Public Health Accreditation Board Local Public Health Department Standards Feedback Form Thank you for participating in the vetting process for the Public Health Accreditation Board (PHAB) standards Your feedback will help PHAB revise and refine the standards that will be proposed for use in the voluntary national accreditation program for local public health departments If possible, we encourage you to provide your feedback online rather than through this form to improve the consistency of comments collected and to allow you to provide more detail in your comments Please visit www.phaboard.org for the online form However, if you are unable to so, please complete this paper-based form and return it to PHAB by April 30 (See below for instructions on returning it.)
