Vision: ‘Working to Improve Lives’ JOB DESCRIPTION Job Title: Specialty Doctor in Dementia Intensive Support Service (DISS) in Mid Essex Responsible to: Dr Maneesh Rajendraprasad Accountable to: Clinical Director Professionally Accountable to: Medical Director Hours: Full time (10 PAs per week) Location: Nominal base is Crystal Centre, Chelmsford but required to work at other Trust locations as requested in response to the demands of the service The Trust Essex Partnership University NHS Foundation Trust (EPUT) is an award winning Trust that provides health and social care services for a population of 1.3million people in Essex, Suffolk, Bedfordshire, and Luton CQC says we’re still GOOD and now we’re OUTSTANDING for caring! The trust works with a wide range of partner organisations to deliver care and support to people in their own homes and from a number of hospital and community based premises EPUT has many modern community based resource centres and clinics to provide local services to local people where possible Our services include: Mental Health Services – we provide a wide range of treatment and support to young people, adults and older people experiencing mental illness both as inpatients (including in secure and specialised settings) and within the community A number of our specialist services have achieved accreditation from the Royal College of Psychiatrists Community Health Services – our diverse range of community health services provide support and treatment to both adults and children We deliver this care in community hospitals, health centres, GP surgeries and in our patients’ homes Learning Disabilities Services – we provide crisis support and inpatient services and our community learning disability team’s work in partnership with local councils to provide assessment and support for adults with learning disabilities As part of our a commitment to improving services for people with learning disabilities we are proud to say that we have signed up to the Driving Up Quality Code Our self-assessment contains the full details of our commitment Social Care – We provide personalised social care support to people with a range of needs, including people with learning disabilities or mental illness, supporting people to live independently EPUT is a Foundation Trust This means that local people have greater involvement in the way we develop and run our services It also gives us more flexibility in the way we can manage and invest our finances, allowing us to be more responsive to the needs of local communities The Role of the Specialty Doctor in Psychiatry The specialty doctor will be expected to work independently and provide clinical leadership to the team, through good multi-professional working The specialty doctor will have shared responsibility for the clinical care of those patients referred to the team and will be expected to work closely with the consultant and team manager to ensure the provision of an effective service Duties and Responsibilities of all Specialty Doctors Observe the Trust’s agreed policies and procedures Provide, with consultant colleagues, a service in the specialty for the Trust with responsibility for the prevention, diagnosis and treatment of illness, and the proper functioning of the department Honour out-of-hours responsibilities, including rota commitments where contracted Provide agreed cover for colleague’s periods of leave Participate in audit and continuing medical education Contribute to the training of junior medical staff and medical students Be involved in research and teaching where appropriate Duties and Responsibilities of Postholder Working independently and accountably as part of a team of clinicians offering Mental Health services Working collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a day a week, 24 hours a day service for 365 days a year In delivering your duties you are expected to display behaviours that are aligned with the Trust values and as required in accordance with the GMCs Good Medical Practice at all times The DISS team was commissioned to provide rapid access and short term interventions for patients with suspected or confirmed diagnosis of dementia The DISS team runs a dedicated helpline through which patients and carers can selfrefer in addition to more formal routes such as GP’s and other hospital teams The DISS team operates a days service and working hours are from 9am to 7pm On referral the DISS team will conduct an initial triage to identify the appropriate service that will best meet the patient’s needs We work closely with a range of services in the community such as PROVIDE, Alzheimer’s society, Social services etc to name a few When patients are considered appropriate for the DISS team they will receive a comprehensive initial assessment culminating in an individualised care plan DISS is a multidisciplinary team and has input from a range of professional groups including nurses, occupational therapists, psychologists and doctors Hence a range of interventions can be provided based on the patient’s needs We aim to provide compassionate, individualised and evidence based care for our patients in their usual place of residence Clinical Responsibilities - Conducting home visits, video consultations or telephone assessments with patients as appropriate Supervising assessments conducted by the nursing team when appropriate Attending Mental Health Act assessments for patients under the DISS team when appropriate Attending and actively participating in daily handover meetings Providing medical input into the weekly MDT meetings Liaising with other teams such as GP’s, Radiology, Liaison team etc as appropriate Actively participating in ongoing teaching and training for the DISS team Actively participating in quality improvement and service development initiatives for the DISS team Team Consultant Psychiatrist x WTE Team Manager x WTE Psychologist (Part time) Occupational Therapist (Part time) Band nurses x WTE Band nurses x WTE Band 4’s x WTE Outline Weekly Timetable This will be agreed with the Clinical Director and the Supervising Consultant on appointment DAY Monday Tuesday Wednesday Thursday Friday AM PM Clinical reviews Handover meeting Academic program Clinical reviews Handover meeting Clinical reviews SPA Admin Clinical reviews Clinical reviews MDT meeting Supervision Admin Handover meeting Clinical reviews Handover meeting Urgent reviews Admin Resources You will have the use of a Laptop and have access to the doctor’s rooms and the appropriate secretarial/administration support provided by a team Training, Research and Continuing Professional Development The post holder is expected to participate in continuing professional development Study leave and facilities to enable the post holder to this, up to the standard required by the Royal College of Psychiatrists, will be provided Educational Activities of the Trust The Trust has a policy of encouraging continuing professional development and will meet the Royal College’s requirements for continuing professional development by allowing the post holder appropriate paid study leave and reimbursing reasonable expenses There is a rotational psychiatric training scheme for trainees, which have the full approval of the Royal College of Psychiatrists There is a comprehensive teaching programme consisting of case presentations, journal clubs, seminars, training in interviewing skills and lectures The Trust has an active Clinical Audit programme with presentations of audits arranged on a three monthly basis The Trust is committed to a policy of further education and training for all disciplines This includes links with Essex University - equivalent medical qualification Eligibility Eligible for full registration with the GMC at time of appointment and Section 12 (2) approval under the Mental Health Act 1983 Eligibility to work in the UK Evidence of achievement of Foundation competencies by time of appointment in line with GMC standards in Good Medical Practice including: • • • • • • • good clinical care maintaining good medical practice good relationships and communication with patients good working relationships with colleagues good teaching and training professional behaviour and probity delivery of good acute clinical care Evidence of achievement of ST1/CT1 competences in psychiatry at time of application and likelihood of achieving CT2 competences in psychiatry by the start date of the post Fitness To Practise Applicant’s knowledge is up to date and fit to practise safely Language Skills All applicants to have demonstrable skills in written and spoken English that are adequate to enable effective communication about medical topics with patients and colleagues, which could be demonstrated by one of the following: • • applicants have undertaken undergraduate medical training in English applicants have scores in the academic International English Language Testing System (IELTS) or equivalent equal to those required for recruitment to MMC specialty training programmes If applicants believe that they have adequate communication skills but not fit into one of the examples they need to provide evidence Health Meets professional health requirements (in line with GMC standards in Good Medical Practice) Clinical skills Experience of working with older adults and dementia services is desirable but not essential Relevant specialty clinical knowledge: capacity to apply sound
