THE UNIVERSITY OF VERMONT & STATE AGRICULTURAL COLLEGE 3+4 DUAL DEGREE PROGRAM Graduate with a B.S Degree from University of Vermont and a Doctor of Pharmacy degree (Pharm.D.) from Albany College of Pharmacy and Health Sciences (ACPHS) HOW TO APPLY: • Initially apply to UVM, enroll in this program and after three years, students will be admitted into ACPHS as full time students for four years HOW IT WORKS: • • • For the first three years, the “home institution” will be UVM, and the last four years the “home institution” will be ACPHS Students will matriculate at ACPHS once the following have occurred: o Recommendation for continuation in the program by their academic advisor o Have a minimum GPA of 3.0 at the end of their fifth undergraduate semester o Completion of writing assignment o Completion of the PCAT with a minimum score of 20 o Completion of all necessary ACPHS admissions paperwork no later than March 1st of their third year of undergraduate studies Students will receive a B.S degree from UVM once they have completed the degree requirements at UVM and the first year of the Doctor of Pharmacy program at ACPHS Albany College of Pharmacy and Health Sciences Office of Admissions 518-694-7221 admissions@acphs.edu Find the full articulation agreement at: acphs.edu/agreements The University of Vermont & State Agricultural College Office of Admissions 802-656-3370 admissions@uvm.edu THE UNIVERSITY OF VERMONT & STATE AGRICULTURAL COLLEGE PREREQUISITES- YEARS 1-3 OF THE DUAL PROGRAM Below is a list of prerequisites for the Doctor of Pharmacy program at ACPHS and corresponding courses at Castleton University: ACPHS Courses General Biology I General Biology II Biology 200+ level Microbiology General Chemistry I General Chemistry II Organic Chemistry I & II Psychology Statistics Calculus I Physics I Humanities1 Public Speaking2 General Elective3 Liberal Arts Electives Hours 4 3 4 3 3 3 Castleton Courses ACPHS ACPHS E BIO 2125 CHE 1041 CHE 1042 CHE 2111 & 2112 PSY 1012 MAT 2021 ACPHS PHY 1051 & 1052 GenEd GenEd KEY: ACPHS ACPHS prerequisite not required in Castleton University program E One of the many courses from a list to satisfy Castleton University major GenEd Course included in Castleton University General Education Requirements NOTES: English Comp (ENG1061), Touchstones of West Lit (ENG2060) plus one more course Effective Speaking (ENG1070) Any of the PLUS Castleton University courses below Students will also complete the following PLUS courses at Castleton University to fulfill the bachelor's degree requirements: A&P (8), AHS Elective (3), PSY Elective (3), CHI Elective (3), and Intern (1) YEAR OF THE DUAL PROGRAM Students would enroll in the following coursework at ACPHS: Fall Semester Pharmaceutics I Physiology/Pathophysiology I Biochemistry Immunology Pharmacy Skills Lab I IPS Workshop Foundations of Pharmacy Hours 3 1 Spring Semester Hours Pharmaceutics II Physiology/Pathophysiology II Molecular Biology Self Care/OTC Pharmacy Skills Lab II IPS Workshop II YEARS 5-7 OF THE DUAL PROGRAM Students would follow the curriculums of the P2-P4 years of the ACPHS Doctor of Pharmacy program as outlined on WWW.ACPHS.EDU 3 1 ...
