01633 623935 Index Introduction This is a new post to support the delivery of orthopaedic activity across ABUHB The post will complement the existing surgeons with the Directorate Service Developments in Place and Planned The Directorate of Trauma and Orthopaedics within ABUHB is a forward thinking, proactive Directorate It works collaboratively to deliver the best service for the patients of Blaenau Gwent, Caerphilly, Monmouthshire, Newport, Torfaen and South Powys A number of clinical pathways have been developed to provide a standardised, quality service These include hip pain, knee pain, back pain, fracture neck of femur The consultants work within elective sub specialities as well as providing a generic trauma service Team Structure/Programmed Activities The Directorate comprises 27 Consultant Surgeons (1 locum) Each Consultant sub specialises in a specific field, including arthroplasty, shoulder and elbow, foot and ankle, hand, spines The Consultant teams are supported by Junior Doctors in training, Associate Specialists, Speciality Doctors, Clinical Fellows, Advanced Nurse Practitioners and Surgical Care Practitioners The Directorate supports the development of multi-disciplinary working A number of nurse led clinics in preoperative assessment, Foot and Ankle assessment, and spines have been introduced, as well as physiotherapy extended scope practitioners Key relationships of the service within and outside the organisation The Trauma & Orthopaedic Directorate is split over five sites, Nevill Hall Hospital (Abergavenny), the Royal Gwent Hospital (Newport), Ysbyty Ystrad Fawr (Ystrad Mynach), St Woolos Hospital (Newport) and the Grange University Hospital, Cwmbran Page of 13 The Directorate forms part of the Scheduled Care Division and has two trauma wards at The Grange University Hospital and elective ring fenced wards based at the Royal Gwent The Royal Gwent Hospital has approximately 774 beds, providing a comprehensive range of hospital services for inpatients, day cases and outpatients Likewise, Nevill Hall Hospital has approximately 450 beds and a similar range of hospital services including a DSU The Grange University Hospital provides a trauma service to the whole of the Blaenau Gwent, Caerphilly, Monmouthshire, Newport, Torfaen and South Powys population Access to a wide range of diagnostic and investigative service helps support the work of the Directorate across all sites Good working relationships exist between surgical, medical and diagnostic specialities and many opportunities exist for collaboration the job content will be discussed on a personal basis Page of 13 Job Description The Job Itself Title: Consultant in Trauma & Orthopaedic Surgery With A Special Interest in Spines Professionally responsible to: The Medical Director Managerially accountable to: The Divisional Director, with ultimate accountability to the Chief Executive Responsibility for Staff as Part of this Post: Junior Medical Staff Base: Royal Gwent Hospital, Newport, South Wales Other hospitals at which duties are performed: In view of the fact that the organisation is currently undertaking a review of its services and the locations at which they are undertaken, it is important to be aware that work patterns may change and the place(s) of duties modified Page of 13 The Clinical Strategy of the Health Board is under constant development and the job holder may be required to undertake different duties agreed at the time of appointment Purpose of the Post and General Responsibilities Specific responsibilities of the post are listed in the next section As a senior employee of the Health Board the post holder will work in close co-operation with, and support other clinical, medical professional and managerial colleagues in providing high quality healthcare to the Health Board’s patients Integral to the responsibilities of the post are the following requirements:To ensure the provision of a first class clinical service To provide effective leadership to all staff engaged in the specialty To sustain and develop teaching and research wherever appropriate To undertake all work in accordance with the Health Board’s procedures and operating policies To conduct clinical practice in accordance with contractual requirements and within the parameters of the Division’s and Health Board’s service plans To maintain the confidence of business plans and development strategies formulated for the specialty, the Medical Division or the Health Board Specific Responsibilities of the Post Clinical To provide, with consultant colleagues (as appropriate) a service in the specialty to the hospitals so designated with responsibility for the prevention, diagnosis and treatment of illness Continuing responsibility for the care of patients in your charge in liaison with consultant colleagues and for the proper functioning of the department in line with the Operational Policy and Strategic Plan (as amended from time to time) Professional supervision and management of junior medical staff If appropriate the post holder will be named in the contracts of junior staff as the person responsible for overseeing their training Requirement to participate in medical audit and in continuing medical education in ways which are acceptable to the Health Board Domiciliary consultants, as appropriate, to Gwent residents or as agreed between the two parties The post-holder will be not be on the on-call rota at the present time, but this will be reviewed in the future However they will be expected to see trauma patients during the day including giving advice and treatment All qualified staff have a responsibility to supervise unqualified/more junior staff and students on a day to day basis to ensure that patients receive appropriate care and that any untoward