3rd Annual Sharing the Experience: Advance Care Planning Conference University of Southern Indiana HP 3123, Health Professions Center Evansville, Indiana Wednesday, September 18, 2019 a.m to 3:35 p.m Registration begins at 7:30 a.m Program Description Speakers This continuing education program will explore the current state of advance care planning in Indiana, including updates to the POST program, ongoing programs across Southern Indiana, discussion of new ideas related to planning and the future of advance care planning in Indiana Kimberly Paul chose to leave her dream job in television and film to discover the authentic stories at the bedside of the dying Working for hospice was supposed to only be a temporary job until she could build relationships in the film industry in Wilmington, North Carolina But this temporary job spread over 17 years as vice president of communications and outreach for a local hospice It never crossed Kimberly’s mind that her true calling would find her and lead to unexpected and bold intentions to become one of the leaders in the death positive movement happening in the United States today She is currently on the Live Well Die Well Tour deathbydesign.com Target Audience Physicians, nurses, social workers, case managers, health facility administrators, clergy, chaplains and other individuals interested in advance care planning Learning Objectives At the completion of this program the participant will be able to: • Identify available resources for advance care planning • Discuss how advance care planning can improve practice For more information about this program and to register go to: USI.edu/health/continuing-education/ fall-programs/#acp Susan Hickman, PhD, is a professor at the Indiana University School of Nursing and Respecting Choices® Advanced Steps® faculty Her research focuses on advance care planning and decision making in the nursing home setting She is the chair of the Indiana Palliative Care and Quality of Life Advisory Council Melinda Samms, LCSW, is a licensed clinical social worker and is the Director of Social Services at the Waters of Princeton Skilled Nursing Facility Previously she provided discharge planning, ER consultation and geriatric counseling at Gibson General Hospital for eight years Melinda has taught at Vincennes University, Ivy Tech Community College and Oakland City University, where she also provided counseling services Her father resides at the Waters and she professes that “their ACP conversation and his advance directive may be the best present he gave me.” Kevin J Valadares, PhD, is a professor of health services and chair of the master of health administration program at the University of Southern Indiana He holds a doctorate in health care ethics from Saint Louis University and is a Respecting Choices® First Steps® and Advanced Steps® instructor Provided by Center for Health Professions Lifelong Learning USI College of Nursing and Health Professions Course Schedule Continuing Education Credit 7:30 – a.m Registration and Breakfast – 8:10 a.m Welcome Kevin Valadares 8:10 – 8:30 a.m Audience participation: Before I die I want to… Kevin Valadares 8:30 – 9:30 a.m Meet in the Middle Kimberly Paul 9:30 – 9:45 a.m Break 9:45 – 10:45 a.m Advance Care Planning Updates Susan Hickman 10:45 – 11:45 a.m Prepare for Your Care and USI’s Geriatric Workforce Enhancement Program 11:45 a.m – 12:30 p.m LUNCH 12:30 – 1:15 p.m iDecide update iDecide at The Waters Kevin Valadares, Melinda Samms 1:15 – 1:30 p.m Break 1:30 – 1:55 p.m Death Deck Discussion Kevin Valadares 1:55 – 2:25 p.m Case Study Kevin Valadares 2:25 – 3:25 p.m Live Well Die Well Kimberly Paul 3:25 – 3:35 p.m Questions and Evaluation Registration Information Cost: $75 if registering by September 1;
