2017 College Road Trip Application Form (Keep this information page; turn in waiver page and permission slip page) Dear Parent or Guardian, During Spring Break your Scholar is invited to a 3-day College Tour visiting USI, IU, and ISU This is an overnight trip where your Scholar will get a personal experience on each of the three college campuses The trip is Monday, March 27th through Wednesday, March 29th The Pick-up and Drop-off location will be at the Southeast parking lot of Arlington High School Arlington High School Address: 4825 N Arlington Avenue Monday, March 27th: 7:30AM – Scholars and Staff will meet at Arlington High School 9:00AM – Bus leaves from Arlington High School College Visits: Day 1: March 27th – University of Southern Indiana Day 2: March 28th – Indiana University Day 3: March 29th – Indiana State University Wednesday, March 29th: 3:45PM – Scholars leave ISU to head back to Indianapolis 5:15PM – Scholars arrive at Arlington High School and will be picked up or taken home on buses Parents should arrive by 5:15PM on Wednesday, March 27th if picking up scholar from Arlington High School The bus will leave promptly at 5:30PM, on Wednesday, March 27th to take any remaining scholars home Additional Details: - Cost of the trip is $30 (which helps cover food costs) -Food will be provided throughout Scholars can bring extra money for snacks and souvenirs - For student drop-off, parents should arrive at Arlington High School between 7:30AM and 8:30AM on Monday March 27th - For student pick-up, parents should arrive at Arlington High School between 5:00PM and 5:15PM on Wednesday March 29th - IMPORTANT: Students MUST bring overnight items and clothes desired A packing checklist will be provided after the final selection of the applicants -There will be a Parent Meeting on Sunday, March 19th from 3:30PM – 5:00PM Location will be announced Please turn in the attached application form and waiver by: Wednesday, March 1st (upper classmen due date) Charles Akinbola Program Director cakinbola@dreamaliveinc.org Office Number: 317-445-9880 Please turn in this application by: Monday, March 1st (upper classmen due date) PLEASE SIGN AND RETURN THIS PAGE PLEASE INCLUDE THE UPDATED DREAM ALIVE WAIVER FOR 2017-2018 PLEASE INCLUDE THE $30 FEE (money will be returned if scholar isn’t selected) I give my scholar permission to attend the College Road Trip with DREAM Alive from 3/27 - 3/29 I am aware of the Parents’ Meeting I am aware that this is an overnight trip I am aware that the cost is $30 (which will be returned to me if he/she isn’t selected to attend) I am aware that my scholar should bring any overnight belongings that they will require I release and hold harmless DREAM Alive, Inc against all liability as a result of my scholar's participation DREAM Alive Scholar’s Name Parent/Guardian’s Signature _ Date Double-check if you have everything below!  This signed application  Your completed updated DREAM Alive Waiver  $30 Fee (will be returned if scholar isn’t selected)
