Site Supervisor’s Phone E-mail _ General Information about the Internship Project Title Position Title _ Start Date _ End Date Hours per Week _ Estimated Total Hours _ Note: Total hours must be 150 hours or more Learning Objectives On completion of the course, students will be able to: Demonstrate an ability to apply computing principles and technologies relevant to their major in a specific real world project Demonstrate an ability to work effectively in teams in a working environment Demonstrate ethical behavior as a computing professional and an understanding of social, professional and ethical issues related to computing Demonstrate both written and oral communication skills appropriate to the major to convey the tasks and accomplishments related to the internship assignment Display the skills and knowledge to be employable in positions that utilize computing principles and technologies This Internship Learning Agreement is established to provide a basis of understanding between Faculty Sponsor representing Kennesaw State University (KSU), the Student intern, and the Industry Sponsor representing the internship site This agreement commits neither the internship site nor the student to permanent employment The Faculty Sponsor agrees to: Set and grade related academic assignments coordinated with the internship experience Communicate with the internship supervisor throughout the semester to evaluate student’s work performance Communicate with the student and meet with the student during the semester to evaluate the internship experience from the student’s perspective Assess the degree to which student meets stated learning objectives The Student agrees to: Complete the activities, discharge the duties, and produce the deliverables, associated with the learning objectives listed above, following the company rules and policies that apply to all employees Perform assigned tasks in a responsible manner Demonstrate honesty, punctuality, cooperation, courtesy, and a willingness to learn Maintain regular attendance both at KSU and at the internship site Avoid unsafe acts and be alert to unsafe conditions Notify appropriate internship site personnel or KSU Faculty Sponsor of any significant difficulties experienced at the internship site Maintain a weekly journal listing the activities completed, and accomplishments Facilitate obtaining a completed, end-of-semester evaluation form from site supervisor Meet with the faculty sponsor during the semester to discuss the internship experience 10 Complete a report after completing the internship, in which the student reflects on the internship experience 11 Create a poster reflecting the accomplishments from the internship The Industry Sponsor agrees to: Provide an internship experience that permits student to meet her/his learning objectives Provide supervision that, as much as possible, ensures the student’s safety Clarify to permanent employees the expectations for the student’s internship Notify KSU in a timely manner of any serious problems related to the internship, including a need to terminate student’s participation Furnish all necessary supplies and equipment Meet with the KSU Faculty Sponsor at least once during the course of the internship for the purposes of the KSU Faculty Sponsor to evaluate the student’s performance Communicate periodically with KSU Faculty Sponsor regarding student’s work performance Complete and submit a final internship evaluation on a designated form at the end of each semester This evaluation will provide input for the student’s course grade We the undersigned agree to the conditions set forth in this Internship Learning Agreement PRINT NAME SIGNATURE DATE Student Industry Sponsor Faculty Sponsor (Date received with completed information and signatures _) revised 20160918
