Elizabeth Peterson, MA, CCC‐SLP Ms. Peterson received her master’s degree in Speech, Language and Hearing Sciences from the University of Colorado‐Boulder. She began working with Dr. Lorraine Ramig’s research team while completing her master’s thesis. She has worked as a research associate at the National Center for Voice and Speech‐Denver and the University of Texas Health Science Center, San Antonio and primarily delivers LSVT LOUD in a research setting. Ms. Peterson is currently involved in Dr. Ramig’s research investigating the short and long‐term impact of LSVT LOUD on neural underpinnings of speech in Parkinson disease • Non‐financial relationships include a preference for LSVT BIG and LSVT LOUD as treatment techniques Disclosures Heather Cianci, PT, MS, GCS Ms. Cianci is the Neurological Team Leader and founding therapist of the Dan Aaron Parkinson’s Rehab Center at Pennsylvania Hospital in Philadelphia, PA. She received her Bachelor of Science in Physical Therapy from the University of Scranton, and her MS in gerontology from Saint Joseph’s University, both in Pennsylvania Heather received her Geriatric Clinical Specialist Certification in 1999 and was certified in LSVT BIG in 2007 She is a 2004 graduate of the Parkinson’s Foundation’s Allied Team Training for PD (ATTP), and now serves as co‐coordinator and PT faculty for the program Ms. Cianci has written and lectured for several national PD organizations, on‐line CEU providers, and PT programs at Philadelphia area universities. Her research is on falls and bed mobility in PD, and she is also a former board member for CurePSP • Financial Relationships include: Ms. Peterson is an employee of LSVT Global and receives lecture honorarium and travel reimbursement. There are no modified versions. After treatment, continue daily dose of LSVT homework exercises! + Treatment fidelity is KEY! If a clinician offers any modified version, don’t do it! You will not receive the benefits of treatment we have established in our 30 years of research. 17 Copyright LSVT Global, Inc. 2020 + = The best combination for success! 18 02/19/2020 Home Routine is Critical • By the end of treatment, clients should have a well‐ established homework routine they can and will continue on their own Homework during Treatment • Importance of homework should be discussed during assessment • If homework routine is not followed, grounds for dismissal 19 • 15‐20 minutes one other time on treatment days • 15‐20 minutes twice a day on non‐treatment days 20 Components of Homework LSVT LOUD You have completed the full course of LSVT LOUD or LSVT BIG LSVT BIG • Daily exercises • Sustained Exercises • Repetitive Stepping and Rocking Exercises • Functional Component Tasks • BIG walking • Carryover Assignments • Daily exercises • Long ahs • High/low ahs • Functional phrases • Speech hierarchy exercises • Carryover Assignments What are your discharge recommendations? Discharge Now What? How do you continue to be successful with the gains you have made in therapy? Homework is NOT optional! Homework is a KEY to long‐term success! Homework makes you feel better and speak/move better! 21 4x per week for 4 weeks 16 total treatment sessions When should you see your clinician again for a tune up? 22 What do data say about maintenance? …Clinically one of the challenges many people with PD face following therapy is how to maintain treatment effects over time and keep exercising • Research data have documented maintenance of outcomes in LSVT LOUD at 6 month, 12 months and 24 months post‐ treatment • • • • No longer regularly attending therapy Disease progression Other medical complications The feeling of “I’m doing well, I don0t need to do this anymore.” • Lack of motivation • Sensory impairment impacting quality of practice (Ramig et al., 1995; 1996, 2001a, 2001b) • BUT… 23 Copyright LSVT Global, Inc. 2020 24 02/19/2020 AFTER LSVT LOUD and LSVT BIG Treatment • Practice at home daily – Homework 1 time per day FOREVER!! • Participate in community‐based classes • Schedule Tune Ups every 3‐6 months 25 While Regular Intensive Exercise is very important, it does NOT replace a “daily dose” of LSVT BIG or LSVT LOUD. Both are needed! 26 • Not all exercise focuses on AMPLITUDE • Amplitude focused exercise needed EVERY day due to hypokinesia and bradykinesia WHY? WHY? • Not all exercise focuses on sensory awareness challenges in people with PD.
