Only complete applications with attachments will be considered Personal Statement – Please provide a one page, single-spaced, typewritten personal statement Your statement should describe the following: a) Native American family background and/or community involvement b) Student leadership activities and academic involvement c) Honors and awards d) Educational and career goals e) Financial need One fully legible copy of Tribal Enrollment Card or Federal Certificate Degree Indian Blood (CDIB) Tribal enrollment is not necessary for scholarship eligibility One fully legible copy of your most recent official OSU transcript Three letters of reference from a person familiar with your Native American background and community involvement and/or your academic background and involvement Certification All of the information on this form is true and complete to the best of my knowledge I hereby give OSU NAFSA permission to use this information for the purpose of recruitment, public relations, and employment inquiries I further certify that I am enrolled as an OSU student for the current academic year and that I will use the award toward expenses directly related to my education I further understand that the scholarship funds will be deposited into my Bursar's account Signature of Applicant _ Date http://nafsa.okstate.edu/ Continuing Undergraduate Scholarship Application Page of Continuing Undergraduate Student Scholarship Application – Background Information SCHOLARHIP AMOUNT – $1,500 Application Period and Deadline (No exceptions or extensions will be made) • Completed applications (excluding letters of reference) should be scanned and submitted, preferably as a single PDF file, to justin.moss@okstate.edu by PM on April 12, 2013 • Word documents and scanned images will also be accepted, but a single pdf file is preferred • Letters of reference can be sent directly to Dr Justin Quetone Moss at justin.moss@okstate.edu or to the address below • Paper applications, transcripts, and letters of reference can be submitted to: Dr Justin Quetone Moss, Department of Horticulture & Landscape Architecture, 358 Agricultural Hall, Oklahoma State University, Stillwater, OK, 74078 by PM on April 12, 2013 Scholarship Purpose The purpose of the Oklahoma State University - Stillwater Native American Faculty and Staff Association (NAFSA) Scholarship is to assist Native American students and those involved in the Native American community in completing their higher education and to recognize and reward academic performance, honors, leadership, and community involvement Oklahoma State University Native American Faculty & Staff Association Scholarships are available on a competitive basis to undergraduate and graduate students with a Native American background or involvement and who are attending classes on the OSU Stillwater campus Eligibility Requirements The applicant must have a Native American background or Native American community involvement, and be a United States Citizen or a permanent resident of the United States The applicant must be attending Oklahoma State University (Stillwater campus only) as a full-time student and have a minimum 2.5 cumulative GPA A full-time undergraduate student must take a minimum of twelve credit hours Scholarship awardees will be invited to attend the 2013-2014 NAFSA scholarship ceremony at Oklahoma State University Awards and Selection Process The Native American Faculty and Staff Association at Oklahoma State University sponsor the scholarship A review committee will evaluate all applications using the following criteria:  Native American community involvement/contributions  Leadership  Academic achievement  Financial Need  Letters of reference Candidates will be notified of their selection by May 3, 2013 For scholarship awardees, a total of $1,500 will be deposited into their Bursar account in two disbursements of $750 for the Fall 2013 semester and $750 for the Spring 2014 semester Continuation of funding is dependent upon completion of a minimum of 10 total hours of community service each semester including at least hours of Native American cultural community service Community service forms can be found at: http://nafsa.okstate.edu/scholarship.htm Native American Faculty & Staff Association The purpose of our organization is to:  Create an organization, receptive to the needs of all Native Americans at OSU (Stillwater campus only)  Provide an institutional forum, which will improve the education of Native American people  Create a vehicle, which can advise the administration on issues affecting Native Americans at OSU  Provide programs and services, which will assist in the recruitment and retention of Native American students, faculty and staff http://nafsa.okstate.edu/ Continuing Undergraduate Scholarship Application Page of  Support other appropriate minority organizations and services within the university For more information about the Native American Faculty & Staff Association at Oklahoma State University visit: http://nafsa.okstate.edu/ Scholarship History The Native American Faculty and Staff Scholarship was established in 1995 For 2013-2014, up to continuing undergraduates will be awarded $1,500 individually, based on availability of funds http://nafsa.okstate.edu/ Continuing Undergraduate Scholarship Application Page of ...
