What impact will this scholarship have on your career goals?  Share and explain how you put patients first  Explain any financial need that you have o Course schedule or proof of acceptance in an accredited program o Letter of recommendation from supervisor (mentor/teacher if not an employee) Must include:  Name of applicant  Date of LMH Health employment (if applicable)  At least one example of how the applicant displays our cultural beliefs (attached)  Supervisor/mentor/teacher signature or email signature and title • Deliver the application in person or electronically to Human Resources by 4:30 p.m on Friday, April 30, 2021 Only complete applications will be considered Scholarship Committee: • Includes: o LMH Health Board of Trustees member o Physician o Nursing leader o LMH Health Foundation representative o HR representative o Other representatives as determined by the LMH Health Foundation and HR senior leaders • Unless otherwise stated on individual scholarships, winners of each scholarship will be based on the application rankings of the scholarship committee • Selection criteria: o GPA o Cultural beliefs incorporated into work/education o Scholastic achievement o Enthusiasm eligibility It is the Peterson family’s intent to provide a scholarship to an LMH Health employee that is Enhance the delivery of nursing care through career development in the nursing passionate career development in the nursing profession and to promote innovation in nursing care at LMH Health Donna Osness served on the LMH Health Board of Trustees for 12 years and also raised five children while working as a nurse and school administrator It is the Osness family’s intent to provide a scholarship to an LMH Health employee that is passionate about the nursing profession and wants to increase his/her ability to serve by furthering their education Promote innovation in nursing care at LMH Health The Naomi Lescher Nursing Naomi Lescher Scholarship fund has been established through LMH Health Foundation in recognition of Nursing Scholarship Naomi Lescher, RN, for her dedicated service to the LMH Health nursing department The ideal candidate is facing the challenges of managing family, their profession and their educational pursuits Must be enrolled in an accredited nursing program, full-time or part-time for Initial program of study, LPN to RN program, RN to BSN, graduate study in nursing or postgraduate study in nursing Recipient chosen by: Peterson family Osness Family Provide a scholarship to a deserving, eligible applicant who is enrolled in a nursing program or has been accepted for enrollment in a school of nursing Must be enrolled in an accredited nursing Committee program, full-time or part-time, for initial program of study, LPN to RN program or RN to BSN Enhance the delivery of nursing care through career development in the nursing profession and to promote innovation in nursing care at LMH Health Must be enrolled in an accredited nursing program, all employees interested in pursuing a meaningful career in the study of oncology and to enhance the delivery of the study of oncology Must be enrolled in an accredited nursing program, full-time or part-time or continuing education Committee seminar, workshop or institute related to the study of oncology KSNA District 17 Nursing Scholarship To enhance the delivery of nursing care through career development in the nursing profession and to promote innovation in nursing care at LMH Health KSNA District 17 joined the East Region of Kansas on January 1, 2016 Members of District 17 voted to disburse remaining funds as a district to form a scholarship for nursing education to promote the profession It is the District 17 members’ intent to provide a scholarship to an LMH Health employee who is passionate about the nursing profession and wants to increase his/her ability to serve by furthering their education Weinberg Nursing Scholarship Enhance the delivery of nursing care through career development in the nursing profession and to promote innovation in nursing care at LMH Health Monti Belot Nursing Scholarship The ideal candidate is facing the challenges of managing family, enrolled in a healthcare-related program or has been accepted and enrolled an accredited academic program Committee Leslie Riner Memorial Nursing Scholarship Leslie Riner was a young, beautiful, inspiring and compassionate woman who passed away too soon Leslie lost her battle with Breast Cancer in 2013 She was 29 years old Her love for life was best seen in the way she cared for her patients as a Registered Nurse Nursing was her passion and her gift Leslie dreamed about being a nurse since she was very young She dressed up in little nursing outfits when she was just a toddler She reached her goal when she graduated from Emporia State Nursing School in 2007 The Meyer Family Scholarship Promote innovation in healthcare at LMH Health The Meyer Family Scholarship fund was established in March 2008, through LMH Health Foundation Leslie touched many hearts and lives during her lifetime and she would be thrilled to know that through the awarding of this scholarship, her spirit lives on Must be enrolled in an accredited nursing program at Emporia State University, Washburn University, or Baker University full-time for Junior/Senior level nursing program, RN to BSN, graduate study in nursing or post-graduate study in nursing Provide two scholarships to the children of LMH Health employees who are pursuing a career in healthcare or have been accepted for enrollment at an accredited school This includes nursing programs, therapy programs, medical records programs, public health and administrative programs Family of Leslie Riner Committee The LMH Health employee must have at least one year of successful employment at LMH Health, and their child must be enrolled in an accredited healthcare program, full-time or part-time The Employee Campaign Scholarship Leona Loney Volunteer Scholarship Established in 2017 through LMH Health Foundation These scholarships are intended for children of LMH Health employees, with preference given to families in financial need This opportunity was coordinated by the 2017 Employee Campaign Committee, led by The LMH Health associate must have at least one year of successful employment at LMH Health, Cory Wapp and Marla Potter The awards are made possible by employee campaign gifts and their child must be enrolled in an accredited school, full-time or part-time to the Employee Education Fund Recognize an outstanding LMH Health volunteer that is continuing education in a healthrelated field and to promote innovation in healthcare at LMH Health Recipient must have at least one year of current successful service to LMH as a volunteer, be enrolled in an accredited program (full-time or part-time) and have a current program of study related to improving the delivery of healthcare Committee Loney Family HR/Foundation use only full-time or part-time; be enrolled in an accredited nursing/healthcare/or related program of study, full time or part time; and have a minimum GPA of 3.0 All scholarship funds will be paid to the
