HOW TO REQUEST AN AYP SCHOOL CHOICE TRANSFER Applies Only to Students Enrolled at an Eligible Campus! (See list on page 2.) A Title I school that misses Adequate Yearly Progress (AYP) for two years in a row in the same area (i.e., math, reading, attendance rate, graduation rate) enters the Title I School Improvement Program Parents of students attending such a school, also called an AYP school/campus, have the following two options: OPTION – Let the child remain at the AYP school, and assist the faculty and staff in making improvements No transfer application needs to be completed OPTION – Transfer the child to one of the Schools of Choice offered by the district Submit the application in one of two ways: A Submit application through the district’s central office:  Submit an AYP application to the HISD External Funding Department by mail, fax, e-mail or hand delivery by the deadline date (Mailing address: 4400 W 18th Street, Houston, TX 77092; fax: 713-556-6946; e-mail: ext.funding@houstonisd.org)  District staff will verify the child’s eligibility, process the application, and send the parent an AYP acceptance letter in the mail  Upon receipt of the acceptance letter, the parent may withdraw their child from the AYP campus, take the withdrawal paper and the AYP acceptance letter to the new School of Choice and enroll the child The child may remain at the School of Choice until he/she has completed the highest grade  If transportation is needed, the parent should make the request at the time of enrollment  Students must be enrolled in the new school of choice by the deadline date —OR— B Submit application through the child’s new School of Choice:  The parent should complete the AYP application, withdraw their child from the AYP campus, and take the withdrawal papers and the application to the new School of Choice by the deadline date The campus will verify the child’s eligibility, process the application, and send it to the External Funding Department Your child may remain at the School of Choice until he/she has completed the highest grade  If transportation is needed, the parent should make the request at the time of enrollment  Students must be enrolled in the new school of choice by the deadline date NOTE: If a child is not eligible for an AYP school choice transfer and the application is denied, an attempt will be made to contact the parent Students New to HISD If a parent would like to request an AYP transfer for a student who is new to HISD, after enrollment and actual attendance at the AYP campus, the parent should contact the HISD External Funding Department (713-556-6928) for assistance AYP Transportation HISD will provide transportation to the child’s School of Choice if the school exceeds the two-mile radius based on federal guidelines Transportation may take up to two weeks to begin If approved, transportation will only be provided at a selected pick-up location Door-to-door service is NOT available for an AYP School Choice transfer IMPORTANT NOTE: If the AYP campus that your child transferred from exits the School Improvement Program, transportation will cease at the end of the school year in which the school exits the program The district will not provide transportation for your child for the new school year Parents will be notified by mail if the AYP campus could potentially exit the program Magnet Students If your child is in the Magnet program at his/her current school and you decide to transfer your child to an AYP School of Choice, he/she will not have Magnet status at the new school The only way your child will be considered for the Magnet program is through the magnet application process at the new School of Choice Enrollment requirements vary for each Magnet program; therefore, you should contact the School of Choice before making a decision to transfer Special Needs If your child is a student with disabilities or needs specialized language support, please contact your School of Choice before transferring your child to ensure that the school has the services available to meet your child’s special needs 2012–2013 Students enrolled at the following schools are eligible to receive an AYP School Choice transfer Benavidez ES Blackshear ES Bruce ES Davila ES Foerster ES Garcia ES Grissom ES Gross ES Isaacs ES Lewis ES C Martinez ES R Martinez ES McNamara ES Paige ES K Smith ES Attucks MS Black MS Dowling MS Fondren MS Fleming MS Hartman MS Henry MS Hogg MS Holland MS Jackson MS Las Americas MS Long MS Marshall MS Revere MS Ryan MS Wilson ES Woodson ES Sugar Grove MS Thomas MS Williams MS Page 2 of 2    Bellaire HS Jones HS Kashmere HS Madison HS Sharpstown HS Sterling HS Waltirp HS Westbury HS Westside HS Wheatley HS Yates HS ...IMPORTANT NOTE: If the AYP campus that your child transferred from exits the School Improvement Program, transportation will cease at the end of the school year in which the school exits the program... for the new school year Parents will be notified by mail if the AYP campus could potentially exit the program Magnet Students If your child is in the Magnet program at his/her current school and... his/her current school and you decide to transfer your child to an AYP School of Choice, he/she will not have Magnet status at the new school The only way your child will be considered for the Magnet