action is addressed All clinical staff are accountable and responsible for their own clinical competence and should limit their actions to those for which they are deemed competent Clinical staff are also required to comply with the requirements of their professional organisations regarding supervision Mentorship for career support and professional development will be given by the colleagues within the Spine subspecialty group within the Directorate as led by Mr Manoj Thomas Teaching/Training (as agreed with Clinical Director) To provide conditions for improved training opportunities in line with national and local recommendations arising out of MMC and related requirements To participate in the undergraduate teaching programme To assist and participate with the development of postgraduate training for F1s, F2s, SpRs, STs and other staff as appropriate Where agreed, to act as Educational Supervisor to junior and middle grade medical staff Audit (as agreed with Clinical Director) To promote evidence-based clinical practice To undertake regular audit Quality and Standards To be familiar with and actively promote adherence to the regulatory framework including NICE and National Service Framework Guidance and to actively promote professional other subsequent patient safety campaign To adhere to the Health Board’s Clinical Incident Policy Personal Development To attend accredited conferences and meetings to update personal level of clinical practice, teaching and management skills in line with CME requirements To participate in an annual Job Planning Review process To participate in the Health Board’s Annual Appraisal process The Health Board will provide support for CPD in accordance of the Royal College Management (as agreed with Clinical Director) To provide medical information for the development of systems appropriate for Health Board needs To participate in departmental consultant and senior staff meetings To attend other departmental, Divisional and Health Board meetings as necessary To attend regional and national meetings as necessary To undertake all work in accordance with Health Board procedures and operating policies To work within the financial and other restraints decided upon by the Health Board Additional expenses of any kind will not be committed without the approval of the appropriate manager/budget holder The post holder has a general duty of care for their own health, safety and wellbeing and that of work colleagues, visitors and patients within the hospital This statutory duty is in addition to any specific risk management or clinical governance accountabilities associated with the post Finally, the post holder is expected to: Observe the rules, policies, procedures and standards of Aneurin Bevan University Health Board together with all relevant statutory and professional obligations Observe and maintain strict confidentiality of personal information relating to patients and staff Be responsible, with management support, for their own personal development and to actively contribute to the development of colleagues Location The principal place of work for this post is currently Royal Gwent Hospital, Newport The post holder will generally be expected to undertake their programmed activities at the principal place of work or other locations agreed in the Job Plan Exceptions will include travelling between work sites and attending official meetings away from the workplace A planned and cost effective approach is expected, and is included in the DCC sessional allocation The post holder may be required to work at any site within the Health Board, including new sites A robust system of medical cover has been put in place across The Grange University Hospital, The Royal Gwent Hospital and St Woolos Hospital to ensure that patients are able to be reviewed and cared for in a safe manner when the consultant is not physically on site Review This job description will be regularly reviewed It is intended as a guide to the general scope of duties third SPA is temporary and will be reviewed three months after the successful candidate is appointed For the first three months the third SPA will cover induction however, after three months the third SPA will be removed if the successful candidate has not taken up an approved SPA role Page of 13 Surgical working requires good team working The role of each Consultant is intended to be as an equal senior to other Consultants in the Department and in the Trust The RCS supports measures to ensure that a new appointee should be appointed on a similar footing to other Consultants within the job plan is subject to the agreement of the Health Board through the Clinical Director and the appointee The consultant will be required to be on-call and work a 1:20 trauma rota The job plan will be a 12 session job plan with a 1:20 on-call rota Internal directorate clinical management duties are not included in the present allocation of SPAs, but additional SPAs (up to a maximum of 3) may be within the Scheduled Care Division, one of 11 Divisions within the Health Board The Trauma and Orthopaedic Directorate co-ordinates, plans and manages sub specialist trauma and orthopaedic services across the Health Board Inpatient services are currently provided in the Royal Gwent Hospital (RGH), Nevill Hall Hospital (NHH), Ysbyty Ystrad Fawr (YYF) and St Woolos Hospital (SWH) In addition to their sub specialist commitments, consultants at these hospitals participate in the general trauma on-call rota and maintain responsibility for patients admitted during their intake until the care of that patient is transferred or discharged Outpatient services: elective orthopaedic clinics for new and follow up patients are located at RGH, NHH, CDMH, County Hospital, YYF, YAB, Chepstow, Monmouth
