THE FOUNDATION OF THE ISLAMIC CENTER OF GREATER TOLEDO, INC THE YEHIA AND EMNEY SHOUSHER FAMILY SCHOLARSHIP APPLICATION 2010 INFORMATION SHEET PURPOSE To provide two annual scholarships for students pursuing a degree at an accredited northwest Ohio college or university, who will be full-time (as defined by the institution) during the 2010-2011 academic year, and in need of financial assistance Applicants must be U.S citizens To provide two annual scholarships to graduating high school seniors from the Islamic Center of Greater Toledo membership who will be entering an accredited school of higher education in the fall of 2010 It is anticipated that two $1,000 scholarships will be awarded in mid-July 2010 for the 2010-2011 academic year HOW TO APPLY Complete a scholarship application form This includes an extra curricular activities sheet and no more than a one-page essay about your career goals The application form must be signed and dated by a faculty member at your high school, or if you are taking PSO classes, a faculty member at the college or university you attend Obtain a Two signed letters of recommendation – at least one letter must be from a faculty member; b An official copy of your most recent transcript; c Evidence of your FAFSA application Submit these materials to: The Foundation of the Islamic Center of Greater Toledo, Attn.: Amir Khan, PO Box 350, Perrysburg, OH 43551 and POSTMARKED no later than June 30, 2010 Please be sure sufficient postage is on the envelope BASIS FOR SELECTION The criteria used for selection of scholarship recipients include consideration of the following: a Scholastic aptitude and prior academic achievement (GPA OF 2.5 or above), as demonstrated by your transcript; b Financial need; c Individual motivation, ability and potential, as demonstrated by your essay and interview with the Foundation’s Scholarship Committee; d Recommendations of those individuals in 2a above; e Intent to pursue a degree at an northwest Ohio college or university; f Extracurricular activities such as clubs, internships, work/study programs, as demonstrated by the attachment noted on the application form; and g You or your family must have been a member of the Islamic Center of Greater Toledo for the last out of years (2006-2010) Based upon recommendations from the Scholarship Committee, the above-stated criteria may be revised from time to time as deemed appropriate by the Foundation We have read and agree to the information herein Dated: _ Yehia Shousher Emney Shousher
