New Employee Checklist Before Day  Complete all Staff Employment Paperwork and bring hardcopies (unless otherwise specified) and proper identification to The Bear Essentials in HR on your first day  Review the Benefits Overview o If you have any questions, bring them to The Bear Essentials to review with our Benefits Specialist  Review the New Employee Resources  Review the Payroll page (prepare questions for your manager)  Review the Time Off and Leaves of Absence page DAY  Bring completed paperwork and identification to The Bear Essentials in HR (Suite 200 in Robinson Tower)  Attend The Bear Essentials from 8:30am-10:00am  Before leaving Robinson Tower: o Finish up paperwork at the HR Front Desk o Activate your Bear ID (may use the HR lobby computers) o Get a parking pass from Parking Services on the first floor of Robinson Tower o Get your ID Card from the ID Card Office on the first floor  Manager or coworker will meet you in HR at 10:00am  Later in the day, complete the Baylor Bowl worksheet o Your manager will give you an hour to complete it Month  Attend Essentials for Success – bring completed Baylor Bowl worksheet  Register for benefits  Review Baylor University Personnel Policies (BUPP) Tools & Resources - following Required Forms Baylor University Personnel Policies (BUPP) Required Forms Click here to download the Staff Employment Paperwork Packet, which includes all the hard copy forms you should complete and bring with you to The Bear Essentials on your first day At the top of the first page, enter your name, Baylor ID number (if you know it, otherwise leave blank), and the date you will begin work at Baylor This will auto-fill these fields throughout the entire packet You may complete the remaining information electronically (the form is a fillable-PDF document) and print the completed packet, or you may print the packet and hand-write the remaining information Note: If you complete the packet electronically, be sure to a “Save As ” and save the file on your computer before closing The file will not retain information entered unless it is saved Below is a list of the forms included in the packet If you’d like more information about these forms, you can click here, call Baylor HR at (254) 710-2000, or email Baylor HR at askHR@baylor.edu  Form I-9 – Electronic so no hardcopy of form necessary Be sure to bring appropriate documentation (listed here) to the HR office – original documents required, copies not accepted  Form W-4 – Social Security Card needed when submitting this document  Personnel Information Form (PIF)  Request for Direct Deposit of Payroll Earnings  Electronic Form W-2  Self-Identification Disclosure  Payroll Deduction Authorization  Copies of unofficial transcripts – required for all positions that require a degree unless you have previously submitted your transcripts or attended Baylor University  Memorandum of Understanding Baylor University Personnel Policies It is recommend that all employees review all of our personnel policies Please read the following policies and continue to the website for additional ones BUPP 023 – Standards of Personal Conduct BUPP 024 – Code of Ethics BUPP 025 – Technology Systems Usage Policy BUPP 028 – Civil Rights Policy BUPP 031 – Sexual Conduct Policy BUPP 037 – Whistleblower Policy BUPP 408 – Family and Medical Leave Act (FMLA) Please review the rest of the personnel policies here
