Ung th ph khoa th ng đ c ch n đốn tr , đi u tr h u h t là ph u thu t và th i gian đi u tr các ung th ph khoa kéo dài. Tuy nhiên, m i vi c đã b t đ u thay đ i. Nh ng hi u bi t v sinh lý b nh h c ung th m c đ sinh h c phân t và di truy n h c cho phép phát tri n các chi n l c m i trong t m sốt và ch n đốn s m, và cĩ th s chuy n t đi u tr ung th sang phịng ng a ung th . Báo cáo viên s trình bày v ung th ph khoa nĩi chung và t p trung vào ung th bu ng tr ng.

RECENT ADVANCES IN GYNAECOLOGIC ONCOLOGY

The last 10 years has seen advances in many aspects of gynaecologic cancer prevention, screening, diagnosis, staging, treatment and management. This lecture will highlight issues pertaining to HPV vaccinology, the 2009 revisions in FOGO staging to gynaecologic cancers and 3D image-based treatment planning in cervical cancer brachytherapy.

The advent of the quadrivalent and bivalent HPV vaccines represents the most important advance in cervical cancer prevention in recent decades. However the rapid proliferation of knowledge has led to much confusion among clinicians regarding issues such as the need for further screening, the need for boosters, the role of antibodies and their measurement, disease coverage, the vaccination of sexually active women and safety aspects of both vaccines. These issues will be discussed to clear misconceptions.

Gynaecologic cancers are the only group of tumours that has its own unique staging system developed by FIGO. Following much discussion and debate, FIGO has this year adopted major revisions in the staging of vulvar, cervical, and endometrial cancers as well as of the uterine sarcomas. The new staging systems will be explained and briefly discussed.

A recent advance in cervical cancer treatment involves the use of 3D image-based treatment planning in cervical cancer brachytherapy. Currently available intracavitory applicators often lead to inadequate target coverage and dose inhomogeneity for the lateral tumour parts. MRI-based treatment planning in intracavitory BT enables tailoring of the dose distribution to the target while simultaneously sparing critical structures. Preliminary experience in Vienna demonstrated excellent local control with a low rate of morbidity.

Finally the advent of gynaecologic robot-assisted cancer endoscopic surgery and also the role of genetic, molecular and biological markers, represent very exciting advances which will be covered in detail in separate lectures.
